WHEN DO ON-CALL NURSES MAKE A DIFFERENCE? Schedule options available as an After-Hours Nurse would be 4:30p-5:00a. $3,000 S ign-on bonus for FT. WHAT YOU WILL DO: Assess physical, psychosocial and spiritual needs of the patient and family (RN only). Gather data related to patient and family's physical, psychosocial and spiritual needs (LPN). Provide intermittent skilled nursing services to the patient under the patient's physician's direction. Monitor and evaluate pain and symptoms; provide information and assistance to the patient and family to manage those problems. Serve as a liaison to the patient's physician(s) when needed; provide information to the physician based on knowledge relating to the patient's needs. Provide emotional support to the patient and family. Refer patients and/or family members to other services when appropriate. Provide instruction (health care teaching) to the patient and family as related to care in a residence. Make visit to patient's residence at the time of death. Maintain, promptly complete, and return all documentation as required. Communicates and acts as liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity. Relay pertinent information to appropriate team members. Participate in meetings as initiated by team leader. Participates in mandatory or elective HWR University events, trainings, or in-services as necessary. RN's shall participate in triage coverage as needed Perform other duties as assigned. SUCCESS CAPABILITIES: High School Diploma or equivalent required. Graduate of an accredited school of nursing, with a Bachelors degree preferred. Licensed RN/LPN in state of Ohio. Experience preferred in pain and symptom management, oncology, or mental health. Ability to communicate effectively, orally and in writing Ability to work independently and collaboratively with a team Must have demonstrated knowledge and skills to provide appropriate care to all age groups. Ability to handle stressful and constantly changing conditions; must be able to make good professional decisions quickly despite stressful atmosphere. Ability to access patients and families in any residential and/or public setting in any weather condition (i.e., climbing stairs or snow filled walkways, etc.). Proficiency in Microsoft Office suite of products, particularly proficient in Word and Excel, use of shared team files, a donor database, grant portals and virtual meeting technology Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR's insurer. PHYSICAL REQUIREMENTS Ability to stand and walk for extensive periods of time. Manual and finger dexterity and eye-hand coordination to provide nursing care and operating equipment. This is heavy work requiring the exertion of up to 75 lbs. of force occasionally as well as 50 lbs. frequently utilizing appropriate safe transfer techniques and body mechanics when working with patients. Ability to stoop, squat, or bend frequently. Corrected vision and hearing within normal range to observe and communicate with patients and professional staff. Possibility of exposure to environmental elements, internal and external, such as disease, bloodborne pathogens, and chemicals and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations.
