To serve as a member of the team to provide appropriate and adequate professional nursing care as prescribed by the physician in compliance with applicable RN licensure and agency policies and procedures to those patients who desire to receive hospice care.

DUTIES AND RESPONSIBILITIES
Skilled Nursing Services
Performs physical, psycho-social and spiritual assessment at start of care and establishes plan of treatment in cooperation with remainder of IDG members. Submits paperwork within 24 hours of assessment.
Performs intermittent skilled nursing evaluation on assigned patients. Documentation within 24 hours of evaluation.
Assess and documents current pain level and outcome with every visit. Complete pain assessment minimum of once per week on all patients assigned.
Instructs patient's family and caregivers as appropriate to overcome identified knowledge deficits, documents in notes and home charts.
Reports changes in patient's condition to Team Leader weekly and physician as appropriate.
Obtains physician orders and implements interventions and changes in treatment as appropriate. Documents phone contact that same day.
Monitors medication levels at each visit, insuring the patient always has sufficient quantity, but adjusts levels to coordinate with patients declining status.
Makes appropriate referrals to other disciplines, recognizing need for additional assistance.
Initiates prescribed treatment and evaluate medication effectiveness as designated in the POT within 24 hours of implementation.
Makes visits at the time of death of patient to pronounce death as authorized by the attending physician.
Coordinates minimum of 3 visits per week with SS for all hospitalized patients.
Visits all patients within 24 hours of hospital discharge.
Makes follow up visits within 24 hours of SOC, completing the plan of care and the CNA plan of care.
2. Administration
Provides appropriate documentation at start of care visit within 24 hours of admission.
Revises POT minimum of every 2 weeks, more often PRN with changes in level of care, medication, treatment, and informs all members of IDG of all changes. Insures care plan matches visit notes.
Provides appropriate documentation of intermittent skilled nursing visits and all patient related telephone calls within 24 hours of contact.
Coordinates placement of appropriate paper work on hospital chart for hospitalized patients within 24 hours of hospitalizations.
Provides instruction, leadership, and direction to LVNs as needed.
Provides instruction, leadership, and direction to CNAs as well as supervisory performance evaluations every 14 days.
Clears all chart deficiencies every week with HIMS.
Transcribes orders from physicians and updates medications and records daily. Informing all necessary team members. Documenting all communication.
Updates home medication record with every visit.
Prepares bi-weekly patient care reports. Has a report available 2 days prior to care conference. Documents minimum of 1 phone contact with family every week for all patients in nursing facilities or foster homes.
Complies with agency policies and procedures, mission and values statement.
Attends PCC at nursing facility PRN, informs IDG of outcome.
Provides accurate care status report every Monday to Team Leader.
Revises CNA care plan.
Provides leadership, supervision to personnel when assigned.
Provides shift report daily to Team Leader at assigned time and format.
Executes duties as assigned by the Team Leader.
Covers after hours and weekend call as needed.
Responds to pages in a timely manner.

3. General

PURPOSE OF POSITION
To provide the highest standard of specialized holistic nursing care professional nursing care as prescribed by the physician in compliance with applicable RN licensure and agency policies and procedures to those patients who are admitted to Hospice El Paso Center for Compassionate Care.
DUTIES AND RESPONSIBILITIES
Skilled Nursing Services
Assessing, planning, implementing and evaluation individual care from admission to discharge or death.
Performs physical, psycho-social, and spiritual assessment at start of care and establishes plan of treatment in cooperation with remainder of IDG members. Performs intermittent skilled nursing evaluation on assigned patients, and as needed.
Works with Certified Nursing Assistants to ensure that the highest standard of patient care is provided and will complete CNA duties as needed.
Assess and documents current pain level and intervention outcomes with every contact and PRN.
Educates patients to overcome identified knowledge deficits and documents all education given.
Reports changes in patient's condition to DCO and physician as appropriate.
Obtains physician orders and implements interventions and changes in treatment as appropriate. Documents all communication contact by the end of shift.
Assesses and evaluates medication levels at each shift, ensuring the patient always has sufficient quantity, and adjusts levels to coordinate with patient's condition.
Maintains accuracy in recording, maintenance, and administration of medications in accordance with policy and practice.
Initiates prescribed treatment and evaluated medication effectiveness, as designated within one hour of intervention.
Makes appropriate referrals to other disciplines as needed for family/caregiver support.
Follows Practice Standard for Pronouncement of Death.
Works with and communicates with IDG members for admissions, transfers, live discharges and changes to the POC.

Job Summary: Responsible for building relationships in the contracted healthcare facilities. Identifies potential referrals and facilitates timely admissions in the nursing facility. Responsible for the education and training of healthcare facility staff, especially in the area of hospice care and pain and symptom management. Duties and Responsibilities: A. Relationship Building and Referrals
Visits all contracted nursing facilities on a regular scheduled basis, establishing relationships with the administrative and clinical staff and Hope Hospice patients.
Inquires about any potential hospice referrals in the nursing facility and coordinates those referrals with the hospice admissions department. Assists with assessing and gathering information for hospice evaluations as needed.
Responds to nursing facility staff requests for resolution of problems/concerns.
Notifies Outreach and Clinical Departments of special occasions, awards or upcoming events in the nursing facility.
B. Education and Training
Provides yearly in services on hospice services to all contracted healthcare facilities.
Identifies education needs of healthcare facility staff related to pain and symptom management and end-of-life care issues.
Develops and provides presentations specific to the needs of the healthcare facility and end-of-life care.
Educates hospice staff on healthcare facility's issues/concerns.
Serves as a resource to healthcare facilities, physicians and other community agencies for information regarding end-of-life care.
Provides education on end of life care to Hope Hospice staff, volunteers, and public.
Promote/Help develop clinical disease specific programs. Use clinical knowledge to help provide clinical education.
C. Tracking Systems
Document in Repsly timely individual contact accounts records for existing referral sources and potential new referral sources.
D. Liaison Activities
Works closely with referral sources and staff to monitor client satisfaction and reports problems to the appropriate department for problem resolution and enhancement of customer service.
Serves as Outreach/Marketing staff support at assigned facility activities, community organizations, public relations, special events and/or fundraising activities.
Identifies opportunities for engagement/involvement in the community and facilities.
Responsible for coordinating care and services for patients transitioning between various levels or locations of care - typically from the hospital to home or other healthcare facilities.
Ensures a smooth and efficient transition process by evaluating patients and facilitating timely admissions when appropriate.
E. Internal Networking and Development
Works closely with admission staff, updating them frequently on activities, to ensure smooth transition for new referral sources and to alleviate potential customer service problems.
Keeps abreast of agency's internal programs and services and effectively communicates this information to appropriate supervisory staff.
Assists and supports the Business Development Manager in facility marketing, cultivating new referral activity and providing consultations to families. Also provides back up coverage to BDM as needed.
Assist and support the Admission department and acts as a back up to intake as needed.
Works with internal staff to collect competitive, referral source, and marketplace information.
Works closely with the BDMs to identify education/training opportunities at agencies/facilities and potentially coordinate education events.
F. Documentation
Assures that nursing facility and hospice's plans of care reveal collaboration and coordination of care.
Periodically reviews and updates nursing facility's medical records to assure that all hospice documentation is current and correct.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Listed below is an outline of the major duties and responsibilities that will be required. Administrative authority, responsibility, and accountability necessary to perform the assigned duties under the supervision of the Director of Admissions are delegated to this position.

Obtains all the necessary intake information and physicians start-of-care order.
Enters information into office systems PRN.
Assists in obtaining needed, equipment and supplies to begin care.
Maintains accurate tracking records and log of daily activities.
Reviews H&P, completes clinical parameters.
Schedule pre-admission visit and evaluate every potential patient for admission criteria. Coordinates with Hospice Source and admission counselors.
Involves social services assistance at the earliest possible time.
Coordinates discharge planning with family, social worker, hospital staff and attending physician.
Liaisons with physician and discharge planner to facilitate admission.
Clarifies all medications with pharmacy and attending physician during to admission.
Review medications with patient and family.
Provides extensive teaching with patient/family on symptom management and the disease process to ease anxiety.
Performs admission and submits completed admission paperwork within 24 hours to medical records.
Provides report to Team Leader and Case Manager when admissions is completed.
Informs Admissions Counselor (as scheduled) and/or on-call nurse of admission time.
Maintains close contact with admission office and Director of Admissions in between admission activities.
Performs after-hour admissions as needed.
Follows up on pending and/or non-admit patient referrals as needed.
Visits various hospitals and nursing homes to assure that professionals are aware of the scope of hospice services and have an understanding of admission criteria.
Assumes triage duties when assigned.
Executes other nursing care related duties as assigned by the Director of Admissions.
Shall know and conform to the Texas Nursing Practice Act: Current Board rules, regulations, and standards of professional nursing; and all federal, state and local laws, rules and regulations affecting the RN's current area of Nursing Practice.

Be able to present evidence of 20 contact hours of continuing education if/as requested by the Board of Nurse Examiners with a renewal period-two year period determined licensee's birth month and year.
