Under the direction of the Integrated Transition Care Department Leadership, the incumbent will work with Hospitals and Independent Physician Association (IPA) with their assigned Team Center, that may include an LCSW, CHW, Pharmacist, Pharmacy Technician, Coordinator, Care Transition, and Transportation to initiate coordinated and continuous cost-effective quality healthcare to ensure the continuity of the Member's care needs are met timely throughout the continuum of care and readmission prevention for our high-risk Members. This will be accomplished through concurrent review processes using nationally recognized criteria. The Integrated Transitional Care Nurse, RN will participate in providing transitional care services to the Members with each transition from one care setting to the next.

Key Responsibilities
Conduct Integrated Transition Care discharge risk assessment for high risk/ high acuity Members.
Oversee and perform concurrent and retrospective reviews for medical necessity per evidenced based criteria, appropriateness of service and level of care, either through Telephonic review, clinical documentation submitted by respective facilities and/or electronic medical records (EMR) access.
Conduct reviews to ensure Member's treatment plan is consistent with Diagnosis(es), specifically initial review within twenty-four (24) hours to ensure Members meet specified criteria for the respective admission(s).
Work in collaboration with the coordinator to ensure timely arrangements for transitions to higher or lower level of care and assist with transfer orders as needed.
Ensure the concurrent or retrospective review process include referring cases that require clinical consultation with the medical director in a timely manner.
Ensure cases are appropriately referred to Care Management, Behavioral health, Health Education, Housing, community health.
Identify outliers and prepare documentation as well as report on potential quality of care issues as identified.
Process timely completion of denials process per policy. The RN will review variances in cases followed by an LVN for possible consultation with the Medical Director to determine approval for due to medical necessity or denial of days.
Conduct timely compliance and completion of documents per regulatory requirements.
Serve as the IEHP Liaison between hospitals, IPAs vendors, outside agencies and provider to ensure effective communication and collaboration in an effort to meet the Member's treatment plan and goals.
Oversee verification of delivery of ordered Durable Medical Equipment (DME), Health Home visits, set up hospital follow up visits with PCP, review of Medications with Member and transportation needs.
Oversee and complete Discharge planning assessment.
Perform medication reconciliation based on acuity and type of medications.
Outreach to Member post discharge to ensure Member attended follow up appointment, Health Home starts visits, possible in-home visit by RN or assigned LCSW as appropriate.
Generate additional follow up needed to assign to appropriate regional Behavioral Health & Care Management team.
Qualifications
Education & Requirements
Two (2) or more years of Utilization Management/Case Management in a health care delivery setting in Acute or Skilled nursing, with an emphasis on Concurrent Review and Utilization Management
High school diploma or GED required
Bachelor's degree in Nursing from an accredited institution preferred
Possession of an active, unrestricted, and unencumbered RegisteredNurse (RN) license issued by the California BRN required
Key Qualifications
Must have a valid California Driver's license and valid automobile insurance. Must qualify and maintain driving record to drive company vehicles based on IEHP insurance standards of no more than three (3) points
Knowledge of evidence based clinical criteria and California Children Services (CCS)
Must have analytical skills. Excellent communication, interpersonal and organizational skills
Self-starter and a team player
Time management, and problem-solving abilities
Must have a high degree of patience
Position may require working onsite at an IEHP contracted provider facility location 5-days per week as a remote work location depending on business need

Under the direction of the Supervisor/Manager, this position is responsible for triaging incoming calls, assessing needs and providing appropriate healthcare options including facilitating referral to primary providers, health care facilities and community resources. Required Qualifications:

Reporting to Director of Hospital Relations, the Quality Improvement, RN will facilitate quality improvement initiatives across the continuum of care (i.e. acute care hospitals, skilled nursing facilities, hospice agencies, etc.) to enhance the overall quality of care delivered to members. This position is responsible for identifying, developing, coordinating, promoting, supporting, and implementing assigned clinical quality, and/or service improvement projects and programs. The Quality Improvement, RN supports evidence-based practice integration, provider training/education, policy review/development, and regulatory/accreditation interpretation. The incumbent develops continuous and targeted activities to improve performance related to Centers for Medicare & Medicaid Services (CMS) Star Ratings, HEDIS, CAHPS, HOS, P4P and other identified performance measures. The Quality Improvement, RN provides direct support for multiple initiatives aimed at improving the quality and effectiveness of health care delivery and/or health care services. This position communicates with external partners, internal quality leaders, and stakeholders to ensure multi-disciplinary teams have the resources and information they need.

Key Responsibilities
Participate in departmental and cross-functional teams; this includes supporting IEHP and external stakeholder workgroups that focus on quality improvement projects. Assist in tracking progress for improvement efforts related to Hospital value-based incentives (P4P, VBI, VBP, etc.), network-wide CMS star ratings, and specific performance metrics.
Coordinate activities to foster good working relationships with contracted hospitals, skilled nursing facilities, hospice agencies, providers, and other external partners to encourage their participation in quality improvement groups. Help organize forums for shared learning and best practice exchange to improve patient care outcomes.
Apply knowledge of clinical processes or hospital operations to support the development and implementation of specific activities (based on current results and best practices) intended to improve CMS Star Ratings, HEDIS, CAHPS, HOS, P4P, and other targeted performance measures.
Gather and compile information on evidence-based practices, national and regional benchmarks, and industry standards, and help integrate those findings into current improvement initiatives.
Track and monitor improvement activities by performing ongoing analysis of measured outcomes and reporting findings.
Compile and summarize performance outcomes data and draft initial feedback for review by senior staff before distribution to external partners regarding improvement progress.
Utilize performance improvement methodologies (i.e., Plan Do Study Act (PDSA)) to support the implementation and tracking of rapid-cycle improvement projects.
Participate in outreach initiatives, and document potential barriers, experienced challenges, and activities taken to resolve issues and improve outcomes.
Assist in the maintenance and organization of annual quality improvement program documents, compliance audits, policies and procedures, and related evaluations.
Support the development and delivery of materials for quality improvement training sessions and problem-solving tool workshops for QI teams and stakeholders. Lead intradepartmental quality improvement teams, programs, projects, and initiatives as assigned.
Continuously seek to enhance delivery and efficiency of external-facing quality improvement initiatives via technological advancement and internal-facing process improvement.
Translate data into meaningful information, draw conclusions, and relate findings to industry standards. Provide recommendations to departmental stakeholders and Quality Improvement Leadership Team regarding performance gaps, program implementation, and performance outcomes in order to maintain an effective Quality Improvement program.
Perform any other duties as required to ensure Health Plan operations and department business needs are successful.
Qualifications
Education & Requirements
Minimum four (4) years of clinical/health care experience, including at least two (2) years performing quality improvement analysis or project coordination functions required
Bachelor's degree in science or health/healthcare related field from an accredited institution required
Master's degree in science or health/healthcare related field from an accredited institution preferred
Certified Professional of Healthcare Quality (CPHQ), or other recognized quality improvement certificate required or obtained within one (1) year of hire (maintained through employment)
Possession of an active, unrestricted, and unencumbered RegisteredNurse (RN) license issued by the California BRN required
Key Qualifications
Working knowledge of healthcare quality performance and measurement sets including CMS quality withhold, Managed Care Accountability Set, NCQA Quality Improvement Standards and HEDIS
Ability to apply knowledge of tools of continuous improvement, and work process redesign
Knowledge of advanced clinical processes and/or hospital operations
Requires knowledge of the local healthcare market, patient demographics and needs and plan-specific information
