Intake and Admissions Representative
Full -Time
8am - 5pm (might be open to 7am - 4pm)
ESSENTIAL FUNCTIONS:
Delivers great customer service when communicating with patients, families, referral sources and hospice team
members.
Receives and responds to hospice referrals and inquiries via phone, portal, email, and/or fax at a fast pace.
Creates the initial referral record in the electronic health record and starts the intake process through a virtual intake
platform with a high level of accuracy.
Schedules admission visits with patient families with goal of 4-hour response.
Retrieves and uploads pertinent medical records from hospital electronic health record platforms.
Monitors open/pending referrals and conducts and/or assigning necessary and appropriate follow up with a high level
of urgency.
Verifies patient benefit information and obtains and/or assigns initial prior authorizations to Finance/Patient Accounts.
Collaborates with supervisors/leadership regarding high risk and complex referrals.
Ability to work independently as well as part of a team.
QUALIFICATIONS:
High school diploma or GED required.
A minimum of three years' experience in customer service, secretarial or healthcare required.
Excellent communication skills (verbal and written)
Ability to multitask and prioritize in a fast-paced environment with a high level of accuracy.
Ability to work independently; self-motivated
Proficient at basic phone and computer skills
Must be comfortable answering phones with a heavy call volume.

Provide office functions including handling correspondence and word processing for hospice organization, managing incoming calls, filing clinical notes, maintaining administrative and clerical files; and ensuring postage is purchased and kept on hand
Assist BOM in maintaining minutes of meetings
Assist BOM in ensuring all Doctor's order forms and related forms are completed and signed in compliance with Medicare and Medicaid regulations, place forms and applicable documents into patient files
Assist BOM in sending Doctor's orders to Physicians when received from nurses for his/her signature; keep current log of what is sent to make sure it is returned with signature
Assist BOM in keeping all manuals up to date, as assigned
Assist BOM in closing out patient's clinical record when discharged
Enter DARs and log/track their submission, if needed when online systems are down or emergency situations
Assist BOM in accurately entering patient billing data
Assist BOM in accurately processing and billing Medicare, Medicaid, private payer and patient claims in accordance with payor requirements and organization policy
Assist in the preparation of monthly billing and accounts receivable reports, alert appropriate management team members regarding late or missing documents for billing
Establish and maintain positive working relationships with patients, payers, and other customers
Maintain confidentiality of patient and organization information
Maintain adequate inventory of medical supplies/items needed to provide appropriate patient care; notify the supervisor when inventory needs to be restocked
Other duties as assigned
Requirements:
Must have at least one (1) year of experience in health care data entry
Preferred one (1) year of pervious health care related billing experience
Knowledge and Skills needed to be successful in role:
Ability to type 50 words per minute
Proficient word processing skills
Excellent computer skills
Familiar with medical terminology

Perform personal care needs after given a patient's assignment and attend to his/her requests promptly; if unable to perform a certain task, report to the Case Manager immediately
Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality
Meet safety needs of patient and use equipment safely and properly (foot stools, side rails, O2 etc.)
Provide personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned
Assist in dressing and undressing patients, as assigned
Plan and prepare nutritious meals, including shopping, as assigned
Assist in feeding the patient, as assigned
Take and record oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency
Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence
Assess and report on patient's condition and significant changes to the Case Manager; also, being aware of the caregiver or other individuals living with the patient and interpersonal issues
Assist in ambulation and exercise as instructed by the hospice nurse or therapist
Provide normal range of motion and transfers/positioning, as directed
Assist with self-administered medications as allowed by state regulations, such as “reminders”
Offer and assist with bedpan and urinal aid, as assigned
Aid with light laundry needs if necessary
Perform range of motion and other simple procedures as an extensional therapy service, as ordered with appropriate completed/demonstrated skills competency
Provide respite for patient's family/caregiver when on-site, as appropriate
Maintain patient's living area is clean and orderly, as assigned
Adhere to the organization's documentation and care procedures and standards of personal and professional conduct
Participate in quality assessment performance improvement teams and activities
Respect patient and family/caregiver environment and patient's personal needs
All other duties as assigned
Requirements:
Must be a Certified Nursing Assistant (
applicable for all States, additional requirements for California residents, see below).
CALIFORNIA REGULATION: Must be a CNA in good standing AND must have a current Home Health Aide (HHA) certification in good standing in the State of California.
Preferred to have one (1) year experience in community health/hospice or medical/surgical environments
Must have completed a minimum of 75 hours of classroom and supervised practical training
Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
Willingness to submit to a criminal background check prior to hire and annually thereafter
Current CPR certification

Summary
The Team Leader is responsible for coordinating and providing quality patient care based on the Mission and Guiding Principles of the Choices Healthcare. By supporting the philosophy of patient care, the Team Leader provides leadership to the interdisciplinary team (IDT). Education and Experience:
Degree in Nursing with PA licensure

Must demonstrate strong leadership skills and clinical competency
Overview:
Serves as a resource in problem solving, mediating, and negotiating between IDT members.
Acts as a role model in the development of staff.
Supervises employees for performance and provides feedback.
Completes annual employee performance appraisals.
Empowers the IDT to take ownership of their actions and responsibilities.
Submits annual goals to the Director of Clinical Care and encourages nursing staff to participate on organization committees.
Facilitates growth and change in a proactive manner.
Demonstrates professionalism through accountability and confidentiality.
Participates in the interviewing process and provides input to the Director of Clinical Operations regarding potential candidates.
Performs duties in accordance with Standards of Conduct and overall Mission of the organization.
Meets regularly both individually and in a group with the Director of Clinical Operations to assure effective communication and coordination of care provision among all IDT members.
Coordinates patient caseload to provide consistent quality care and visit frequency.
Reviews schedules daily to ensure visit coverage and address specific patient/family issues.
Monitors, evaluates, and promotes collaborative, productive team functioning of all interdisciplinary members.
Directs staff to appropriate resources to enhance delivery of care.
Collaborates with other Patient Care Directors and liaisons to facilitate quality patient care and adequate staffing to meet patient care demands.
Addresses Service Concerns with patients, families and staff in accordance with policy.
Promotes open communication, positive physician/staff interactions and the timely resolution of inter/intradepartmental issues.
Assesses and adjusts staffing to provide for changing patient care needs.
Performs Director of Clinical Operations delegated responsibilities, utilizing the appropriate chain of command.
Assists the Director of Clinical Operations to provide follow-up of events and issues in a timely manner.
Communicates effectively in inter/intradepartmental matters.

Essential Functions:
Performs initial assessment of volunteer skills and interests, supports volunteer throughout the training and entry process, employs good listening skills to connect the volunteer successfully to the organization, and to maintain a healthy connection.
Performs initial assessment of organization's volunteer needs related to specific role and uses critical thinking skills to implement plan to assign and utilize volunteers in meeting these needs.
Maintains accurate records and documentation of volunteer assignments and schedules to meet organization and Medicare reporting requirements.
Serves as liaison between volunteers and organization staff to assure support of volunteers and that organization needs assigned to volunteers are being met.
Develops recruitment and outreach plans to generate interest in the community and engage new volunteers.
Ensures accuracy and completeness of volunteer records, including criminal background checks, TB testing and other required documentation, prior to utilizing volunteer in any assigned role.
Participates in staff meetings, supports other team members, maintaining good communication, and assists with publications to keep volunteers current with programs and organization happenings.
Efficiently manages time and resources in meeting productivity standards and ensuring concise documentation and records.

Qualifications:

JOB SUMMARY:
The social worker facilitates effective referral coordination and information exchange with the Interdisciplinary Team (IDT), prospective and current patients, their caregivers, and referral sources.
Provides explanations about the hospice benefit and organization services in a manner consistent with the patient and family level of understanding.
Plans and coordinates discharge needs for the prospective patient and collaborates with community senior living community staff and hospice teams to facilitate a safe and effective discharge plan from hospital.
Utilizes SW interventions, education, counseling, and support services to address the identified psychosocial needs of patients and caregivers.
ESSENTIAL FUNCTIONS:
Conducts high-risk admission visits in collaboration with the team where psychosocial issues are present, including but not limited to patients living alone, inadequate caregiving/decision-maker situations, and significant mental health/substance abuse concerns.
Initiates, coordinates, and revises the plan of care on an ongoing basis with input from the IDT, patient, caregiver, hospital providers, and staff.
Participates in hospital discharge planning, when needed, collaborating with hospital case management on the plan.
Participates in IDT meetings to report on the plan and update the comprehensive assessment.
Provides education and support to patients and caregivers regarding diagnosis, care, treatments, medications, and end-of-life expectations in a method or manner that they understand.
Provides education and support to hospital staff.
Supports special needs of patients and families including the pinning of Veteran patients.
Communicates effectively with organizational staff, physicians, patients, families, hospital staff, outside contractors, and community agencies.
Anticipates, assesses, and responds to pain and other symptoms of physical, emotional, and spiritual suffering.
Works in collaboration with IDT members, managing the psychosocial care of assigned caseload.
Identifies patient and caregiver stressors and strengths to direct interventions throughout the patient's illness, including initiating post-death bereavement support,
Demonstrates proficiency in navigating systems, knowledge of community resources, advocacy, individual/group counseling, and rapport building,

Provides ongoing communication and updates with organizational staff, hospice physicians, patients, families, hospital staff including but not limited to care/case management staff and physicians, community palliative care providers, community partners such as skilled or assisted living facility staff and admissions planners.
Serves as a clinical liaison between patients, families, and the hospice care team to ensure a smooth and compassionate start to services.
Conducts remote follow ups for hospitalized patients.

Perform personal care needs after given a patient's assignment and attend to his/her requests promptly; if unable to perform a certain task, report to the Case Manager immediately
Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality

Assist in ambulation and exercise as instructed by the hospice nurse or therapist
Provide normal range of motion and transfers/positioning, as directed
Assist with self-administered medications as allowed by state regulations, such as “reminders”
Offer and assist with bedpan and urinal aid, as assigned
Aid with light laundry needs if necessary
Perform range of motion and other simple procedures as an extensional therapy service, as ordered with appropriate completed/demonstrated skills competency
Provide respite for patient's family/caregiver when on-site, as appropriate
Maintain patient's living area is clean and orderly, as assigned
Adhere to the organization's documentation and care procedures and standards of personal and professional conduct
Participate in quality assessment performance improvement teams and activities
Respect patient and family/caregiver environment and patient's personal needs
All other duties as assigned
Requirements:
Must be a Certified Nursing Assistant
Preferred to have one (1) year experience in community health/hospice or medical/surgical environments
Must have completed a minimum of 75 hours of classroom and supervised practical training
Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
Willingness to submit to a criminal background check prior to hire and annually thereafter
Current CPR certification

Keep ED informed of day-to-day activities, insurance case issues and patient care concerns
Work with ED to keep agency in compliance with all regulatory, licensing and accreditation requirements
Actively participate in performance improvement activities as directed by ED
Collect data from clinical records and field staff, for insurance verification to staff treatment plans and services needed by patients
Keep ED informed of day-to-day activities, personal accomplishments, staff concerns and problems
Assist with new hire orientation
Co-facilitate IDT with ED as needed to ensure flow of process and documentation
Monitor census and track homecare admissions, discharges, certification and re- certification dates on all patients and provide this information for IDT to staff providing services to patients
Coordinate OASIS transmission to the state meeting regulatory guidelines
Participate in and assists with community activities, i.e. health fairs, conventions, community education programs, etc. as assigned by the Executive Director
All other duties as assigned
Requirements:
Must have one (1) year hospice experience
Must have two (2) years of admissions experience
Must be 18 years old
PR certification preferred

We are a Central Pennsylvania based Non-profit Hospice & Post-Acute Healthcare Organization specializing in home-centered care and we are seeking a talented PRN RN Admissions Nurse.
Orientation: 2 weeks full time, 8am - 5pm + additional orientation over the next 16 weeks that can more flexible.
Job Summary:
The RN Admissions Nurse provides prompt and efficient access and admission of patients referred to Hospice of Central PA. Provides explanations about the Hospice benefit and organization services in a manner consistent with the patient and family level of understanding. In this role, it is necessary to communicate, collaborate and enhance relationships with referral sources, with the overall goal of increasing admissions to the organization.
Qualifications:
RN, with current PA license
Minimum of one year clinical nursing experience-hospice or home care experience helpful but not mandatory

Functions:

Makes routine visits to facilities to assist in discharge planning, making symptom management recommendations and collaborate with facility staff for the coordination of the plan of care for the patient.
Manages all aspects of care for clients under a General Inpatient Benefit (GIP) while in contracted facilities.
Facilitates a smooth transition of patients from inpatient to hoe or to the Hospice Residence.
Meets with targeted facility professionals to acquaint them with the hospice care philosophy and services provided by HCP.
Proposes new initiative to meet the needs of referral sources and improve patient access to end of life and palliative care.
Documents patient needs, interventions, recommendations in compliance with organizational policy and federal and state requirements as well as industry standards.

Qualifications
On an Average Day You Will:
(includes but not limited to)
Perform personal care needs after given a patient's assignment and attend to his/her requests promptly; if unable to perform a certain task, report to the Case Manager immediately
Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality
Meet safety needs of patient and use equipment safely and properly (foot stools, side rails, O2 etc.)
Provide personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned
Assist in dressing and undressing patients, as assigned
Plan and prepare nutritious meals, including shopping, as assigned
Assist in feeding the patient, as assigned
Take and record oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency
Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence
Assess and report on patient's condition and significant changes to the Case Manager; also, being aware of the caregiver or other individuals living with the patient and interpersonal issues
Assist in ambulation and exercise as instructed by the hospice nurse or therapist
Provide normal range of motion and transfers/positioning, as directed
Assist with self-administered medications as allowed by state regulations, such as “reminders”
Offer and assist with bedpan and urinal aid, as assigned
Aid with light laundry needs if necessary
Perform range of motion and other simple procedures as an extensional therapy service, as ordered with appropriate completed/demonstrated skills competency
Provide respite for patient's family/caregiver when on-site, as appropriate
Maintain patient's living area is clean and orderly, as assigned
Adhere to the organization's documentation and care procedures and standards of personal and professional conduct
Participate in quality assessment performance improvement teams and activities
Respect patient and family/caregiver environment and patient's personal needs
All other duties as assigned
Requirements:
Must be a Certified Nursing Assistant
Preferred to have one (1) year experience in community health/hospice or medical/surgical environments
Must have completed a minimum of 75 hours of classroom and supervised practical training
Licensed driver with automobile that is insured in accordance with organization/state requirements and is in good working order
Willingness to submit to a criminal background check prior to hire and annually thereafter
Current CPR certification

Seeking an experienced RN to work day shift on our Admissions team.
