The Human Resource Assistant oversees the daily functions of the Human Resource (HR) department while serving as a resource to all internal and external clients to provide excellent service through a variety of responsibilities related to all cycles of an employee's experience while supporting the HWR initiative to Recruit, Retain, Reward, Retire.
WHAT YOU WILL DO:
Provides guidance to internal and external clients which may include answering questions and explaining procedures, processes or team activities; distribution of forms, fliers, HR communication or other agency materials. Manages HR inbox and responding or forwarding inquiries to appropriate person
Performs routine tasks required to administer and execute human resource programs
Processes employee status, pay changes, or informational changes received in HR and monitors information for accuracy and completion.
Reviews, tracks, and documents compliance with mandatory and non-mandatory employment related documentation including but not limited to continuing education or training and work assessments, safety training, workplace practice training, professional licensure, and exams or certifications.
Supports the recruitment, onboarding and recruitment process through collaboration with HR Team, managers, and employees to ensure consistent and accurate processing of information.
Verifies licensure of Physicians, RN's, CLPN's, HNA's, SW's, and Counselors; monitors expiration as required for licensure of all active staff.
Assists with new hire orientation preparation
Maintains compliance with federal, state, and local employment laws and regulations, and recommended best practices; ensures all work locations have required labor law and worker's compensation postings adequately displayed.
Maintains knowledge of trends, best practices, regulatory changes, and new technologies in human resources, talent management, and employment law.
Ensure stock of supplies and order HR office supplies, as needed
Maintain and ensure compliance of HR personnel files in conjunction with volunteer support
Assists in audits as needed.
Other related duties as assigned.
SUCCESS CAPABILITIES:
High School Diploma or equivalent required

The Director of Development (DOD) will be accountable for all major fundraising efforts for Hospice of the Western Reserve (HWR) and its affiliates. A senior member of the development team, the DOD will work closely with the CEO of HWR, Foundation President and development team members to lead the planning, implementation, cultivation, solicitation and stewardship of gifts in the five to seven figure range. You will personally manage a portfolio of 75-100 individual and family foundation relationships with deep connections to HWR. The successful candidate will be a natural collaborator and develop strong internal relationships with the clinical and program leaders, finance team, and foundation and operating boards of directors. In additional to growing annual support, the DOD will be responsible for raising funds for organization-wide strategic initiatives, county specific events and endowed funds.
WHAT YOU WILL DO:
In partnership with the Development team, lead the creation of all major gift fundraising strategies and activities to achieve annual fundraising goals.
Work with multiple high-level stakeholders to develop compelling cases for support and proposals.
Utilizing the Microsoft Dynamics CRM platform, develop a comprehensive moves management system for all gift officers.
Manage the annual budget
Provide support for HWR Foundation meetings and retreats
Strategize with key project staff members to identify annual operating, programmatic, and capital needs as prioritized by the agency
Continually identify new grateful family members whose funding priorities match HWR's needs annually; develop the strategy for initial approach, cultivation, and proposal submissions
Coordinate donor meetings, site visits and tours including the creation of agendas, preparation, and talking points for HWR leadership
Coordinate with HWR's project, program, and team leaders to develop accurate and persuasive narratives, budgets, and outcome measures
In partnership with the Marketing team, develop and implement strategies for regular donor communication and engagement which may include mailings and events
As needed, provide institutional voice for leadership articles, speeches, and white papers
Support all aspects of HWR Development in team planning and goal-setting sessions, prospect management, proofreading and review, working at events and reporting to management and Board

SUCCESS CAPABILITIES:
Bachelor's degree required; CFRE or advanced degree preferred.
A minimum of seven (7) years in fundraising, including a proven track record in five-figure gifts.
Excellent oral and written communication skills.
Ability to work independently and confidently with senior leadership and clinical teams at HWR.
Strong managerial style and experience developing and promoting direct reports.
Excellent interpersonal skills and the ability to relate to Development colleagues, donors, volunteers, clinicians, and staff.

The Director of Provider Relations is pivotal in providing leadership to identify opportunities for growth and profitability while developing and executing strategies that identify new provider relations opportunities.
WHAT YOU WILL DO:
Develops and executes all initiatives in areas inclusive of sales management, service excellence, new account acquisition, strategic relationship building, and team development
Creates effective, meaningful, ongoing relationships with a vast array of professional and community referral sources, including, but not limited to physicians and their staff, facilities and other programs as needed; promotes continued growth and understanding of hospice and palliative care to ensure that service excellence is attained within the community.
Analyzes and prepares detailed reports on referrals from physicians, hospitals, case managers and other referral sources; utilizes reports and analysis to establish and implement short- and long-term goals, objectives and operating procedures; drafts and updates quarterly territory plans and oversees the positioning of hospice marketing while running day to day sales growth activities.
Identifies and develops plans for underserved or unserved populations; participates, as indicated, in collaborative end of life care projects and business development endeavors with healthcare and community organizations; ensures adequate and timely quantitative documentation of marketing and referral activities.
Develop and oversee the Provider Relations strategic plan. Create goals and measurable objectives for the marketing plan. Evaluate the plan and its performance routinely and annually.
Create effective, ongoing relationships with a vast array of professional and community referral sources, including, but not limited to physicians and their staff, hospital nursing, social work, and discharge planners; managed care/payer organizations, social service and nursing administration, staff of alternative homes, home health agencies, disease related groups, MRDD organizations, legislators, employers and community to enhance understanding and utilization of hospice and palliative care services.
Develop and evaluate the continued use of the systematic monitoring of referrals, admissions and average length of stay (ALOS) history and other data for all referral sources.
Serve as HWR representative to identified community and professional healthcare organizations. Attend meetings and participate in networking, speaking engagements, health fair participation and committee work.
Support negotiations of contracts with nursing facilities.
Initiate and maintain liaison relationships with inpatient facilities including GIP, respite and residential contract negotiation, and implementation.
Participate in the Quality Assurance/Utilization Review process on an ongoing basis
Provide input and follow-up with professional and community referral sources on quality concerns.
Plan and implement educational programming and events for physicians and other allied health professionals
Participate as an active member on committees as needed.
Coaches staff and others in group presentations, seminars, workshops, displays and special events for physicians and other allied health professionals

MANAGEMENT
Responsible for the oversight and leadership of the Provider Relations team.
Oversee the initiation and participation in creative problem-solving dialogues with the Provider Relations team and HWR staff to effectively address the needs of patients, families, and external referral sources.
Monitors the overall results of team sales efforts and makes recommendations and/or necessary changes in strategy to build and develop external business leads and relationships, and effectively promote hospice services with referrers.
SUCCESS CAPABILITIES:
Bachelor's degree in business or healthcare related field.
Master's degree from an accredited college or university in business or public administration preferred.
Minimum 5 years' healthcare experience: hospice, homecare, assisted living and/or skilled nursing exp preferred.
Minimum 5 years' experience with business development.

Support agents will be the first point of contact for those seeking hospice services. This role provides patients, physicians, and hospital facilities with guidance through out the organization. GENERAL RESONSIBILITIES
Under general direction and supervision of the Hospice Medical Director/CMO, provides administrative and patient care services.
Position is Fulltime.
ESSENTIAL FUNCTIONS
Consistent with 42 C.F.R. § 418.64 (a) Physician acknowledges that he/she is, along with the Hospice Medical Director, Hospice's physician employees, contracted physicians, and each patient's attending physician, responsible for the palliation and management of the terminal illness and conditions related to the terminal illness of Hospice's patients.
Physician will perform administrative and patient care services as requested by Hospice or the Hospice Medical Director, which may include, but are not limited to, the services described in this document.
Physician, as a physician member of the hospice IDG, will perform certification/recertification of terminal illness consistent with 42 C.F.R § 418.22 (c).
Physician will participate in IDG/transdisciplinary team conferences to develop and revise patient care objectives and procedures; provide expert consultation and participate in team decisions regarding appropriate settings and levels of care for patients; and collaborate with pharmacists concerning medication formulary and best prescribing practices.
Physician will perform face-to-face encounters, as needed, along with the performance of other clinical aspects related to the Hospice's patient care program, including coordination of admissions and discharges.
Physician will provide direct medical care to patients in their home setting or in the hospice residential facility, which may include being the patient's designated attending physician if requested by the patient or the patient's representative.
Physician will keep abreast of standards and regulations, attend trainings, participate in QAPI, and meet all other regulatory and agency requirements.
Physician will develop and maintain positive relationships with the physician community, promoting hospice and palliative care; precepts medical trainees.
Physician will participate in Hospice's committee work, as requested.
Physician will participate in and/or perform palliative care consultations, as requested.
If serving as the Physician Designee, Physician will assume responsibility of the medical component of the Hospice's patient care program pursuant to the Physician Designee Policy and the Appointment of Physician Designee form. Lead Physician will not assume the Physician Designee role without direction from the Hospice or the Hospice Medical Director.
Physician will perform other duties, as required.
REQUIRED EDUCATION AND EXPERIENCE
Current, unrestricted licensure of Doctor of Medicine or Osteopathy.
Must be able to obtain physician licensure in the State of Ohio at time of start of employment.
Must maintain a valid DEA license and board certification.
Initial board certification in a primary ABMS specialty. Preferred primary specialties of oncology, internal medicine, family practice, hospitalist medicine, or geriatrics.
Minimum 2 years of demonstrated work experience in hospice and/or palliative care or another similar setting managing the care of terminally ill patients.
Experience caring for patients in a hospice inpatient unit setting preferred.
Board-certification or Board-eligibility in Hospice and Palliative Medicine, PREFERRED.
Applicants currently participating in a qualified Hospice and Palliative Medicine Fellowship may apply.
If Board Eligible, Board Certification in Hospice and Palliative Medicine is required within 2 years of employment.

Provide personal care under the supervision of a nurse according to the patient's plan of care. Collaborate with transdisciplinary team in a variety of settings. This is a 36-hour per week position with an every other weekend, every other holiday requirement. The position out of Ames Family Hospice House located in Westlake, Ohio
WHAT YOU WILL DO:
Provide personal care and assist with ADL's according to the POC.
Provide safe environment for patients.
Safely ambulate/transfer patient as appropriate.
Assist with passive and active ROM exercises as directed by the nurse.
Prepare nutritional snacks as appropriate for the patient.
Under the supervision of the nurse, assist patient with pre-poured medications that are ordinarily self-administered and that are prescribed by a physician under the supervision nurse (Home Care only).
Under the direction of the nurse, perform simple wound care, skin care, and bowel care protocols according to the plan of care where appropriate.
Recognize and promptly report any changes in the mental or physical status of the patient to the nurse.
Record in clinical record, on appropriate documents, the activities provided and observations of patient at the time of visit.
Complete all charting, timesheets and mileage logs as defined by the agency policy and procedures.
Attend and actively contribute during patient care conference.
Participate in HNA discipline meetings, team meetings, staff meetings, retreats and in-services as appropriate.
Attend in-services to obtain 3 education hours per quarter.
Participate in the on-call rotation as scheduled (Home Care only).
Assist the Team Leader with other designated activities as necessary (scheduling, supply organization, answering office telephone, etc.)
Demonstrate positive customer-service responsiveness according to agency standards.
Flexible in meeting patient/family needs that require variable shifts and weekends.
Participate in chart review quality improvement activities and other meetings as assigned.

They assist to carry out Care Solutions Center operational strategies by conducting needs assessments, performance reviews, capacity planning, and cost-benefit analysis: defining user requirements, establishing productivity, quality, customer service standards, contributing information and analysis to organizational strategic plans and reviews.
Builds relationships with staff in hospitals they serve, being a trusted partner.
Trained and qualified nurse who can help guide teams to provide support for families and patients with empathy and confidence
They implement the Care Solutions systems by assisting the Director, and Chief Clinical Officer with patients, families, referral providers, doctors, and Hospice of the Western Reserve field staff.
Collaborate with trainers to implement a comprehensive training program for staff in the field.
Determines training needs by observing agent and service encounters, studying all phases of the Care Solutions services, technical, service and customer experience results reports, conferring with management.
Updates job knowledge by participating in educational opportunities, reading professional publications, maintaining professional/personal networks
Educational awareness with EMR (Suncoast) on a regular basis.

Maintain and promptly complete all necessary plans and reports related to hospice care.
Participate in care conferences and bereavement follow-up.

The Clinical Admissions Nurse is responsible for assuring for assuring that HWR programs are effectively and responsibly represented to patients, families, and others who have interest in hospice services through assessment, information sharing and/or admissions in a patient's home, nursing facility, or in a health care facility. This position works closely with the Referral Department and other referral sources to promote the hospice philosophy of care.
WHAT YOU WILL DO:
Performs all duties and responsibilities in accordance with the Nurse Practice Act and in accordance with basic principles and guidelines of professional nursing.
Provides information assessment, and admission services to patients who have been referred to HWR: conducting assessment visits in the home nursing facility or health care facility, as necessary.
Communicates with Referral Department, the referring Physician, and Hospice MDs to assure appropriateness of each referral and those necessary certifications are obtained.
Keeps the Referral Department informed about the progress of the referral/admission.
Applies nursing principles and techniques in the treatment of patients. Provides nursing care, emotional support and education to patients and families.
Utilizes computer skills to access medical information and documents in electronic medical records as required.
