This is a part-time, hourly, non-exempt position Schedule: Saturday and Sunday 7am-7:30pm
This position is based in the Hospice IPU and is responsible for the administrative and clerical duties related to the operations of the IPU. This includes but not limited to, patient medical records, customer service and other regulatory options.
What are the essential duties day to day?
Provide administrative support for the IPU by performing general clerical and administrative duties, such as word processing, data entry, answering telephones, filing and operating office machines.
Oversee medical supply usage and ordering
Provides general administrative assistance to the Director of the IPU.
Completes documents and reports as assigned. This includes but not limited to:
Weekly IDT meeting agenda
Preparing Chart Audit Reports for VP of Quality and Compliance
FTF reports
Assists with payroll by new staff members into time clock, updating time cards in the payroll system according to the Time Edit Log.
Assist with training new volunteers. Provide direction and monitoring of work to ensure accuracy and completeness of duties assigned.
Work in collaboration with referral/admissions department to ensure data entry availability of patient information 7 days/week.
Provides assistance in developing, organizing, maintaining and updating the patient medical record. This includes opening the chart upon admission, ensuring all required documents are present, according to applicable regulations and agency policy and procedures, and closing the patient's chart at time of death or discharge.

Following death of assigned hospice patient makes appropriate bereavement communication and referrals per agency standards and facilitates role closure with primary caregiver/family.
Acts as an advocate for the rights of those with life-limiting illnesses by initiating and facilitating discussion of advanced care planning when appropriate.
Consult with other social workers regarding client-related issues when needed.
Participate in staff training and education programs.
Job requirements:
Master's degree (MSW) from an accredited school of social work
Licensed by the NC Social Work Certification and Licensure Board as a Licensed Clinical Social Worker preferred
Minimum of one year healthcare experience

Essential Functions
Conducts the admission of the patient to the Agency by: obtaining a medical history, to include Oasis when
needed, particularly as it relates to the present condition, from the patient and / or family member (s); performing a physical assessment of the patient; assessing the patient's support systems to determine what assistance will be available from family members and their ability to meet the patient's immediate needs upon admission and throughout care; assessing the patient, family member (s) and home situation to determine what health teaching will be required; assessing the patient's condition and home situation to determine if other home health services are needed; interpreting nursing and other services of the Agency to patient's and families as a part of planning for care.
Develops and implements the patient care plan in cooperation with other Agency professionals and
paraprofessionals who will be providing care to the patient in MatrixCare.
Manages assigned cases and provides skilled nursing care as outlined on the physician's plan of treatment
and in accordance with the Agency's policies and procedures.
Observes signs and symptoms and reports to the physician and other applicable personnel, reactions to
treatments, including drugs and changes in the patient's physical or emotional condition and outcomes of intervention. Ensures on-going evaluation of patient / family response to care.
Adequately educates, teaches, supervises, consults, and counsels the patient / family members regarding
the nursing care needs, other related problems of the patient at home, the disease process, self-care techniques, end-of-life care, and processes for dealing with issues of ethical concern. Also provides specialized hospice training to other staff, family members and informal caregivers to ensure adequate care.
Initiates appropriate preventative and rehabilitative nursing procedures, as appropriate, for the patient's
care and safety.
Ensure that care is provided by the Home Health Nursing Assistant by providing the Nursing Assistant
with a written plan of care and specific duties to be performed, by supervising and evaluating the care given at least once every two weeks, and by participating in periodic conferences with the Director of Clinical Practice or Assistant Director of Clinical Practice concerning the Nursing Assistant's performance.
Appropriately consults with the attending or on-call physician concerning alteration of the plan of treatment
and documents, in writing, appropriate change of orders where necessary.
Communicates with supervisor concerning problems of the patient(s) on a timely basis and communicates
information using current process and technology available to the organization.
Turns in all paperwork to the designated person in the office by 5pm on Monday, and syncs ipdad nightly when visits are done. Makes a verbal report of new admissions and ER visits to the primary nurse Sunday night or Monday morning between 8:00-9:00 a.m.
Collaborates with supervisor to meet assigned productivity standards
Keeps up-to-date on reimbursement criteria and charting needs for all patients under his/her care and
supervision.

Conducts a complete assessment of the patient to identify appropriate care needs: (a) Assesses, reports, and documents patient's status and progress on appropriate Agency laptop and to the health care team; (b) ensures initial and on-going re-assessments to determine level of functioning and evaluation of patient/family response to therapy, including OASIS assessments at appropriate time points. (c) Identifies the problem and performs physical therapeutic treatments as prescribed by the patient's physician, (d) develops and revises a plan of treatment and care as indicated in collaboration and consultation with the physician, ITC members, and other health professionals, (e) develops and instructs Agency staff and family members in restorative and or maintenance therapy programs and therapeutic treatments as approved by the physician. (f) creates Home Health Aide care plans as needed.
5. Prepares and maintains appropriate clinical and administrative records in a prompt and comprehensive manner, documenting patient limitations/interventions and progress. Timely records all assessment and evaluation data, treatments and patient's response to therapy intervention. Completes paperwork within 24 hours of visit, and communicates daily. Responds to e-mail requests for documentation correction/completion within 48 hours.
6. Provides regular supervision to Physical Therapy Assistant at a minimum of 30 days and a certified nursing assistant at a minimum of 14 days as appropriate in carrying out the plan of treatment as established by the nurse or therapist. Ensures supervisory visit is competed per agency policy and procedure, including following PT plan of care and ensuring skilled services delivered each visit.
7. Provides an example for correct ethical/legal activities. Works as team player to coordinate visits within payer source time points.
8. Maintains regular communication with supervisor: (a) communicates problems of the patient(s) on a timely basis following chain of command, (b) utilizes current process and technology available to 3HC, (c) collaborates with supervisor to meet assigned productivity standard, and (d) ensures appropriate referrals are evaluated timely with a NPU rate of less than 6%.

Functions:

3. Provides skilled nursing care as outlined in the physician's plan of treatment, according to 3HC's policies and procedures and as allowed by the Practice Act of the N.C. State Board of Nursing; observes and documents signs and symptoms and reports to the physician and other applicable personnel, reactions to treatments, including drugs and changes in the patient's physical or emotional condition and outcomes of intervention; ensures on-going evaluation of patient/family needs and response to care; refers to other services as needed; initiates appropriate preventative and rehabilitative nursing procedures, as appropriate, for the patient's care and safety; educates, supervises, consults and counsels the patient/family regarding the nursing care needs, other related problems of the patient at home, the disease process, self-care techniques, and processes for dealing with issues of ethical concern and assures the development and implementation of the patient individualized plan of care and updates as needed.
4. Home Health Patients: Conducts a complete assessment of the patient to identify appropriate care needs: (a) obtains a medical history to include OASIS when needed, particularly as it relates to the present condition; (b) performs a physical assessment; (c) assesses the support system to determine what assistance will be available from family members and their ability to meet the patient's immediate needs upon changes throughout care; (d) assesses the patient, family member(s) and home situation to determine what health teaching will be required; (e) assess the patient's condition and home situation to determine if other home health services are need; (f) interprets nursing and other services of the agency to the patient and family as a part of planning for care; and, (g) plans for discharge from services.
5. Hospice Patients: Provides comfort to patient/family to facilitate quality of life and death with dignity: (a) provides skilled nursing care as outlined in the physician's plan of care, according to 3HC's policies and procedures and as allowed by the Practice Act of the N.C. State Board of Nursing; (b) responsible for completing hospice admissions, educating patients and families on the hospice concept and the hospice benefit. (c) Plans, coordinates, provides and directs the delivery of compassionate, quality patient care. (d) Actively participates in the coordination of all aspects of the patient's hospice care, in accordance with current professional standards and practice, including participating in on-going interdisciplinary comprehensive assessments, developing and evaluating the plan of care, and contributing to patient counseling and education. (e) Provides continuity of care to all hospice patients. (f) Assist the interdisciplinary team in the development and implementation of the patient/family care plan (g) Refers hospice patients to other services as needed. (h) Provides end of life care as outlined in plan of care and policy and procedures. Provides ongoing assessment of medicinal, DME needs.
6. Documents in the medical record electronically and downloads daily: (a) those services rendered to the patient; (b) conferences held with other disciplines, physician, or community resources; (c) changes noted in the patient's condition and/or family home situation; (d) revisions in the nursing care plan as needed; (e) evaluation of patient care and clinical and progress notes that demonstrate progress towards established goals. Download will include time and travel documentation. Responds within 48 hours to requests for documentation correction/completion.
7. Ensures the physician and/or Hospice Medical Director is involved ongoing with the patient's care: (a) reports to the physician and/or Hospice Medical Director and other applicable personnel, reactions to treatments, including drugs, and change in the patient's physical or emotional condition and outcomes of intervention, and (b) consults with the attending physician and/or Hospice Medical Director concerning alterations of the plan of treatment and documents appropriate change of orders where necessary. Verifies that physician listed on order/plan of treatment will sign orders.
8. Ensures that 3HC can bill for services appropriately: (a) completes certifications and re-certifications within designated timeframes; and (b) obtains verbal orders from Hospital, MD and attending within 48 hours of SOC; and (c) keeps up-to-date on reimbursement criteria and documentation requirements for all patients.
9. Completes home health and hospice admission visits and resumption of care visits during hours scheduled to work.
10. Covers territory assigned by supervisor and conducts on call visits during hours scheduled to work. Triages all patient calls timely and professionally according to patient acuity.
11. Notify attending physicians, staff members, and family members of patient deaths when they occur.
12. Collaborates with Procare Pharmacy regarding medication changes, administration, and recommendations as needed.
13. Maintains regular communication with supervisor. Communicates problems of the patient(s) on a timely basis following chain of command. Communicates information using current process and technology available to 3HC. Coordinates home health and hospice care with Director of Clinical Practice/Assistant Director of Clinical Practice, Director of Home Hospice Services and the interdisciplinary team.
14. Stays abreast of overall patient care and improves nursing care skills: (a) participates in office staff meetings, patient care conferences, record audits, utilization reviews, quality improvement activities, and in-services; (b) attends and participates in interdisciplinary team conferences as needed; (c) attends agency mandatory in-services and satisfies CEU requirements of the State Board of Nursing; and (d) strives to improve nursing care through continuing education, active participation in professional and related organizations and individual research and readings.
15. Demonstrates a willingness to be cost effective in the use of agency resources, the monitoring of waste, and the proper and safe use of supplies and equipment.
16. Adheres to 3HC's Personnel Policy and performs other appropriate duties as assigned by supervisor to promote the successful operations and future growth of 3HC.

Summary:

Licensed Practical Nurse (LPN) Essential Job Functions:

Board of Nursing; (b) observes signs and symptoms and reports to the RN, reactions to treatments, including drugs and changes in the patient's physical or emotional condition; (c) adequately reinforces teaching completed by the RN and counsels the patient and family members regarding the nursing care needs, preventive and rehabilitative nursing procedure as appropriate for the patient's care and safety, as assigned by RN; (d) assist the RN or physician in performing specialized procedures and duties, (e) assist patient's family and other health care team members in providing continuity of care and meeting the emotional needs of the patient and immediate family members , and (f) reports outcome of any unscheduled visits to the RN within 24 hours of the provision of service.
3. Documents in the medical record electronically and downloads patient information in NetSmart periodically throughout the day: (a) those services rendered to the patient; (b) clinical and progress notes, conferences held with the physician and other disciplines, physician, or community resources; (c) changes noted in the patient's condition and/or family home situation; (d) revisions in the care plan with supervision by the RN (e) and submits downloads all paperwork including time and travel daily (f) Updates medication profile with each medicinal change. (g) Keeps up-to-date on reimbursement criteria, hospice criteria, PPS, and charting needs for all patients under care including private authorization. Responds within 48 hours to requests for documentation correction/completion.
5. Collaborates with supervisor to meet agency's clinical needs. Rotates weekend, after hours, and holiday on-call: with an RN on back up call. Assists with coverage of visits in other offices as assigned.
6. Maintains regular communication with supervisor. Communicates problems of the patient(s) on a timely basis following chain of command. Communicates information using current process and technology available to 3HC.
7. Stays abreast of overall patient care and improves nursing care skills: (a) participates in office staff meetings, patient care conferences, record audits, utilization reviews, quality improvement activities, and in-services; (b) attends and participates in interdisciplinary team conferences/ IDG at least 75% of the time; (c) attends agency mandatory in-services and satisfies CEU requirements of the State Board of Nursing; and (d) strives to improve nursing care through continuing education, active participation in professional and related organizations and individual research and readings
