The House Manager (Residential Habilitation Manager - RHM) - Tier III is responsible for assisting the individuals we support with day-to-day maintenance of his/her home and participation in community activities other than primary day activities (i.e. day treatment and supported work). The RHM supervises staff that provide the supports and skill training specified in the individualized residential habilitation plan.

At least 1 year experience in a supervisory capacity
Previous manager experience (this role is not an entry level manager) and the ability to display strong leadership skills is required.

Living Resources is seeking Direct Support Professionals for our Albany, Rensselaer, Schenectady & Saratoga County Group Homes. Our Direct Support Professionals support individuals in our Group Homes with their daily activities, including personal care. The Arts Instructor (AI) will assist each student to achieve his/her maximum growth through art-based activities. The majority of the AI workday takes place floating in either the Albany, Schenectady, Malta or Saratoga Springs arts studios, but also involves overseeing the artists activities at community based art exhibits and galleries. The AI will be expected to be a role model by using correct social interactions in both classroom and community environments. The AI must exercise good judgment in organizing and prioritizing responsibilities as well as have strong problem-solving skills while working with up to eight participants per class. It is essential that the AI establishes a group atmosphere conducive to habilitation and art instruction while at the same time accommodating the well-being and safety needs of each student. This position also involves daily record keeping and frequent contact with both family members and residential /day program staff.

The Arts Instructor should have no driving or physical restrictions which might prohibit or interfere with ones ability to provide transportation to program participants as scheduled and/or as needed. The Art Instructor should also be opened to traveling and working at all 3 locations (Albany, Schenectady and Malta).
Requirements
4-year college degree preferred/strong artistic background with proven record of art instruction and production.
Background in working with persons with developmental disabilities or related experience.
Course work/experience in a variety of art mediums.
Valid NYS drivers license and reliable transportation required.
Excellent communication, organizational, problem solving and leadership skills and the ability to be flexible and work independently.

This Case Manager-Day Habilitation is based out of the Schenectady office.
The Case Manager-Day Habilitation is responsible for the development and review for all Day Habilitation Plans and "Outcomes" at our Day Community Opportunities Program.
The Case Manager will also ensure "Outcomes" are being carried out according to the Day Habilitation Plan and will act as an advocate for all individuals enrolled.
Job Responsibilities:
* Develop and write Day Habilitation plan for each Participant.
* Rewrite the Day Habilitation plan to reflect changes.
* Coordinate and review the Participant's plan with the family, Service Coordinator, and Participant on a semi-annual basis to discuss progress/regression and overall performance within Day Habilitation.
* Communicates with the Participant and/or provider to ensure that the Day Habilitation plan of service meets the Participant's needs and desires; supplies guidance or direct intervention when it does not.

The Career Counselor II provides vocational counseling, job coaching, and individualized support to help individuals identify and achieve their career goals. This position involves collaboration with employers, families, and other agencies, which requires local travel in the Capital Region. Candidates must be organized, adaptable, and able to manage a dynamic schedule based on client needs. What You'll Do:
* Manage a caseload of individuals receiving OPWDD and/or ACCES-VR services
* Assess individuals' interests, strengths, and employment goals using person-centered planning
* Provide vocational counseling & guidance tailored to each individual's abilities and career interests
* Conduct Community-Based Work Assessments
* Provide Pre-Employment Transition Services and/or support clients participating in OPWDD's Employment Training Program
* Assist individuals in developing job-readiness skills and preparing for interviews and job searches

Job Content:
Meal preparation in accordance with prescribed diets and consistencies in accordance to the planned menu. Communicate any dietary concerns/information to the dietitian as needed. Shopping for food and supplies as needed and maintaining a clean, safe and sanitary kitchen and dining area, which includes kitchen equipment, in accordance with the agency standards and regulations. Additionally, the Cook will be responsible for maintaining a clean and organized pantry/food storage area. Other duties may include inventory checks, attending meetings and trainings as needed and menu planning.
The Cook will be assigned a minimal amount of personal care responsibilities , but may need to play a greater role if program is short-staffed.
On average, the Cook with spend approximately 75% of the scheduled shift engaged in meal preparation and 25% with Direct Support Professional responsibilities.
Requirements
The candidate should have some cooking experience and organizational skills. The ability to learn, follow a menu and work independently is also required. The candidate needs to be a self-starter, able to create a menu and shop accordingly within the budget. Experience working with people with disabilities is a plus.
High School Diploma/GED is required. Proof will need to be provided prior to employment.
Valid NYS Driver's license is required.
