Staff working under the WISEWOMAN grant work closely with NARA's Chronic Disease Program which includes the Breast & Cervical Cancer Early Detection Program, the Diabetes Treatment and Prevention Program and the Colorectal Cancer Control Program. Individuals in this position are responsible for guiding program participants through a goal making process while involving lifestyle modifications that will lower cardiovascular disease risks. This position will serve as a subject matter expert on hypertension control and cardiovascular disease risk reduction. This will include outreach, scheduling, counseling, tracking and reporting outcomes of the WISEWOMAN Healthy Behavior Support Services and related clinical measures. The Lifestyle Coach will assist in creating, conducting, and implementing health promotion activities such as Diabetes Prevention Program classes, walking programs, cooking presentations, and employee health activities. They will also assist with training clinical staff in obtaining accurate blood pressure readings. An ideal candidate will be someone who role-models wellness. The Lifestyle Coach will be responsible for a panel of patients and, in collaboration with primary care providers, help patients meet their preventive, chronic and acute care needs related to CVD prevention. This will include completing nutrition assessments, providing nutrition counseling, individual health coaching, and group health education. Part of engaging patients and encouraging them to take an active role in their health will involve providing necessary tools to make healthy lifestyle choices and adopt lifelong healthy behaviors.
Duties:-Serve as a subject matter expert on hypertension control and cardiovascular disease risk reduction. -Assist with the training of clinical staff to obtain accurate blood pressure readings. -Complete nutrition assessments and provide nutrition counseling, individual health coaching, and group health education (including DPP, DSME, SMBP group facilitation). -Take and evaluate patient vitals including blood pressure, capillary blood glucose, weight, height, and point of care lab measures including A1C and Cholesterol. -Provide CVD Risk Assessment, health education, resources and tools to help patients establish sustainable lifestyle risk reduction.
Qualifications:-Master's Degree -Registered Dietitian with a license to work in Oregon and Washington, or eligible to apply for an Oregon/Washington dietitian license -Experience working in CVD and diabetes management and prevention coordinating health behavior change and providing health coaching and health education. -Proficient computer skills including Microsoft Office Excel, Word and PowerPoint. -Flexible schedule - position requires some travel, evening, and weekend work. -Must have access to a reliable vehicle and a valid drivers' license and be willing to travel between 2-3 different sites within the Portland Metro area. -Experience working with the Native American /Alaska Native community is preferred. -Experience working as a Lifestyle Coach with the National Diabetes Prevention Program is preferred.

Duties:Assist the Medical Director, to assure that the NARA NW Indian Health Clinic's medical services are in full compliance with standards, ethics and license requirements of the medical profession. This may include supervision of medical students, coordination of locum tenens physicians, contractual physicians, and development of linkages with non-allopathic health care providers such as naturopathic doctors, acupuncturists or herbalists. Engage in collegial consultative relationship with NARA NW Nurse Practitioners, other physicians, mental health practitioners, registered nurses, dieticians, Residential Treatment Center staff, and Outpatient Treatment Center staff. Participate as a member of the clinic's management team. Establish consultative referrals network and coordinate linkages with contractual pharmacy, hospital-based admitting physicians, and reference labs. Provide comprehensive primary health care services defined as holistic health care which the client receives at the first point of contact with the health care system and in continuous and comprehensive and included the following; health promotion; prevention of disease and disability; health maintenance; rehabilitation; identification, diagnosis and management of health problems; and referral to other providers as appropriate. (Oregon Revised Statutes, Chapter 678, Division 50) Provide clinical treatment of acute and chronic illnesses and minor injuries. Treat superficial lesions such as warts, cysts, minor lacerations, abrasions, minor first and second-degree bums within the physicians scope of practice. Duties:What you will do: -Works closely with Triage Nursing Team in monitoring incoming calls taking lead on all calls related to Psychiatry. -Promotes clear communication amongst the care team and treating clinicians, including, Nursing, prescribing Psychiatric professional and primary care Providers. -Takes crisis calls and consults with Psychiatric providers, and Mental Health Specialist as needed. -Performs Chart Reviews of clients scheduled to be seen by a psychiatric provider and ensures that the clients are following through with their Plan of Care. -Coordinates labs, EKG's, appointments, psychiatric medication follow up, and assists in the overall management of Psychiatric patients, particularly those with chronic diseases, and those at risk of psychiatric hospitalization including administration of long acting injectables as ordered by prescriber. -Contacts patients to report changes in medications or plan of care. -Coordinates prescriptions as needed. Assists psychiatric providers with Prior authorizations from insurance companies, as needed. -Provides education, and/or self- management goal setting as written in plan of care. -Does routine and timely documentation in clients EHR of all core process work activities including, but not limited to, evidence-based chart reviews, patient calls & correspondence, and patient education activities. -Coordinates continuity of patient care with external healthcare organizations and facilities, including the process hospital admission and discharge, working closely with the Psychiatric mental health intake specialist.
Qualifications:We would like to hear from people that have: -B.S.N. equivalent in experience, education or degree. -At least three years nursing experience, part of which has been in care coordination and psychiatric nursing preferred -Qualified Mental Health Professional (QMHP) certification through MHACBO preferred -Must possess a current license to practice as a Registered Nurse in the State of Oregon. -Must have current CPR certification -Phlebotomy, and EKG ability preferred -MHP certificate preferred -Current driver's license with a clean driving record, and proof of auto insurance. -Ability to receive direction and independently follow tasks through with minimal supervision. -Ability to operate basic office equipment necessary to perform duties: fax, copy machine, computer, and multi-phone lines. -Ability to uphold attendance and performance standards. -Must pass a pre-employment or for cause drug tests. -Must pass criminal background and DMV checks.

Main Duties:

Read and strictly follow the In-Home Usage Test Daily Schedule provided with each product testing project (may include tasks such as unpacking, reading instructions, journal entries, online or mobile feedback, usage of product for a certain amount of time, writing reviews, taking pictures, etc.)
Some In-Home Usage Tests projects may require participants to use MFour's Mobile In-Home Use Test Technology (cutting-edge smartphone technology to capture Point-of-Emotion insights to gain unparalleled depth of responses)
There are times when the product being tested may be discussed in a private chat room that is opened by a market research firm
Write reviews as requested in the In-Home Usage Test Daily Schedule for each project

DNA is seeking to hire an individual for our open New Mexico Senior Attorney position located in our Farmington, New Mexico Office.
*REQUIREMENTS:* Senior Attorney must be a graduate of an accredited law school and a member of the New Mexico bar, or if licensed in another jurisdiction, able to gain admission to the New Mexico Bar within one year by motion or reciprocity. Admission to the Arizona or Utah bar is a plus, as is admission to the Navajo, Hopi, or Jicarilla Tribal Court bar. Must have at least five (5) years of experience as an attorney in a legal aid organization or similar non-profit law firm with strong litigation skills; strong oral and written communication skills; the ability to travel and work throughout the DNA service area; competence in working with diverse individuals and communities, especially with Native Americans, persons of color, and other marginalized communities; a commitment to providing legal services to the poor; the ability to identify and successfully pursue strategic, systemic, and affirmative advocacy; good judgment, the ability to handle stress, take initiative, and have a willingness to work as a team; and the ability to manage and supervise others, including the ability to mentor other staff and law students. *RESPONSIBILITIES INCLUDE: *
