Job Summary:

Physicians also must be able to effectively communicate with the senior medical directors and providers as well as other personnel. Physicians are responsible for the delivery of appropriate medical care to all patients who present themselves at a Network Health Center, associated facility or are admitted to the hospital under the care of Network staff. Physicians render that care in which they have expertise as attested to by their credentials, delineation and experience.
Essential Duties and Responsibilities: Physicians assume the following responsibilities:
* Provides health care services to patients. Examines patient, performs comprehensive physical examination, and compiles patient medical data, including health history and results of physical examination.
* Order or administer diagnostic tests, such as x ray, electrocardiograms and laboratory tests; interpret and act upon test results to diagnose injury or illness; prescribe medication or therapy to manage acute and chronic illness as well as foster wellness. Communicate all results to patients through means such as the portal, phone, mail or in person.
* Perform therapeutic procedures by administering injections and immunizations; suturing; managing wounds and infections if appropriate.
* Cultivates a climate of trust and compassion for patients to maximize compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living, health maintenance and preventative actions to promote overall health
* Based on thorough and comprehensive exam and assessment, determines the need for and level of urgency for patient referrals or follow-up; cultivate open communication with a variety of specialty providers to determine the need for initial consult versus continued involvement in care.
* Supervise and is listed as collaborating physician for APC's.
* All providers are expected to maintain authorized provider status with the Workers Compensation Board.
* Consults with Chief Medical Officer, Associate Chief Medical Officer, Lead Provider and other professionals on staff as needed regarding patient care, assessment, and education issues.
* Responds well to feedback, both positive and negative, regarding patient care; strive for good patient satisfaction scores and responds well to remediation if indicated. No adverse trends noted in patient satisfaction survey results.
* Assists in the resolution of complaints, requests and inquiries from patients.
* Meets or exceed Network standards for productivity, chart maintenance and coding accuracy.
* Participates in rotation of nighttime pager call coverage in support of APC's.
* Demonstrates scheduling flexibility to provide patient access across health center locations by
* participating in weekend, evening and holiday coverage. There is the option to end weekend work at the age of 70 years old.
* Remains up to date with developments and best practices in medicine by attending conferences and seminars.

Professional Qualifications:
Physicians must be licensed to practice medicine in New York State; meet all Network credentialing requirements as set by the corporate by-laws; and demonstrate the appropriate skills, knowledge and leadership abilities to successfully fulfill the requirements of the position.
Administrative Relationships:
Your principal reporting relationship will be to the Chief Medical Officer or his/her designee. You will also be expected to maintain collegial working relationships with administrative and clinical staff members of the Network, with appropriate regard for the responsibilities that those individuals hold. Initially, you will work with an assigned mentor who will assist you in acclimating to the Network's practice environment. On an ongoing basis, you will meet periodically with a designated regional or lead provider to review your progress and discuss any matters of mutual concern.

The Ultrasound Technologist is responsible for performing ultrasound studies following ACR, State, Network, department, and manufacturer's recommendations and guidelines to create diagnostic quality images. Exams are performed as directed by an MD, DO, PA, or NP while embracing the core values of Quality, Appreciation, Creativity, and Sustainability in pursuing the HHHN Mission and Vision.

Select proper technical factors and patient positioning for the creation of diagnostic quality images.

Note: This position will be required to float between two of Hudson Headwaters' southern region Health Centers.
Position Summary
The Registered Dietitian provides age appropriate, population specific, nutrition consultation and education for patients and families across the lifespan for a variety of health conditions, such as chronic disease management, weight management, disease prevention and nutrition support. Care is delivered, either one-on-one or in a group setting, in a professional, compassionate, caring, culturally and ethnically sensitive manner, while embracing the Core Values of Quality, Appreciation, Creativity and Sustainability in pursuing the HHHN Mission and Vision. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily:
Essential Duties and Responsibilities:
* Collaborate with the Network team as the content expert for Nutrition to implement population health focused initiatives and programs.
* Assist in the development of Network educational materials and tools to improve care.
* Provide ongoing patient and population specific education and outcome-oriented nutrition counseling to patient/family/caregiver regarding nutritional status, adequacy measures and results, and all nutrition related lab parameters pertaining to their disease process.
* Demonstrate competency in the application of the Nutrition Care Process (NCP).
* Assesses patient's knowledge of dietary needs and provide tailored education regarding the relationship of diet, lifestyle, and chronic illnesses.
* Monitor patient adherence and response to nutrition therapy, adjust therapy as needed to achieve and sustain an effective nutritional status, document why the patient was unable to achieve the goals and implement the plan of care to address issues impacting the patient's ability to achieve the goals.
* Conduct and support Network group nutrition classes.
* Attend meetings and serve on various organization committees that support the Mission, Vision, and Core Values.
* Demonstrate excellent critical thinking skills and flexibility within the role.
* Perform other duties as directed.
Qualifications:
The requirements listed below are representative of the knowledge, skill, and ability to perform the essential functions:
* Bachelor and/or Master of Science degree in Nutrition/Dietetics (Didactic Program for Dietetics) with completion of an accredited dietetic internship program.
* Current registration by the Academy of Nutrition and Dietetics (AND).
* NYS Education Department licensure as a certified dietitian-nutritionist required.
* 3-5 years of clinical nutrition experience preferred.
* Must possess current BLS certification.
* Current membership inf the Academy of Nutrition and Dietetics preferred.
* Experience with diabetes and weight management required.
* Ability to travel throughout the Network.
* Proficiency with Microsoft applications and electronic medical records.
* Self-directed, detail-oriented, motivated and ability to work independently as well as in a team atmosphere.
* Excellent verbal and written communication skills.
The pay range for this position is $34.00/hour.
