Job Description This position is responsible for providing personal care services and other support to individuals with disabilities, elderly, or chronic illness to enhance their well-being and skills needed to live on their own. This includes, but is not limited to, providing assistance and training activities of daily living, personal care services, and job-related supports. Services are provided primarily in the residence of the person(s) served in a variety of independent living settings. Personal Assistance Services for Instrumental Activities of Daily Living (IADLs), including, but not limited to: housekeeping; food preparation and food storage, laundry and transportation. Medication monitoring and administration (with training provided by nurse). Implements behavior plan to help people with identified target behaviors. Completes and submits required documentation legibly and accurately within approved timelines. Provides proper notifications to Supported Living Services of Critical Incidents involving consumer(s)/staff and prepares appropriate incident reports. Ensures that all licensing requirements, agency, and Supported Living Services policies and procedures are followed, including all initial and annual training requirements.
