The Personal Care Specialist's Essential Duties Are:
Our STNAs are titled "Personal Care Specialist" because they are valued for their expertise in taking care of the patient's personal needs with dignity, compassion and empathy. You could become a Personal Care Specialist that provides quality nursing assistance to patients, assists the family with the care of the patient and works with a team that relies on your input from the close relationship you develop with the patient. The Unit Coordinator position would be responsible for Hospice House communications between staff and patients, and facilitation of the admission and transfer process for patients entering and leaving the Hospice House and maintaining medical records
Qualifications:
* High school diploma or GED;
* Certified State Tested Nursing Assistant with the State of Ohio without any board actions;
* one year of experience preferred;
* Certification in Hospice & Palliative Medicine (CHPNA) preferred, required within two years of employment;

The Media Graphic Specialist Essential Duties Are:
To promote the brand of Ohio's Hospice and its affiliates by building awareness though the creation of online, print and social media graphics that are published to internal and external audiences. This role is responsible for developing static and interactive graphics related to website, micro-sites, the intranet as well as comprehensive print collateral materials. This person will be responsible for designing, placing and styling content on the websites, blogs, internal kiosk display information and as needed, on social media channels; will assist with internal and external communications efforts including creating graphics and layouts, internet and intranet posts, and printed materials and projects. This role will be an integral part of the communications team and will additionally work with multiple internal clients on creative requests and projects.
Qualifications:
* BA or comparable demonstrable experience in web or graphic design, content development, website management or related communications or graphic fields
* Technical competency including experience in Adobe Creative Suite (InDesign, Photoshop, Illustrator, and/or additional other appropriate software products)
* Must be able to multi-task in a very fast paced environment with the proven ability to meet or exceed project deadlines

This is a wonderful opportunity to experience working at multiple affiliates across Ohio's Hospice, while serving the same mission of providing superior care and superior services to each patient and family. This pool of RN's will be placed at an affiliate based upon needs and on 12 week assignments.
Job Summary/Purpose
The Travel RN carries the responsibility of flexibility in diverse Ohio's Hospice RN roles as a float team member assisting Care Management, Extended-Hours/On-Call and the Admission Departments, Home Health and IPU as needed. The Travel RN staff will collaborate with various IDT members both internal and external of the organization. When in the field, staff will provide excellent clinical care to all patients that meet the needs of our patients/families standards, Medicare Conditions of Participation and achieve the desired outcome of comfortable and peaceful death for our patients.
Schedule:
Expected shifts can vary and may include days, evenings, nights, weekends and holidays
You will be trained to work in all departments - inpatient unit, admissions, case management, home care and extended hours
This is an hourly position eligible for differentials and overtime, 36-40 hours/week
This position will travel within Ohio's Hospice regions of coverage to include the South Region, Central Region and the North region.

The Care Manager plans and delivers care to patients utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation. You will effectively interact with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing.
Qualifications:
* Graduate of an accredited nursing school; Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred
* Active Registered Nurse license in the State of Ohio without any board actions.
* Two years acute care nursing experience preferred
* Certification in Hospice & Palliative Medicine (CHPN) preferred; required within two years of employment. (Eligible upon completion of 500 hours in last 12 months of hospice/palliative experience.)
* CPR Certified
* Computer skills sufficient to properly document services and care.
* Ability to drive during daytime, nighttime or inclement weather.
* Valid Driver's License with Safe Driving Record
* State Minimum Automobile Insurance Coverage
