In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for providing assistance in dispensing prescription medications to patients. Responsibilities * Provide assistance to the pharmacist in dispensing prescription medications: typing orders, billing orders, performing quality control functions and inventory management, including 340B program management, placement and receiving of medication orders from multiple sources, out-of-date medication management and processing, hazardous waste management, and maintenance of the medication take back kiosk. * Prepare prescription medications for delivery to patients and perform prescription tracking using pharmacy systems, electronic medical records and collaboration with internal and external pharmacy teams to ensure timely medication delivery. * Contact patients to ensure medication delivery and return medications to stock per established pharmacy procedures. * Enter prescription and patient information into pharmacy database and determine patient eligibility for pharmacy benefits, including but not limited to interim coverage programs, copay assistance programs, charity care (MAP/Basic), ADAP Program, Ryan White, patient assistance programs, 340B eligibility including referral management, grants management and pharmacy pilot programs. * Coordinate with Enterprise Pharmacy Benefits and Patient Assistance Program teams to ensure patient coverage and access to medications including facilitating charity program "prior authorization" management and follow-up communication with patients and providers. * Provide excellent internal and external customer service in-person and electronically, answering questions verbally, in writing, in emails, via secure text messaging, and within the electronic medical record; provide patient and staff navigation of pharmacy services, including referral to other departments such as eligibility or medical, and provide coordination with external pharmacy partners, to ensure patient access to their medications. * Register Management: Open and close registers, maintaining accurate transaction records. * Multilingual Patient Support: Identify patient language preferences and connect them with appropriate translators for effective communication. * Use translation services for prescription typing and clear patient communication. Under direct supervision of a pharmacist, the Intermediate Pharmacy Technician accurately prepares and labels medications (including sterile compounds, such as chemotherapy, high risk and batch preparations), fills automated dispensing devices, squad boxes, code trays and replaces floorstock. The Intermediate Pharmacy Technician fills and exchanges patient-specific medication supplies, monitoring for returned and discontinued medications. In addition, the Intermediate Technician functions routinely in specialty positions (Precepting, Procedural Area inventory/replenishment, Controlled Substance security and inventory). The technician is responsible for maintaining a safe and clean work environment and communicating in support of Valley Health values. Duties include: Sterile Compounding - The Intermediate Technician is able to competently perform all steps of chemotherapy, high risk and batch product preparation, including selection, compounding and appropriately labeling ordered products for check by a pharmacist, prioritizing as needed. Prepares and labels adult, NICU and pediatric sterile preparations as needed to fulfill patient orders and avoid stock-outs. Documents all compounding and preparation activities as required prior to check by a pharmacist. Follows procedures stipulated in USP 797, USP 800 and Pharmacy policies and procedures for sterile compounding. Consistently compounds products accurately, with close attention to stability and cost issues. Successfully demonstrates competency for sterile compounding annually. Completes all required cleaning and quality assurance processes and documents appropriately. Fulfills Responsibilities of at Least Two Specialized 2 Shift Assignments (C-2 SAFE/Controlled Substances, Procedural Area, or Preceptor) - Accurately and completely fulfills job responsibilities as outlined in workflows or orientation checklists. Mentors and assists staff for increased efficiency and service to patient care areas. Order Processing and Drug Distribution - Continually selects and appropriately labels ordered medications for check by a pharmacist, prioritizing as needed. Prepares oral adult, NICU and pediatric preparations into unit dose packaging as needed to fulfill patient orders and avoid stock-outs. Documents all compounding and prepacking activities as required prior to check by a pharmacist. Answers all telephone calls within specified time period, using appropriate customer service skills. Triages calls for technician or pharmacist intervention and follows through in an appropriate manner. Verifies correct supply of medications and intravenous medications delivered to the patients' respective nursing unit daily. Screens for expired or discontinued medications, drugs requiring transfer or doses still to be administered that should remain on the unit. Arranges activities to meet designated turnaround times for STAT and routine orders. Manages robot and Pyxis fills, completing scanning process to assure accurate doses reach the patient and maintain an accurate inventory level. Completes processes within designated times and facilitates pharmacist checks. Resolves or reports all discrepancies. Completes risk reports to improve processes. Communication - Continuously communicates workload status to fellow team members. Keeps the pharmacist-on-duty informed of all workload issues as well as customer concerns. Coordinates with pharmacist-on-duty all work and meal breaks. Effectively communicates workload status between shifts. The Pharmacy Technician at Valley Pharmacy embraces and utilizes customer service standards adopted by Valley Pharmacy. Executes cash register transactions, to include cash, credit card and check transactions, with minimal errors. Maintains adequate cash levels for daily store use by proactively monitoring till levels throughout the day. Remains readily available and eager to assist at both pick-up and drop off areas. Performs the Discharge Technician duties to provide coverage when needed; this includes serving as a discharge prescription liaison for communication and filling needs of the program when needed. Responsible for inventory duties that include maintaining stock areas and workstations in a neat and orderly manner. Documents and communicates products to be ordered. Performs cycle counts as assigned correcting inventory on-hand quantities and verifying discrepancies. Reviews inventory stock and selects correct medications per prescription instructions. Regularly communicates workload status to fellow team members including the pharmacist on duty to ensure workflow efficiency. Coordinates with pharmacist-on-duty for all work and meal breaks.
