Responsible for informing the community and creating awareness about Borinquen Medical Centers (BMC) services, including: substance use, mental health, HIV prevention, and prevention education on the risks of substance misuse, HIV/AIDS, and STDs. The Behavioral Health Outreach Specialist also conducts outreach in high-risk areas to recruit participants for testing and BMC services, provides needed linkages to service provision for individuals with mental health, substance use and/or co-occurring disorders, and participates in the coordination and facilitation of presentations to educate the community about our services. TASKS AND RESPONSIBILITIES Assist Project Director and Behavioral Health Lead Navigator in SAMHSA annual reports, continuation applications, etc. for Behavioral Health Department. Essential Duties and Responsibilities Provide quality direct behavioral healthcare to patients and family members Provides/ conducts psychosocial assessments and screening to assists the patient in utilizing the support systems and community resources available Provide crisis intervention services to patients as needed Provide brief counseling support to identified patients Support the medical treatment plan to support patients with improving their chronic health conditions Demonstrates critical thinking skills by performing appropriate interventions Identifies the impact of cultural diversity and lifestyle choices on patient condition and makes accommodations for such differences Assess, plan and implement individualized treatment plans for patients engaged in behavioral health services Build and maintain relationships with caregivers and healthcare providers to support patients in addressing the physical and psycho-social barriers to optimal outcomes. The Licensed Behavioral Health Therapist (LCSW / LCMHC / LMFT)) is responsible for counseling clients; helping them gain access to various resources and assisting clients to confront issues that are negatively affecting their way of living such as poverty, abuse, mental health and substance use. ESSENTIAL DUTIES: Performs diagnostic assessments. Interviews individuals and groups to assess needs, as well as develops appropriate response plans for their clients. Provides information and referrals for counseling and service acquisition as appropriate to the assessed need. Provides consultation services. Initiates treatment interventions; e.g., individual, family and group counseling and crisis intervention. Helps people in accepting various situations with emphasis on events that can't be changed.
