The Chief Clinical Officer will be responsible for general oversight of clinical services and operations according to established policy and procedures. Responsible to plan develop, organize, implement and evaluate clinical, practices, and operations. Responsible to create, communicate and implement the clinical practices vision in alignment with the strategic plan. Develops and implements clinical programs, procedures and processes in a systematically standardized approach throughout the organization. Develops the annual budget for areas of responsibility. General oversight of the daily management within the allocated budget. Assists members of the leadership team with projects as assigned. WHAT YOU WILL DO: Articulate a clear definition of the organization's strategy and communicate it to clinical staff. Remain apprised of change initiatives needed to carry out strategy. Engender commitment to strategic plans. Ensure that decisions at all levels align with strategic objectives. Tract multiple projects assigned. Assist with the preparation of grants or proposals as assigned. Analyze information at a conceptual level and interpret to recommend action to the Management Team and/or Chief Operating Officer. Develop and present materials for management education or input. Plan and assist with staff retreats, director leadership development and team building. Succession Planning for all clinical team leaders and Directors on continuous cycle. Maintain current knowledge of health care, environment and industry trends and advise the management team and/or Chief Operating Officer. Participate in National Hospice Forums or workgroups to gain insight into best practices and new innovations. Participate in Pharmacy utilization oversight and relationships. This includes analyzing costs per day, trends in use of non-formulary meds. Serves as the clinical liaison to DeltaRx pharmacy. Ensure that daily operations and actual practice conform to requirements as outlined in policies and procedures. Cooperate with any investigations of privacy violations/quality issues. Assist in fostering awareness of the importance of protecting patient privacy and developing an organizational culture committed to the protection of health information. MANAGEMENT: Administers, directs, and coordinates activities in clinical operations, including hospice and palliative care programs. Responsible for general oversight of all clinical teams, including Regional Clinical Care, Care Solution Center (Referral Assessment & Liaison), On-Call, Grief Services, In-Patient Units, and Palliative Care. Responsible for general oversight of dietary services. SUCCESS CAPABILITIES: Licensed RN in State of Ohio Master's degree in nursing or related field preferred MBA preferred Minimum of three years in managerial and leadership level experience Proven supervisory experience Ability to work in teams Experience in End of Life, hospice or palliative care preferred Have own transportation, a valid driver's license and insurance coverage as required in the state of Ohio Ability to manage people at multiple sites Ability to manage multiple tasks and priorities Other duties as assigned by Chief Operating Officer Ability to effectively communicate written and orally Ability to conduct public presentations Able to take initiative, work independently within teams Effective conflict resolution skills DETAILS: Total Rewards Package to include Retirement, Health, Dental, Vision, voluntary benefits and Corporate Discounts Tuition Assistance Exempt role Technology Package CONDITIONS OF EMPLOYMENT: This majority of this position is completed in office-type setting with prolonged computer and telephone use while primarily sedentary; some work may be completed in patient care settings. This role is responsible for driving business development and managing both inbound and outbound communications. Ensure timely follow-up on referrals and maintain accurate documentation of outreach activities. Market Leadership: Provide formal and informal education opportunities with physicians, physician office staff, hospital social workers/discharge planners/patient unit staff, nursing home staff and staff in community agencies. Collect and analyze data of referral patterns from all referral sources, to include physicians, social workers, discharge planners, hospital vs community physicians and SNFs. Maintain a thorough knowledge of the community's health care market, competition, consumer demands and governmental policies which influence the agency's outreach goals and access for patients/families Act as the face of the organization in the local market, promoting brand awareness and trust. Monitor market trends and competitor activity to inform strategic planning. Skills and Experience Required Proven experience in healthcare marketing, sales, or community outreach-preferably in home health, hospice, or a related field. Demonstrated ability to meet or exceed performance goals and key performance indicators (KPIs). This includes: Referral volume targets Patient admission and conversion rates Community engagement metrics Timely follow-up and documentation standards Relationship development with referral sources Strong interpersonal and communication skills, with the ability to engage diverse audiences. Knowledge of Medicare/Medicaid and other payer sources is a plus. Knowledge of Hospice eligibility and levels of care is preferred Knowledge of customer referral source [CRM] management systems for activity tracking Ability to work independently and collaboratively in a fast-paced environment. The Clinical Admissions Nurse is responsible for assuring for assuring that HWR programs are effectively and responsibly represented to patients, families, and others who have interest in hospice services through assessment, information sharing and/or admissions in a patient's home, nursing facility, or in a health care facility. Provides information assessment, and admission services to patients who have been referred to HWR: conducting assessment visits in the home nursing facility or health care facility, as necessary. Communicates with Referral Department, the referring Physician, and Hospice MDs to assure appropriateness of each referral and those necessary certifications are obtained. Keeps the Referral Department informed about the progress of the referral/admission. Applies nursing principles and techniques in the treatment of patients. Provides nursing care, emotional support and education to patients and families. Utilizes computer skills to access medical information and documents in electronic medical records as required. Promotes teamwork by participating actively and appropriately as a member of an interdisciplinary team, shares essential patient information with all members to coordinate care. Completes and submits documentation within agency timelines policies that is complete, legible, and clearly reflects care given. Utilizes appropriate resources to maintain knowledge and skills for the hospice care in all settings including regulatory/ reimbursement guidelines. Collaborates with other members of the IDT in coordinating care to be efficient and financially responsible. Makes appropriate referrals to other discipline services and medical equipment/ supply companies to meet patient care needs in a timely manner. Works with diverse social and economic situations without imposing own values on patients or family, maintaining appropriate professional boundaries. Meets standards for continuing education and maintain licensure. Demonstrates understanding of HIPPA privacy Standards and related Agency policies. SUCCESS CAPABILITIES: Graduate of an accredited nursing program: BSN and/ or CHPN Experience with volunteer management and HR practices preferred Required Nurse Licensure by the state of Ohio, valid driver's license with an automobile that is insured in accordance with the state and/or agency requirements and is in good working order. Minimum of one year of nursing experience required: intake, home care, hospice and/or oncology nursing experience required.
