Job Summary:

Works periodically on holidays and during special events as instructed by Director of Life Enrichment.
Possesses an understanding of the social, psychological, physical and recreational needs of the geriatric population especially those experiencing cognitive decline.

Job Summary:
This position is primarily responsible for providing psychosocial needs and services to residents in the Health Center which included Skilled Nursing, Assisted Living, and Memory Support Assisted Living. Key Responsibilities
Admission
Oversee the admission process of new residents to include move-ins, chart reviews, and resident assessments.
Meets with visitors on a need be basis.
Responds to telephone and fax contacts.

Aids residents and families through individual or group conferences to understand, accept, and follow medical recommendations.

Completes mental illness report and maintains resident chart to include social assessment, social history, MIMR, and discharge plans.
Coordinates with staff to see the psychosocial and emotional needs of residents are met and documents the same.
Discharge Functions
Assists with discharge of residents from Healthcare Center and completes all necessary discharge paperwork.
Meets with family and resident at the time of discharge.
Discusses discharge procedures and additional course of action for the resident.
Conducts final assessment of resident and completes discharge paperwork in conjunction with the other disciplines.
Regulatory Action
Maintains regulatory functions for all resident units within the community and ensures proper compliance to those regulations.
Maintains a working relationship with Department of Health Services and other agencies and remains in contact with specified agency as appropriate.
Transfers residents within units and works to ensure requests made by the resident are in compliance with all regulatory agencies.

Identifies resident problems in emergency situations and initiates immediate measures as appropriate to the situation.

Counts narcotics and follows emergency kit procedures.
Assists with admission/transfer/discharge procedures, coordinating with other departments, physicians, and ancillary service providers.

Assisted in transitioning to various levels of care including assessments for services, resident transition, and family guidance.
• Maintains contact with clinics to receive information on new residents moving into the community. Inform staff of new admission as need be.
• Works in conjunction with the clinic to provide new resident assessments (i.e., UAI) and meets with the family to discuss services to be provided.
• Completes mental health screening of new residents with the clinic to ensure completion upon move in day.
• Creates resident chart to include UAI and other pertinent information regarding resident physical and mental health.
• Coordinates move-in procedures with residents, family, and staff to ensure resident transition is as smooth as possible.
• Meets with family prior to moving in and within 24 hours following admission.

Scheduling & Admin Responsibilities:
