We're hiring for a full-time Provider Extender RN to join our Family Medicine Facility at Perry Hall. What You'll Be Doing Health Solutions Family Medicine at Perry Hall is launching an innovative Provider Extender RN program designed for nurses who are ready to think bigger, lead change, and redefine how care is delivered in a fully integrated medical and behavioral health setting. The Provider Extender RN will be a key player in our multidisciplinary team-working side-by-side with two providers and a medical assistant to advance access, quality, and patient outcomes. This is not a traditional RN role. It's a dynamic, hands-on, productivity-driven position for a nurse who thrives in fast-paced environments, embraces challenge, and puts patients and teamwork first. RESPONSIBILITIES: As a Provider Extender RN under the supervision of the Director of Integrated Healthcare you will be responsible for: Clinical Excellence & Patient Care Conduct patient check-ins, nursing assessments, and intakes with thorough documentation. Perform direct patient care services, including vital signs, immunizations, injections (IM/SC), venipuncture, and point-of-care testing under standing orders and SOPs. Administer medications as ordered, including injection-based therapies and vaccines. Assist patients with medication management, including pill box organization and education on safe medication use. Conduct behavioral health screenings (PHQ-9, GAD-7, AIMS, Daily Living Assessments). Witness' and collect urine samples. Provide case management, supportive therapy, and disease management follow-up to a caseload of physical/behavioral health patients. Facilitate lab work, review results, and bring significant findings to providers' attention. Coordinate care with pharmacies, community agencies, specialists, assisted living, and long-term care facilities. Support integrated behavioral health and multidisciplinary team meetings. Supports and includes family members and others involved in the care of clients. Perform care coordination and outreach as assigned. Crosstrain and provide coverage for other clinic staff, as needed. Administrative & Documentation Excellence Check patients in and out, ensuring accurate capture of visit data and billing compliance. Maintain complete, timely, and accurate records with 100% compliance to Health Solutions' documentation standards. Always uphold patient confidentiality and privacy. Maintain proficiency in the electronic health record and data entry workflows. Receive and accurately record provider orders; to assure accurate and timely medication orders are met. Participate in daily huddles, staff meetings, and multidisciplinary case conferences. Support other departments as needed and cross-train in allowable duties per licensure. Perform additional job-related duties as assigned by the Supervisor, Director, or Vice President of Medical Services. Program Development, QI & Innovation Identify workflow and process improvement opportunities-then lead the change. Develop, implement, and maintain standard operating procedures (SOPs) and best-practice workflows. Lead or co-lead at least one quality improvement (QI) project during the pilot period. Collaborate with providers and leadership to expand care models (group visits, education sessions, chronic disease programs). Partner with behavioral health and data teams on outcome measurement, quality reporting, and population health initiatives. Serve as a role model for ownership, adaptability, and Cover & Move teamwork principles.
