Required Qualifications • At least 5 years health care, preferably in care coordination, and at least 2 years of experience serving the target groups of the IRIS program (adults with physical/intellectual disabilities or older adults), or equivalent combination of relevant education and experience. • A bachelor's degree in social work, psychology, human services, counseling, nursing, special education or a closely related field, and one year of direct experience related to the delivery of social services to the target groups required.
