Orientation: 2 weeks full time, 8am - 5pm + additional orientation over the next 16 weeks that can more flexible.
JOB SUMMARY:
The social worker facilitates effective referral coordination and information exchange with the Interdisciplinary Team (IDT), prospective and current patients, their caregivers, and referral sources.
Provides explanations about the hospice benefit and organization services in a manner consistent with the patient and family level of understanding.
Plans and coordinates discharge needs for the prospective patient and collaborates with community senior living community staff and hospice teams to facilitate a safe and effective discharge plan from hospital.
Utilizes SW interventions, education, counseling, and support services to address the identified psychosocial needs of patients and caregivers.
ESSENTIAL FUNCTIONS:
Conducts high-risk admission visits in collaboration with the team where psychosocial issues are present, including but not limited to patients living alone, inadequate caregiving/decision-maker situations, and significant mental health/substance abuse concerns.
Initiates, coordinates, and revises the plan of care on an ongoing basis with input from the IDT, patient, caregiver, hospital providers, and staff.
Participates in hospital discharge planning, when needed, collaborating with hospital case management on the plan.
Participates in IDT meetings to report on the plan and update the comprehensive assessment.
Provides education and support to patients and caregivers regarding diagnosis, care, treatments, medications, and end-of-life expectations in a method or manner that they understand.
Provides education and support to hospital staff.
Supports special needs of patients and families including the pinning of Veteran patients.
Communicates effectively with organizational staff, physicians, patients, families, hospital staff, outside contractors, and community agencies.
Anticipates, assesses, and responds to pain and other symptoms of physical, emotional, and spiritual suffering.
Works in collaboration with IDT members, managing the psychosocial care of assigned caseload.
Identifies patient and caregiver stressors and strengths to direct interventions throughout the patient's illness, including initiating post-death bereavement support,
Demonstrates proficiency in navigating systems, knowledge of community resources, advocacy, individual/group counseling, and rapport building,
Understands reimbursement and payer source implications to the plan of care and utilizes resources wisely and effectively.
Assists with the orientation of new staff and visiting professionals.

Assist Clinical Supervisors to develop skills and techniques in evaluating the performance of clinicians
Hire, evaluate and terminate organization personnel
Conduct annual evaluations on clinicians, or more frequently, if indicated
Oversee the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities
Assure proper maintenance of clinical records in compliance with local, state and federal laws
Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services
Develop, implement and evaluate the orientation program for new organization personnel
Responsible for orientation of new organization personnel, directly or delegated
Plan and implement in-service and continuing education programs to meet education and training needs of organization personnel
Assist with the evaluation of organization performance via quality assurance and performance improvement program, productivity, quarterly and annual reviews
Assure the quality and safe delivery of hospice services provided through the organization
Assist in the development of organization goals
Develop, recommend and administer organization policies and procedures
Assure compliance with all hospices related local, state and federal laws, including licensure and certification or organization personnel and maintain compliance to the CHAP Home Care standards

In the absence of the Executive Director, the DPCS will become the acting Executive Director and will be vested with authority to act in behalf of the Executive Director

Requirements:
Must be a registered nurse in the state, with current licensure to practice professional nursing
A bachelor's degree in nursing from an accredited program by the National League for Nursing preferred. Equivalent leadership experience may be substituted for a degree.
California specific: Must have a bachelor's degree in nursing from an accredited program by the National League for Nursing preferred.
Must have one (1) year of management experience, hospice or related health care organization preferred.
Must meet state and federal requirements for the position

Perform personal care needs after given a patient's assignment and attend to his/her requests promptly; if unable to perform a certain task, report to the Case Manager immediately
Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality

Responds to community inquiries and requests for education and support.
Serves as a clinical resource to bereavement volunteers, oversees their orientation and training activities in bereavement programs.
Attends weekly meetings and offers anticipatory grief and bereavement resources, education, and consultation to team members as well as patients and families

QUALIFICATIONS:
Bachelors Degree in a human services related field required
Masters Degree in a human services related field preferred

Summary

Facilitates a smooth transition of patients from inpatient to hoe or to the Hospice Residence.
Meets with targeted facility professionals to acquaint them with the hospice care philosophy and services provided by HCP.
Proposes new initiative to meet the needs of referral sources and improve patient access to end of life and palliative care.
Documents patient needs, interventions, recommendations in compliance with organizational policy and federal and state requirements as well as industry standards.

LPN - Admissions Department
64 hrs. per biweekly pay period
Hours:
Every Saturday, 8am - 8:30pm
Every Sunday, 8am - 8:30pm
+ 2 additional 8 hr. weekday shifts during the pay period
Must be able to complete a 5-6, full time orientation
Essential Functions
Initiates hospice services in a variety of settings, including hospitals, private homes, and senior living facilities.
Follows up on patients awaiting hospice services to assume a smooth hospital discharge.
Performs discipline appropriate assessments on patients per discipline frequencies.
Educates patients and families about hospice philosophy, services, and interdisciplinary team approach and addresses concerns to overcome barriers to admission.
Conducts eligibility evaluation, including evaluation of physical, emotional, and spiritual needs, and gathers relevant medical history and payer information.
Collaborates with the hospice physician to determine patient eligibility for hospice care, seeking additional records or documentation as needed to support a case for hospice eligibility.
Identifies and manages pain and other symptoms, providing nursing care as directed by the hospice physician.
Documents all assessments, interventions, and communications accurately and concisely on the day of service to support prognosis, level of care, plan of care changes, and overall care needs.
Provides ongoing communication and updates with organizational staff, hospice physicians, patients, families, hospital staff including but not limited to care/case management staff and physicians, community palliative care providers, community partners such as skilled or assisted living facility staff and admissions planners.
Serves as a clinical liaison between patients, families, and the hospice care team to ensure a smooth and compassionate start to services.
Conducts remote follow ups for hospitalized patients.
Qualifications
Must have 1 year of experience within the last 4 years required.

This specific role includes a new very flexible orientation. Some of the orientation can be done from home. Will require 4 full days of in person orientation.
Works in collaboration with the Interdisciplinary Team (IDT) providing nursing services, assisting with care coordination, and assessing and addressing the ongoing needs of patients and caregivers. This role is out in the community visiting patients in various settings.
PRN Requirements::
At least 24 hrs./month.

We are a Central Pennsylvania based Non-profit Hospice & Post-Acute Healthcare Organization specializing in home-centered care and we are seeking a talented PRN RN Admissions Nurse.
Orientation: 2 weeks full time, 8am - 5pm + additional orientation over the next 16 weeks that can more flexible.
Job Summary:
The RN Admissions Nurse provides prompt and efficient access and admission of patients referred to Hospice of Central PA. Provides explanations about the Hospice benefit and organization services in a manner consistent with the patient and family level of understanding. In this role, it is necessary to communicate, collaborate and enhance relationships with referral sources, with the overall goal of increasing admissions to the organization.
Qualifications:
RN, with current PA license
Minimum of one year clinical nursing experience-hospice or home care experience helpful but not mandatory
Excellent communication skills [written, verbal, listening]
Ability to track and complete multiple tasks at one time
Ability to work independently; self-motivated
Ability to work as part of a team
