What You'll Do:
As a PA-C, you will be taking calls remotely for national level for CCM visits from 7pm-7am or 7pm- 7am rotating with our other providers. These duties can be performed remotely but you must be available to take the calls when you are scheduled to work. The ideal candidate must be licensed in Michigan and complete CCM visits monthly. Typical schedule may be varied and shall include weekends, holidays, and evenings.
You should have experience in one of the following specialties: acute care, family health or geriatrics. Patience, problem-solving, and good communication skills are important. If you are dedicated to advancing your knowledge and clinical expertise through research and professional discourse, we would like to meet with you to share our mission, vision, values for the post-acute industry.
Your daily responsibilities include:
Examine patients medical records
Ordering and interpreting diagnostic tests (labs, imaging, etc.).
Diagnosing and managing acute and chronic conditions.
Prescribing medications and creating treatment plans.
Collaborating with physicians, nurses, and facility staff.
Educating patients and families on health conditions and preventive care.
Documenting patient care accurately and efficiently.
Oversee patients' overall health
Promote good health practices
Other duties as assigned

The Advanced Practice Clinician in the Care Anywhere (CAW) program is designed to provide care and support to our Alignment Healthcare patients by providing an additional level of medical and social support in the comfort of their own home. Responsibilities:
1. Conduct in-home assessments on Alignment members, medication review and health screenings
2. Provide patient education by matching care desired with best care given
3. Identify diagnoses to be assessed in care management and active medical management
4. Partner and communicate with Regional Medical Officer and other team members to discuss and develop the most appropriate care plans possible based on the needs of our members/patients.
5. Coordinate care with multiple stakeholders, including but not limited to PCPs, specialists, and ancillary providers.
6. Lead broader clinical team which may include nurses, health coaches, social workers, and care coordinators

We are looking for a Physician Associate (PhysicianAssistant) to join Akido's Street Medicine team in Kern County and help redefine how healthcare is delivered to the most vulnerable populations. We set compensation based on the level and skills required for the role. We value pay transparency and equity, and are committed to fair pay. In order to prevent pay disparities and reduce time spent in negotiations, we take a “first and best” offer approach: this means we're not holding any compensation back from our candidates, and you can feel confident that our pay is fair and does not vary based on the strength of someone's negotiation skills.
Compensation is dynamic at Ophelia: as long as the company performs well and meets our targets, there will be opportunities for increased compensation annually. We're happy to discuss this approach and our bands if you have questions during the interview process.

We are seeking a dedicated and compassionate Nurse Practitioner (NP) or PhysicianAssistant (PA-C) to conduct virtual Annual Wellness Visits (AWVs) and Advanced Care Planning (ACP) sessions for Medicare patients.
This is a fully remote, flexible role ideal for a self-motivated provider who excels in patient engagement, clinical assessment, and proactive care planning within a telehealth environment.
Key Responsibilities
Conduct comprehensive AWVs for Medicare beneficiaries via secure telehealth platforms.
Facilitate structured ACP discussions, documenting patients' goals and care preferences.
Review medical histories, medications, functional and cognitive status, and preventive care needs.
Identify and close care gaps by guiding patients on screenings, vaccinations, and chronic disease management.
Collaborate with interdisciplinary care teams and primary care providers to ensure coordinated, patient-centered care.
Maintain timely, accurate documentation in the EHR.
Adhere to CMS guidelines and clinical quality standards.
Qualifications
Active, unrestricted Hawaii NP or PA-C license.
Minimum 3 years of clinical experience, including AWVs and ACP.
Strong knowledge of Medicare guidelines and preventive care protocols.
Proficiency with virtual care technology and EHR systems.
Excellent communication and interpersonal skills.
Bilingual in Tagalog or Ilocano is a plus (not required).
Ability to work independently and manage a flexible virtual schedule.
Preferred Skills
Background in geriatrics, primary care, or internal medicine.
Familiarity with HCC coding and risk adjustment.
Experience in value-based care or accountable care organizations (ACOs).

POSITION DUTIES AND RESPONSIBILITIES:
* Manage the implementation and administration of radiologist and physician extender schedules for several local practices
* Generate and manage all changes to the QGenda schedule and provide timely communication to radiologists and clients as schedule changes are made
* Arrange emergency coverage, as well as coverage for conferences/meetings for physicians, as needed or in the event of a no call/no show
* Generate reports including but not limited to distribution of assignments, radiologist specific stats (i.e., shifts worked, extra shifts, extra comp, vacation, etc), payroll-related reports, etc
* Maintain frequent communication with Practice Directors to ensure awareness of staffing requirements, changes and future goal
* Communicate with physicians regarding changes/updates to practice scheduling protocols
DESIRED PROFESSIONAL SKILLS AND EXPERIENCE
* Strong knowledge and experience with QGenda is required for this position Minimum of
* 2 years' experience with physician scheduling, required Managing radiologists' schedules, strongly preferred
* Associate degree or commensurate experience in fast-paced healthcare environment requiring timely responses to requests and effective management of multiple tasks simultaneously, preferred
* Strong phone etiquette and customer service skills Ability to communicate effectively with staff, management, and physicians, both verbally and written
