The primary purpose of the RN Discharge Planner position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, coordination and overall evaluation/monitoring of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the functions and integration of case management, utilization review and management and discharge planning. This position requires collaboration with various departments within the hospital to meet the needs of the patients and their families. Will also correspond with various insurance companies to assure that they are receiving the information they are requesting to approve the patient's admission.

Arrange peer to peer reviews as needed
Document appropriately in the electronic health record
