GENERAL DUTIES AND RESPONSIBILITIES:
Manage the development and implementation of the Workforce Development and Training programs on a territory-wide basis; through program design, program management, implementation and development that results in the individual served receiving quality services in a timely manner.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Based on identified needs of the target populations and employment opportunities, within each territory, design programs that will most effectively help trainees achieve their employment goals while satisfying Bureau of Vocational Rehabilitation, Division of Rehabilitation Services and other agencies requirements.
Maintain an open line of communication with referral sources to ensure proper services are being offered to meet their needs.
Ensure compliance with Goodwill Industries' policies, procedures and safety standards throughout the Workforce Development and Training Division operations.
Responsible for ensuring that the Workforce Development and Training Division staff are delivering quality services and meeting the needs for all trainees.
Responsible for the timely completion of all Training Center reports.
Operate computer to design and maintain Workforce Development and Training Division data base proposals, cost comparisons, etc. Prepare trainee development and case reports for the Administrator, Workforce Development and Training.
Responsible for the highest quality, on-going training programs, individual case management and other program services territory-wide.
Recruit and train Job Coaches, and other Workforce Development and Training employees.
Provide training on intakes, Individual Service Plan (ISP), Individual Employment Plan (IEP) development and Case Management, etc.
Review the Training Centers record keeping program, update as needed.
Plan, develop and implement a territory wide program that will result in target audiences (persons served and local businesses) becoming aware of and participating in the Goodwill Industries Workforce Development and Training Programs.
Represent Goodwill Industries in various vocational and occupational settings, such as community funding committees, and other workforce development areas.
Increase the flow of referrals to and thru Goodwill Industries Workforce Development and Training Programs.
Ensure compliance with organization's policies, procedures and safety standards throughout the Training Centers operation.
Ensure that all Workforce Development and Training programming conform to applicable Federal and State standards and laws and the Corporate Code of Ethics and Vision Statement.

Long Term Disability Claims Case Manager I

The Long Term Disability (LTD) Claims Case Manager is responsible for making accurate decisions on assigned claims through proactive case management according to the plan provisions, state and federal guidelines, and established protocols. This position consults with other team members, as well as, the Professional Resource Team to assist claimants with return to work efforts when the capability and opportunity exist.
The LTD Claims Case Manager can be located in multiple locations including Bethlehem, PA; Plano, TX; Holmdel, NJ or remote.

This position utilizes problem solving, analytical, written and verbal communication skills to deliver timely and appropriate disability claim decisions while providing superior customer service to all internal and external customers. This position partners with coworkers to broaden and enhance their knowledge of complex claim handling.
The LTD Claims Case Manager administers claims within a variety of group sizes ranging from small (2+ lives) to large market (1000+ lives). This position is responsible for determining integrated income to ensure appropriate financial risk and accurate payments are made. The LTD Claims Case Manager is responsible for ensuring all plan provisions are met through the duration of the claim. The LTD Claims Case Manager consults with legal, investigative resources, and financial specialists.
A selected incumbent may be assigned to the Stable and Mature block as well.

The salary range reflected above is a good faith estimate of base pay for the primary location of the position. Lutheran Social Services of Central Ohio is currently seeking a Case Manager for Support Services for Veteran Families (SSVF) for Faith Mission in Columbus, Ohio. Faith Mission is one of the largest homeless shelters in Columbus, Ohio, providing meals and shelter to men, women and Veterans with dignity and compassion.

What will I do as the SSVF Case Manager with Lutheran Social Services?
Work closely with community and government agencies (social services, shelters, faith and community based organizations, prisons, jails, VA facilities, VA grant and per diem programs, legal aid, settlement houses, landlords, etc) to identify very low-income veteran families who are at risk of homelessness.
Using appropriate tools, screen identified families for eligibility, prioritize those families in greatest need and assess whether other interventions may better serve the veteran family being screened.
Perform intake for new participants, including needs assessment, and mental/physical health screenings.
Refer ineligible families to alternate resources/supports as available in the community or through the VA.
Work closely with clients to develop and implement participant specific, individualized, intentional action-oriented goal plans based on the CTI model, with a focus on achievement of housing stability.
Implement planning systems that reflect client strengths and needs, client responsibilities, Case Manager responsibilities, and action steps to help clients in achieving their goals.
Provide budgeting and other planning assistance toward the goal of housing retention.
Coordinate with VA services, veteran programs, Faith Mission Resource Specialists and other community resources to assist clients in seeking income, employment and/or other needed supports.
Link clients to internal resources as appropriate for financial supportive services in the form of direct client assistance payments to third party (e.g., landlords).
Refer clients to partner agencies and community resources that benefit their short-and long-term goals.
Interface closely with all Faith Mission staff, VA staff and public assistance agencies to provide quality wrap-around services and access to benefits for clients.
Gather information on potential resources for clients in the larger community, creating working relationships and disseminating information for immediate and ongoing support to client.
Maintain clear and precise data and case notes, including timely submission of any CQI reports and careful documentation of all client interactions.
Actively participate in weekly Case Review, working with colleagues toward positive client outcomes.
Work with client to maintain on-going support relationships with community resources, family and friends.
Successfully complete the Critical Time Intervention training.
Contribute to an atmosphere of safety, sanctuary, dignity, support and resource through adherence to the Faith Mission Code of Ethics and Confidentiality Agreements.
Assist with crisis intervention and de-escalation by providing leadership, and by modeling professional and calm problem-solving behavior for clients, colleagues and the community.

Joint venture with Select Medical Case Manager PRN Schedule: Per Diem - 20+ hours a week - Flexible hours from between 7:30am - 5pm
Pay: $40/hr
We are seeking a Case Manager with rehabilitation experience for an exciting opportunity in a brand-new level of care.
The Neuro Transitional Center has 12 beds and is designed to provide intensive rehabilitation to individuals with an acquired brain injury or spinal cord injury who are unable to safely return home at the time of discharge from an inpatient rehab hospital due to the severity of their impairments.

Responsibilities
The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans. The Case Manager assesses the psychosocial needs of the patient and provides intervention as part of the discharge planning process.

Minimum Qualifications
Current Licensure in a clinical discipline per state guidelines required. (Registered Nurse or Social Work (SW) preferred)

Previous experience in Case Management and Discharge Planning preferred.
CCM Certification Preferred.

Requirements for this position include:
Must be at least 18 years of age.
High school diploma plus three or more years of relevant field experience or Bachelor's or Master's level degree in a related field.
Related experience or training with mental or behavioral health conditions.
Must have a valid driver's license and reliable transportation.
Willingness to travel throughout assigned service area.

What will I do as the Healthcare Navigator with Lutheran Social Services?
Work with clients to develop action-oriented goal plans with a focus on securing housing, operating from a "housing first" model.
Assist clients with referral and linkage to community-based social service and support systems that promote long-term safety, self-sufficiency, and stability.
Maintain clear and precise data and case notes, including timely submission of weekly CQI reports and careful documentation of all client interactions.
Actively participate in weekly Case Review, working with colleagues toward positive client outcomes.
Assist with crisis intervention and de-escalation by providing leadership, and by modeling professional and calm problem-solving behavior for clients, colleagues and the community.
Connect Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care.
Requirements for the Healthcare Navigator with Lutheran Social Services:
Education: Four Year degree in Social Work, or related field. Relevant experience may be substituted for degree.
Licensed Social Worker,
preferred.
License: Current and valid Ohio driver's license.
Minimum of two years working in a social service setting, required.
Experience working with homelessness, mental health or related services, strongly preferred.
