If you meet these qualifications, we want to meet you!
Master's Degree from accredited school of Social Work or related field; Valid license/registration of permit to practice in state(s) of agency operation if required
Medicare/Medicaid/Insurance specialty preferred
Unless otherwise dictated by the state, at least one (1) year of professional social work experience (post-graduate) in a healthcare setting required; 2+ years preferred
Experience or education in gief counseling preferred.
Discharge planning experience preferred.
Responsibilities:
As a Hospice Clinical SocialWorker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical socialworker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

Perform the duties required of a registered nurse at all times
Care for patients with pulmonary HTN, respiratory failure, COPD, CHF, and A-Fib
Use computer equipment to monitor cardiac and other vital information
Start IV's, draw blood, see the patient from start to finish

Acts as a facilitator and/or advocate for the member in dealing with issues with providers, community programs or other organizations.
Acts as a gatekeeper to ensure that the member is receiving the most appropriate, cost-effective services in the most appropriate setting.
Facility based while remaining within budgetary allowances. Internal customers: all levels of nursing management and staff, medical staff, and all other members of the interdisciplinary healthcare team. Minimum Qualifications

Responsibilities
* The Bereavement Coordinator is a qualified professional who supervises the provision of bereavement services reflective of patient and family needs. Demonstrates proficiency in providing bereavement services to persons of all races, ages, genders, sexual orientation, social class, spiritual beliefs, and mental/emotional status as evidenced by observation and EGSS scores.
* Establishes a Plan of Care that addresses bereavement needs with clear delineation of services to be provided and frequency of service delivery up to thirteen (13) months following the death of the patient.
* Demonstrates active listening and impactful suggestions to bereaved during phone and home visits within LHC-required time guidelines as evidenced by observation, Visit Notes and EGSS scores.
* Leads and coordinates at least 12 effective sessions of Grief Support Group per calendar year as evidenced by observation and EGSS scores.
* Maintains Bereavement Binder with information and attendance sheets from Memorial Services, Grief Support Groups, Holiday events, and other Hospice and Community groups and presentations.
* Demonstrates individualized and compassionate care for each bereaved as evidenced by documentation in Visit Notes, Bereavement Risk Assessment, Care Plan, and EGSS scores.
Education and Experience
Education Desired
* Bachelor's or Master's degree in Social Work, Psychology, Mental Health counseling, Psychiatric Nursing, Pastoral Counseling or the documented equivalent in education, training or experience per state regulations.
Experience Requirements
* Experience in grief and loss counseling.
Skill Requirements
* Excellent writing and presentation
* Current CPR certification
* Current driver's license, valid vehicle insurance and access to a dependable vehicle, or public transportation.

What You'll Do as a Hospice Chaplain:
+ Provide spiritual and emotional support to patients, families, and caregivers in alignment with their beliefs and values.
+ Offer non-denominational, inclusive care that respects all faiths, spiritual traditions, and cultural backgrounds.
+ Serve as an integral member of the interdisciplinary team, participating in patient care conferences and collaborating with nurses, socialworkers, aides, and medical staff.
+ Assess spiritual needs of patients and families and develop individualized spiritual care plans.
+ Conduct visits, prayers, blessings, and rituals as appropriate and requested by the patient or family.
+ Assist families during times of grief, loss, or transition, and provide bereavement support and counseling.
+ Build relationships with local faith leaders and community resources to support patients' and families' unique spiritual needs.
+ Maintain timely and accurate documentation of spiritual assessments, interventions, and outcomes.
About You
The candidate must meet one of the following educational requirements:
+ Bachelor's degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)
+ A minimum of one unit of ACPE-accredited Clinical Pastoral Education (CPE)
Note: Degrees from accredited institutions are preferred. If a degree is from a non-accredited college or university, AVPO approval is required prior to hire.

Qualifications

Strong interpersonal skills with the ability to provide non-judgmental, inclusive spiritual care.
