The RegisteredNurse (RN) Case Manager provides skilled nursing care for hospice patients in their home or living facility. The RegisteredNurse (RN) Case Manager assesses the needs of the patient/family unit and plans, implements, and evaluates the nursing care in accordance with physician orders and the interdisciplinary plan of care.
Responsibilities of the RegisteredNurse (RN) Case Manager include:
Completing nursing assessments and updating plans of care
Providing direct hospice nursing care to patients in order to maintain their highest level of comfort and quality of life, and regularly assessing for nursing care needs
Initiating appropriate interventions and monitoring the outcomes
Educating patients/family members
Coordinating hospice nursing care when outside agency involvement is required
Ensuring necessary medical equipment, supplies, and services are available
Completing Home Health Aide assignments as needed and making supervisory visits to instruct, supervise, and evaluate
Performing on-call coverage as needed/scheduled
Qualifications for the RegisteredNurse (RN) Case Manager include:
Graduate of an accredited school of nursing and current RegisteredNurse (RN) licensure in the State of Michigan
Two years of recent clinical experience is preferred, however recent nursing school graduates are also encouraged to apply. Homecare or hospice experience desired
Sound clinical judgment
Knowledge and/or experience in pain symptom management with end of life care

NOP is a 24-bed specialty med/surg unit. This unit sees a variety of neuro and ortho patients for scheduled procedures and trauma events as well as pediatric patients and is focused on providing quality acute care while promoting safety. The team is experienced and supportive of each other and often complimented by patients for their collaborative teamwork.
How you will contribute
A RegisteredNurse (RN) who excels in this role:
· Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
· Documents patient care given.
· Monitors, records, and communicates patient condition as appropriate to care team, physician, patient, and family.
· Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
· Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral, and spiritual/psychosocial support needs.
· Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.

Cardiac department has 36 beds. Patient population and diagnosis: Cardiac and CVA patients. Post op day 1-2 CABG, pacemaker patients, Bradycardia, Tachycardia, SVT, AFIB w/RVR or new a fib, acute MIs, PCI patients, Fem/Fem bypass, CEAs and more. We also do step down (IMCU) patients. Nurses are exposed to more critical patients, making it a good bridge to departments such as ED and ICU.

Job Description:

3. Facilitates shared decision making among staff. Facilitates unit throughput on given shift to improve LOS (Length of Stay)
4. Coordinates/provides in-service programs to assist staff to maintain or enhance their competence in fulfilling job responsibilities based on identified needs of patient care personnel.
5. May assist manager with interview, selection, retention, mentoring, and evaluation of staff.
6. Participates in development of policies, procedures, and standards for the department.
7. Assists with department Process Improvement and Peer Review.
1. Associates Degree/Diploma required. BSN preferred.

Job Summary
Under general supervision of the manager for the specialty area, participates as a member of a multidisciplinary team to provide consultative nursing services within a clinical area of specialization. Serves as a patient advocate and provides emotional support and home care instructions for patient and family; provides ongoing staff and community education and supports the implementation of outreach/outpatient programs.

PRINCIPAL RESPONSIBILITIES AND DUTIES
CLINIC OPERATIONS:
Manages the operations of the clinic, including costs, processes, staffing, and quality standards.
Provides leadership, coaching, and development plans for all direct reports.
Partners with internal Human Resources, Quality, and Technical Services departments.
Collaborates with or functions as the Home Therapies Program Manager to oversee the facility's Home Therapies Program.
Maintains integrity of medical and operations records and complies with all data collections and auditing activities.
Accountable for completion of the Annual Standing Order Review and Internal Classification of Disease (ICD) coding.
Manages clinic financials including efficient utilization of supplies or equipment and regular profits and loss review.
Responsible for all required network reporting and on-site state or federal surveys.
PATIENT CARE:
Coordinates all aspects of patient care with the appropriate staff members, from admission through discharge of the patient.
Ensure education of the patient regarding quality measures, transplant options, modality awareness, and access care.
Acts as a resource for the patient and family to address concerns and questions.
Accountable for timely completion of patient care assessments and care plans.
Manages timely patient schedules to ensure facility efficiency and develops action plans for missed treatments.
Plans, coordinates, and validates patient eligibility for treatment.

STAFF:

PHYSICIANS:

EDUCATION AND REQUIRED CREDENTIALS:
Bachelor's Degree or an equivalent combination of education and experience.
Graduate of an accredited School of Nursing (RN).
Current appropriate state licensure.
EXPERIENCE AND SKILLS:
Required:
6+ years business operations experience in a healthcare facility.
12 months experience in clinical nursing.
6 months chronic or acute dialysis nursing experience.
Successfully pass the Ishihara Color Blind Test.
Preferred but not required:
3+ years supervisory or project/program management experience.

RN/LPN Responsibilities
Complete ongoing assessments of the patient's physical status and the psycho-social status of the patient/family
Initiate and develop a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family, and the attending physician
Provides education and interventions to support the effective management of the patient's pain and other physical symptoms as well as assesses the patient/family's response to intervention and treatment
Ensures that equipment, supplies, drugs and biologicals, and other necessary items and services are available
Make necessary revisions to the IDG plan of care as the patient's/family's status and needs change
Completes accurate and timely documentation of patient/family services provided
Regularly attends IDG meetings
RN/LPN Requirements
RN/LPN with active, unrestricted license in the State of Michigan
At least one (1) year of relevant experience as an RN or LPN (required)
One (1) year of management experience (preferred)
One (1) year of home health experience (preferred)
Current CPR Certification
Community health and a wide range of Medical-Surgical experience (preferred)
Reliable transportation with a valid state driver's license and current car insurance
