Essential Duties and Responsibilities:
Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions and reports abnormalities to Medical Provider or RN.
Documents nursing observations, interventions, and patient's response to treatment in patient records
Demonstrated competency in performing venipuncture for collection of blood specimens and facilitates the prompt and accurate processing and distribution of specimens collected
Ensures that patients receive timely and appropriate care within the delivery system and facilitates internal and external referrals in collaboration with the RN
Attends internal and external educational seminars/ staff trainings.
Administer prescribed medication to clients and keeps accurate records of all medication
Performs all related duties, as assigned, or unrelated duties, as requested, during emergencies
Ensure that SRH standard procedures and workflows are implemented and followed, including but not limited to: medication management, pre-visit planning, team huddles, accurate and timely completion of Nursing Quality Logs, exam room readiness, ordering and storing of supplies and equipment
Demonstrate knowledge of the principles of growth and development over the life span, and provide age- appropriate and culturally sensitive care, including use of medical interpreters as indicated
Functions as preceptor to students, new hire candidates and other clinical team members/ staff when needed
Utilizes standing orders in accordance with Sun River Health policies and procedures
Attend mandated training and demonstrate competence on all nursing procedures, including relevant lab procedures
Perform direct patient care duties of an LPN: interview patients for health history, prepare equipment and assist practitioner during examination and treatment of patients, document in patient's medical record according to SRH standard and gather information from patients as needed.
Provides educational information to patients for health promotion and disease prevention to improve health outcomes
Requirements:
Minnimum Education Required: High school diploma/ GED/ Vocational/ Technical School/ Diploma Program - Nursing required
Minimum Work-Related Experience: 0-1 year
Preferred Work-Related Experience: 2-4 years
Minimum Licensure:
Valid New York State LPN License
CPR/ BLS-HCP Certification

Position Summary
A LicensedPracticalNurse renders nursing care to patients, within unit of the Center or authorized community site, in support of medical care as directed by the medical staff and pursuant to the objectives and policies of the Center. She/he will perform job duties in compliance with systems and job specific stands, and additional duties or tasks as assigned.
Essential Duties and Responsibilities
Interviews and maintains patient's medical record to include: nursing observation; health history to include past and present use of medication; vital signs; vision/hearing tests; medications and treatments given; Reactions to tests i.e. PPD measurements; laboratory test results, utilizes patient identifiers. (PEAs, PhQ, Pain, Smoke Status, etc.) CDSS and ensure that CVRs are completed as needed.
Perform in-house labs, collect specimens for out-side labs perform quality control in accordance with organizational protocols.
Complete and update all assigned tasks & logs.
Initiates follow-up as necessary (as directed by provider) i.e. encounter, labs, close non-billables, etc.
Does health teaching as indicated by physician, diagnosis and/or inquires by patient. Directs patient to Registered Nurse as necessary.
Will collect data on patients under supervision of the Registered Nurse or provider. May not triage; may not interpret patient clinical data or act independently on such data.

Position Summary
A LicensedPracticalNurse renders nursing care to patients, within unit of the Center or authorized community site, in support of medical care as directed by the medical staff and pursuant to the objectives and policies of the Center. She/he will perform job duties in compliance with systems and job specific stands, and additional duties or tasks as assigned.
Essential Duties and Responsibilities
Interviews and maintains patient's medical record to include: nursing observation; health history to include past and present use of medication; vital signs; vision/hearing tests; medications and treatments given; Reactions to tests i.e. PPD measurements; laboratory test results, utilizes patient identifiers. (PEAs, PhQ, Pain, Smoke Status, etc.) CDSS and ensure that CVRs are completed as needed.
Perform in-house labs, collect specimens for out-side labs perform quality control in accordance with organizational protocols.
Complete and update all assigned tasks & logs.
Initiates follow-up as necessary (as directed by provider) i.e. encounter, labs, close non-billables, etc.
Does health teaching as indicated by physician, diagnosis and/or inquires by patient. Directs patient to Registered Nurse as necessary.
Will collect data on patients under supervision of the Registered Nurse or provider. May not triage; may not interpret patient clinical data or act independently on such data.
