Provide palliative nursing care and support to pediatric patients and families
WHAT YOU WILL DO:
Assess physical, psychosocial and spiritual needs of the patient and family.
Provide intermittent skilled nursing services to the patient under the patient's physicians' directions.
Monitor and evaluate pain and symptoms and provide information and assistance to the patient and family to manage those problems.
Serves as liaison to the patient's physician(s) when needed; provide information to the physician based on knowledge relating to the patient's needs.
Provide emotional support to the patient and family.
Refer and follow up patients and/or family members to other services when appropriate.
Provide instruction (health teaching) to the patient and family as related to home care.
Maintain and promptly complete all necessary plan and reports related to home care.
Participate in care conferences and bereavement meetings.
Participate in public relations activities when necessary.
Participate in audit review, quality assurance activities and other meetings as assigned.
Plan and supervise activities of HHA as related to patient care.
SUCCESS CAPABILITIES:
Licensed RN in the State of Ohio
BS or equivalent experience
3-5 Years of experience in Pediatrics, or Hospice with the desire to transfer into Pediatrics
Experience preferred in pain and symptom management, oncology, public health, mental health
Experience in pediatrics and knowledge regarding normal growth and development, pediatric assessment skills, and pharmacology
Must work a minimum of 40 hours, four days/week
May at times work 4 days, 10-hour shifts based on department needs and supervisor approval
1 evening per week for call coverage and 1 weekend worked per month.
Must have valid driver's license, car available, and insurance coverage as required in Ohio
Working coverage area may include: Lorain, Cuyahoga, Lake, Ashtabula, Medina, Summit, Geauga, and Portage counties
All attempts will be made to place PED Nurse in the service areas closer to living residence

Provide prescribed medical treatment and personal care services to terminally ill persons in collaboration with the transdisciplinary team in a variety of settings. Coordinate the implementation of the Plan of Care for each patient utilizing the Nursing Process.

Responsible to practice in accordance with HWR clinical standards of practice, policies and procedures

Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR's insurer
Hospice and Palliative Care Certification desired. Will be required within first two years of hire.
Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.

As an On-Call Nurse with Hospice of the Western Reserve have the autonomy to utilize your nursing skills. You are relied upon to take care of patients and families who need help after hours in typically urgent times. WHEN DO ON-CALL NURSES MAKE A DIFFERENCE?
Your schedule is 8p - 8:30a, with an expectation to work every other weekend and every other holiday. This role covers a multi-county territory. This position includes a $3,000 sign-on bonus for FT.
WHAT YOU WILL DO:
Assess physical, psychosocial and spiritual needs of the patient and family (RN only).
Gather data related to patient and family's physical, psychosocial and spiritual needs (LPN).
Provide intermittent skilled nursing services to the patient under the patient's physician's direction.
Monitor and evaluate pain and symptoms; provide information and assistance to the patient and family to manage those problems.
Serve as a liaison to the patient's physician(s) when needed; provide information to the physician based on knowledge relating to the patient's needs.
Provide emotional support to the patient and family.
Refer patients and/or family members to other services when appropriate.
Provide instruction (health care teaching) to the patient and family as related to care in a residence.
Make visit to patient's residence at the time of death.
Maintain, promptly complete, and return all documentation as required.
Communicates and acts as liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity.
Relay pertinent information to appropriate team members.
Participate in meetings as initiated by team leader.
Participates in mandatory or elective HWR University events, trainings, or in-services as necessary.
RN's shall participate in triage coverage as needed
Perform other duties as assigned.
SUCCESS CAPABILITIES:
High School Diploma or equivalent required.
Graduate of an accredited school of nursing, with a Bachelors degree preferred.
Licensed RN/LPN in state of Ohio.
Experience preferred in pain and symptom management, oncology, or mental health.
Ability to communicate effectively, orally and in writing
Ability to work independently and collaboratively with a team
Must have demonstrated knowledge and skills to provide appropriate care to all age groups.
Ability to handle stressful and constantly changing conditions; must be able to make good professional decisions quickly despite stressful atmosphere.
Ability to access patients and families in any residential and/or public setting in any weather condition (i.e., climbing stairs or snow filled walkways, etc.).
Proficiency in Microsoft Office suite of products, particularly proficient in Word and Excel, use of shared team files, a donor database, grant portals and virtual meeting technology
Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR's insurer.
PHYSICAL REQUIREMENTS
Ability to stand and walk for extensive periods of time.
Manual and finger dexterity and eye-hand coordination to provide nursing care and operating equipment.
This is heavy work requiring the exertion of up to 75 lbs. of force occasionally as well as 50 lbs. frequently utilizing appropriate safe transfer techniques and body mechanics when working with patients.
Ability to stoop, squat, or bend frequently.
Corrected vision and hearing within normal range to observe and communicate with patients and professional staff.
Possibility of exposure to environmental elements, internal and external, such as disease, bloodborne pathogens, and chemicals and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations.

Our Clinical Agents (Telephone Triage RN's) provide prescribed medical treatment and personal care services to terminally ill persons in collaboration with the transdisciplinary team in a variety of settings. WHAT YOU WILL DO:
Answer escalated calls from families, hospital staff, nurses' pharmacies, patients etc and providers to begin assessing situation and gaining understanding of the patients specific care needs and begin advising care.
Triage inbound calls for multiple health care professionals and families and provide intervention (be it advise or dispatch)
Demonstrate calm and attentive presence, expert administration of care and a unwavering spirit of compassion
Responsible for relaying updated or new provider contact information to staff and management.
Accurately document inbound/outbound patient information in agency's EMR and associated systems
Provide a next level medical advice to supporting agents by being in a ready status in the Five9 system to assist.
Perform other duties as assigned
SUCCESS CAPABILITIES:
High School Diploma or Equivalent required.
Graduate from an accredited school of professional nursing or a school accepted for candidacy status from an accrediting body. A Registered NurseLicense (RN) in the State of Ohio
2-4 years of clinical support of triage or call experience in medical setting
Prefer a minimum of 3 years recent clinical experience in a physician's office, hospice or palliative care work, home health, critical care and or emergency room
Relevant experience includes direct patient triage, clinical phone triage, demonstrated excellence in clinical assessment using nursing process, or adult/pediatric nursing care in a physician or hospital setting
Excellent verbal and written communication skills
Excellent clinical judgment and decision-making skills
Ability to adapt to effectively handle stress and difficult situations
Ability to work with minimum direction and supervision
Strong attention to detail
Ability to work in a team environment
Proven ability to demonstrate excellent and strong empathic phone and verbal communication with active listening skills; necessary for customer focus and adapting to varying personality types
Ability to multitask, set priorities and mange time effectively; ability to listen, type and update records simultaneously
Proficient with computers
Familiar with EMR systems and practices (Suncoast preferred)

The APN will collaborate with the community physicians to implement the plan of care for the patient. QUALIFICATION REQUIREMENTS
* Graduate of an approved Advance PracticeNurse Program.
* Current unrestricted license (APN and RN) to practicenursing in Ohio and Michigan.
* Clinical specialty in Adult, Family or Gerontological medicine. Maintains certification through required continuing education.
* Minimum of 3 - 4 years experience as an advance practicenurse; home health or hospice/palliative experience preferred.
* Has working experience charting in an EMR system, NetSmart system preferred.
* Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements.
* Must be CPR certified according to the American Heart Association CPR standards.
* Competent in the skills and knowledge required to perform according to scope of practice; requires minimal supervision.
* Possesses strong verbal and written communication skills.
* Demonstrates problem-solving skills and the ability to manage difficult situations effectively.

* Evaluates and revises care plan per patient's response. Prepares and maintains accurate patient records, charts and documents to support sound medical practice and reimbursement for services provided.
* Utilizes approved references and procedure manuals when providing care.
* Collaborates and consults with the palliative care physician as needed and complies with applicable laws and regulations with respect to standard care arrangement.
* Informs referring physician of services provided and treatments recommended and provides a written recommendation report to referring physician.
* Clearly communicates with patients, families and referring physicians.
* Treats patients/families with respect/compassion; involves patients/families in the care planning process; supports social/spiritual/cultural needs of others; ensures family and patient confidentiality.
* Assists with determination of palliative care eligibility and end-of-life care options.
* Participates on a Palliative Care Interdisciplinary Team.
* Provides support to the hospice program by performing clinical visits on hospice patients as assigned.

* Initiates reimbursement for services rendered.
