Ability to complete physically demanding tasks. Valid driver's license and reliable transportation. PREFERRED: Experience with residential or commercial property maintenance with basic knowledge of building codes, safety regulations, and ADA guidelines. Familiarity with accessibility modifications for older adults or individuals with mobility limitations, including Certified Aging in Place Specialist (CAPS) certification. Experience working with older adults or in a similar caregiving role. PHYSICAL DEMANDS: Position requires the ability to lift in excess of 75 lbs. Must be able to walk, stand, climb, kneel, reach, crawl, push, pull and twist the majority of the workday. While performing tasks in patient care areas there may be a risk of exposure to infectious diseases requiring standard safety precautions. Qualifications QUALIFICATIONS: Graduate of an accredited school of social work with Master's degree in Social Work required, licensed Social Worker required. Must have a minimum of 2 years social services experience, family work and healthcare experience preferred. Shall adhere to and promote the ethics and values of the Social Work profession using the NASW Code of Ethic as a guide to ethical decision making in social services practice. Valid driver's license, automobile insurance and reliable transportation required. Excellent communication skills (written and verbal) and the ability to successfully interact with a diverse patient population, families, physicians, Hospice Savannah staff, and the community at-large along with excellent customer service and documentation skills required. Computer proficiency in Microsoft Office, including Word and Outlook, and database knowledge required. PHYSICIAL DEMANDS: Position requires moderate visual concentration for patient charting/documentation and computer work. This position carries a risk of exposure to infectious diseases and requires skill in standard safety precautions. The Social Worker may be required to assist with ambulating or lifting a patient with or without the assistance of devices in excess of 75 lbs. utilizing proper body mechanics. WORK ENVIRONMENT: A full-time position works a 30-40 hour work week, rotating days, evenings, or weekends, with occasional requests to work additional days/hours/holidays as needed The Social Worker is required to participate in the rotating on call schedule and may be required to provide care in what has been determined by local authorities as a high-risk crime area and unsafe situations (potential or actual). This position encounters stressful situations with patients and their families regarding end of life care. The Social Worker must be able to accurately, timely and concisely document the patient/family encounter using a computer. Although each Support Services Provider may be designated to a specific department/region in our service area, at times they may be required to serve patients outside of their department/specific region. This position is required to perform patient visits utilizing their personal vehicle and may require extensive driving.
