PRINCIPAL RESPONSIBILITIES AND DUTIES:
Assesses patient's knowledge and provides education regarding the relationship of diet and kidney disease and the impact on the treatment process.
Provides education to patient/family/caregiver regarding nutritional status, adequacy measures and results, fluid status, and all nutrition related lab parameters pertaining to their disease process.
Completes nutritional assessment and works with the interdisciplinary team to develop an individualized comprehensive plan of care for patients.
Maintains complete and accurate medical records including progress notes.
Provides ongoing assessment and counseling necessary to assist patient in achieving and sustaining an effective nutritional status.
Identifies malnourished or at-risk patients and works collaboratively with the interdisciplinary team to identify appropriate interventions, resources, or solutions.
Monitors, adjusts, and documents response to nutrition therapy and addresses any issues impacting the patient's ability to achieve the goals.
Collaborates with the interdisciplinary team on Urea Kinetic Modeling recommendations to achieve the physician's prescription of Kt/V.
Reports on nutrition QAI results and participates in the interdisciplinary QAI program.
Reviews nutrition related lab results:
Counsels patient and/or caregiver and formulates appropriate action.
Recommends treatment changes to the interdisciplinary team as appropriate.
Identifies candidates to refer to physician for nutritional supplements.
Communicates with physician and/or facility staff as appropriate.
Maintains dietetic registration and continuing education hours as specified by American Dietetic Association and state licensure regulations where applicable.
EDUCATION:
RegisteredDietitian as per Commission on Dietetic Registration.
Board Certified Specialist in Renal Nutrition encouraged.
Current state licensure if applicable.
EXPERIENCE AND SKILLS:
1+ year experience in clinical nutrition as a RegisteredDietitian.
Previous renal experience preferred.

As part of an interdisciplinary team within our Medical Services division, the professional we select will provide patients with counseling and nutritional assessments, and calculate prescription diets. This individual will assess patient knowledge of diet and kidney disease, and offer patient-specific, detailed information about nutritional status, adequacy measures and results, fluid status, and nutrition-related lab parameters.
PURPOSE AND SCOPE:
Supports Fresenius Medical Care's mission, vision, core values and customer service philosophy. Adheres to the Fresenius Medical Care Compliance Program, including following all regulatory and FME policy requirements.
Provides nutritional services for the facility's in-center and home patients as applicable in order to maximize the patient's nutritional status and improve clinical outcomes. Functions as a member of the interdisciplinary healthcare team, supporting the Fresenius Medical Care commitment to the Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities. What will you do?
Conduct intensive nutrition counseling and weight management education via phone, video, chat, text, and email.
Provide medication management and support for comorbidities related to obesity under the supervision of assigned physicians.

Job Profile Summary
The RegisteredDietitian assists our members with a variety of health concerns including, but not limited to, weight management, stress management, tobacco cessation, nutrition, physical activity and other healthy lifestyle changes. Using a collaborative process that includes assessment, planning, implementation and evaluation, the RegisteredDietitian will provide quality coaching programs that are part of an integrated and comprehensive primary care model.
Job Responsibilities
Delivers, measures, and improves high quality coaching programs for our members
Collaborates with members and care teams to assess, develop, implement, and evaluate action plans related to achieving and maintaining an optimal health status
Engages individual members and groups in healthy lifestyle changes using evidence-based coaching techniques that align behaviors to personal values while fostering an environment of individual responsibility and accountability
Conducts coaching assessments, follow-up sessions, and delivers onsite wellness classes
Provides recommendations for healthy behavior changes based on established protocols
Serves as a consultant and educational resource to providers and members
Performs other duties as assigned
Required Qualifications
Bachelor's degree in a relevant health field (i.e. Health Education, Nutrition, Nursing, Public Health, Exercise Physiology, Psychology)
Current RegisteredDietitian license
Minimum 3 years experience as a RegisteredDietitian in an out-patient primary care setting
Current BLS (Basic Life Support) certification
Preferred Qualifications
Master's degree and/or prior clinical experience
Health Coaching certificate from an approved ICHWC organization or completion within 1-year
Prior work-site health experience
Demonstrated ability to communicate, problem solve, and work effectively with people while using good judgment in new and challenging situations
Knowledge and proficiency in current, evidence-based methods of health coaching including motivational interviewing and cognitive behavioral therapy with an emphasis on health promotion and risk reduction
Experience utilizing electronic health record systems
Physical Job Requirements
