The RN Admission Care Liaison Essential Duties Are:
* Assess and evaluates the patient and family including the physical, spiritual, and psychosocial needs, whether residing in hospital, nursing facilities (ECF, AL, IL, etc.), home and other settings.
* Presents hospice philosophy and services offered to referred patients and families in a manner that results in timely admission of eligible patients.
* Develops and implements the initial patient-centered plan of care with Interdisciplinary Team members.
* Communicates clearly all patient information to the Hospice Physician to obtain certification of terminal illness.
* Assists with patient assessments for facility based contracted beds, manages symptoms, collaborates with interdisciplinary team members, and facilitates transfers to home or other facilities as requested.
* Communicates with attending physicians and hospital staff to establish a coordinated plan of care that reflects the hospice philosophy for new patients and existing patients that are in the hospital; which will ensure that transfers to other care sites are expedited as soon as possible.
* Assists in other activities and departments when requested to ensure complete and thorough patient care for all Ohio's Hospice patients.
* Participates in Continuous Quality Improvement activities to support the quality of Ohio's Hospice services.
* Precepts new staff to support professional relationships with newly hired team members as required.
RN Qualifications:
* Graduate of an accredited nursing school
* RN nurse license in the state of Ohio without any board actions
* Two years acute care nursing experience preferred
* Certification in Hospice and Palliative Medicine (CHPN) preferred and will be made available to Ohio's Hospice employees who wish to obtain certification.

Job Summary/Purpose
The Travel RN carries the responsibility of flexibility in diverse Ohio's Hospice RN roles as a float team member assisting Care Management, Extended-Hours/On-Call and the Admission Departments, Home Health and IPU as needed. The Travel RN staff will collaborate with various IDT members both internal and external of the organization. When in the field, staff will provide excellent clinical care to all patients that meet the needs of our patients/families standards, Medicare Conditions of Participation and achieve the desired outcome of comfortable and peaceful death for our patients.
Expected shifts can vary and may include days, evenings, nights, weekends and holidays. You will be trained to work in all departments - inpatient unit, admissions, case management, home care and extended hours
This is an hourly position eligible for differentials and overtime, 36-40 hours/week.

* Collaborates with other PHC and Ohio's Hospice divisions as appropriate to improve communication of needs; if to outpatient support center, coordinates plan of care for follow up and instructions on AVS; if hospice care is desired, initiates referral process.
* Coordinates plan of care between settings (home, hospital/inpatient, outpatient services, etc) to ensure continuity of care and ability to obtain necessary medical treatment, equipment, and physical/emotional needs.
* Documents assessment, plan and interactions with individual, family, community resources, hospital or facility staff, and physicians and establishes/builds account in electronic medical record.
* Participates in the development of processes to improve the clinical experience for referral sources, individuals/families, care management teams, etc. across the continuum of care.
* Regularly attends IDT meetings or other multidisciplinary team meetings based on Individual/family needs or organizational needs.
* Fosters collaborative working relationships within division, organization, hospitals, and community facilities and/or with groups with whom we are engaged.
* Attendance at division meetings 80%; accountability for 100% of information provided via minutes.
* Annual education completed by deadline to ensure staff has the information and tools to complete their job satisfactorily and meet regulatory compliance.

What You Should Know About the RN Travel Foat Role:
* The RN Float position will cover ANY area of need across Ohio's Hospice 3 regions of coverage (South/Central/North)
* This is a Full-Time position with 12 week assignments and the hours could vary between day shift or nights based on the assigned coverage need.
* We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed
Job Summary/Purpose
The Travel RN carries the responsibility of flexibility in diverse Ohio's Hospice RN roles as a float team member assisting Care Management, Extended-Hours/On-Call and the Admission Departments, Home Health and IPU as needed. The Travel RN staff will collaborate with various IDT members both internal and external of the organization. When in the field, staff will provide excellent clinical care to all patients that meet the needs of our patients/families standards, Medicare Conditions of Participation and achieve the desired outcome of comfortable and peaceful death for our patients.
Expected shifts can vary and may include days, evenings, nights, weekends and holidays. You will be trained to work in all departments - inpatient unit, admissions, case management, home care and extended hours
This is an hourly position eligible for differentials and overtime, 36-40 hours/week.
Benefits:
* Mileage reimbursement - 0.70 cents per mile
* Weekly travel stipend to cover lodging

Qualifications:
* Registered Nurse, State of Ohio with no board actions
* CPR certification card
* Valid driver's license with safe driving record
* State minimum auto coverage
