Provide personal care under the supervision of a nurse according to the patient's plan of care. Collaborate with transdisciplinary team in a variety of settings.
WHAT YOU WILL DO:
Provide personal care and assist with ADL's according to the POC.
Provide safe environment for patients.
Safely ambulate/transfer patient as appropriate.
Assist with passive and active ROM exercises as directed by the nurse.
Prepare nutritional snacks as appropriate for the patient.
Under the supervision of the nurse, assist patient with pre-poured medications that are ordinarily self-administered and that are prescribed by a physician under the supervision nurse (Home Care only).
Under the direction of the nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.
Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.
Attend and actively contribute during patient care conference.
Participate in HNA discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate.
Attend in-services to obtain 3 education hours per quarter.
Participate in the on-call rotation as scheduled (Home Care only).
Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.)
Demonstrate positive customer-service responsiveness according to agency standards.
Flexible in meeting patient/family needs that require variable shifts and weekends.
Participate in chart review quality improvement activities and other meetings as assigned.
Participate in coordination of and communication with volunteer staff.
Provide basic care-giving education to caregiver/family members, such as turning and positioning, transfer techniques and changing linens
Maintain Transmission Based Precaution.
Complete annual competency requirements.
SUCCESS CAPABILITIES:
Must be able to read and write English proficiently.
Must be state tested nursing assistant in good standing
Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.

The role
The Hospice Aide provides personal care and related services in the home for the sick or disabled, including assisting the patient to perform on 2 or more activities of daily living (ADL's.) As a Hospice Aide you will:
Key Responsibilities:
Provide assistance with at least 2 of the following ADL's including personal hygiene
Assist patients with medication intake and prescribed exercises
Incidental household services, which are essential to patient health care
Will follow the Plan of Care (Instruction Sheet) as instructed and make appropriate updates during scheduled visits
Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Skills and Experience Required:
Must be a certified nurse's aide (CNA). State Tested Nursing Assistant (STNA) for Ohio
Must maintain a valid driver's license, good driving record and reliable automobile
High school graduates or GED preferred
Demonstrate the ability to be able to work within a multi-disciplinary team setting
Demonstrate the ability to work in field situations

The Hospice Aide provides personal care and related services in the home for the sick or disabled, including assisting the patient to perform on 2 or more activities of daily living (ADL's.) As a Hospice Aide you will:
Key Responsibilities:
Provide assistance with at least 2 of the following ADL's including personal hygiene
Assist patients with medication intake and prescribed exercises
Incidental household services, which are essential to patient health care
Will follow the Plan of Care (Instruction Sheet) as instructed and make appropriate updates during scheduled visits
Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Skills and Experience Required:
Must be a certified nurse's aide (CNA). State Tested Nursing Assistant (STNA) for Ohio
Must maintain a valid driver's license, good driving record and reliable automobile
High school graduates or GED preferred

Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.
Attend and actively contribute during patient care conference.
Participate in HNA discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate.
Attend in-services to obtain 3 education hours per quarter.
Participate in the on-call rotation as scheduled (Home Care only).
Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.)
Demonstrate positive customer-service responsiveness according to agency standards.
Flexible in meeting patient/family needs that require variable shifts and weekends.

Participate in coordination of and communication with volunteer staff.
Provide basic care-giving education to caregiver/family members, such as turning and positioning, transfer techniques and changing linens

Provide personal care under the supervision of a nurse according to the patient's plan of care. Collaborate with transdisciplinary team in a variety of settings.
WHAT YOU WILL DO:
Provide personal care and assist with ADL's according to the POC.
Provide safe environment for patients.
Safely ambulate/transfer patient as appropriate.
Assist with passive and active ROM exercises as directed by the nurse.
Prepare nutritional snacks as appropriate for the patient.
Under the supervision of the nurse, assist patient with pre-poured medications that are ordinarily self-administered and that are prescribed by a physician under the supervision nurse (Home Care only).
Under the direction of the nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.
Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.
Attend and actively contribute during patient care conference.
Participate in HNA discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate.
Attend in-services to obtain 3 education hours per quarter.
Participate in the on-call rotation as scheduled (Home Care only).
Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.)
Demonstrate positive customer-service responsiveness according to agency standards.
Flexible in meeting patient/family needs that require variable shifts and weekends.
Participate in chart review quality improvement activities and other meetings as assigned.
Participate in coordination of and communication with volunteer staff.
Provide basic care-giving education to caregiver/family members, such as turning and positioning, transfer techniques and changing linens
Maintain Transmission Based Precaution.
Complete annual competency requirements.
SUCCESS CAPABILITIES:
Must be able to read and write English proficiently.
Must be state tested nursing assistant in good standing
Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.

CONDITIONS OF EMPLOYMENT:
Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
Provide an active driver's license
Ability to provide proof of eligibility to work in the United States.
