    What You Should Know About the Social Worker Role: * The Social Worker would be for coverage under Ohio's Hospice South regions * This is a full-time position. M-F, 8A-5P with after hours/weekend on call * We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed. The Social Worker's Essential Duties are: We provide superior care for patients in families in homes/facilities. The Social Worker will utilize their advanced educational background, their clinical expertise, judgment and discretion, to help identify the patient and family's unique cultural/spiritual beliefs, coping skills, pre-bereavement and anticipatory grief needs to offer a wide range of services to terminally-ill patients and families. Qualifications: * Licensed Social Worker with the state of Ohio LISW preferred or LSW. * Master's Degree in Social Work or related degree as required for LSW licensure. * One year social worker experience in a health care setting preferred. * previous discharge planning experience * Computer skills sufficient to properly document services and care. * Able to drive during daytime, nighttime or inclement weather. * Valid Driver's License with Safe Driving Record. * State Minimum Automobile Insurance coverage. * Notary designation or intention to complete designation within six months of hire. Provide supportive counseling and crisis intervention to support patients facing emotional, social, or financial challenges. Advocate for patient access to necessary services, including housing, financial assistance, and mental health support. Maintain detailed documentation of patient interactions, care plans, and progress in compliance with regulatory standards. Collaborate with interdisciplinary teams, including the RN CM, to coordinate care and ensure continuity of services. Educate patients and families about available resources and coping strategies to enhance their health outcomes. Assesses for depression and anxiety using the PHQ-9 screening tool and the GAD screening tool as indicated and refers to Master's level social worker when appropriate. Participate in supervisory case consultation, staff meetings, and professional development activities to stay current with best practices. 5 days a week.
