MAJOR FUNCTION:
The Chemotherapy Infusion Pharmacist is responsible for providing dedicated pharmacy support to the Outpatient Infusion Center, with a primary focus on medication order verification, sterile compounding oversight, and dispensing of chemotherapy and infusion therapies. This role emphasizes pharmacy operations and safety processes while providing limited but essential clinical support such as medication management, supportive care, patient education, and collaboration with infusion staff and oncologists. The pharmacist ensures high standards of accuracy, compliance, and efficiency in all aspects of infusion pharmacy services, while serving as the pharmacy liaison to the infusion center team.
QUALIFICATIONS:
Education & Experience:
Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy required.
Completion of a PGY-1 Pharmacy Residency or equivalent 2 years relevant hospital pharmacy experience, hospital/infusion experience preferred.
Minimum of 2 years hospital pharmacy or infusion pharmacy experience preferred.
Experience with chemotherapy order verification, sterile compounding, and USP / compliance strongly preferred.
Certification/ Licensure:
Pharmacist license NJ required.

The Clinical Pharmacist helps develop and implement Monogram's medication therapy management (MTM) program which will ensure appropriate use of medication management resources. Roles and Responsibilities
Perform and write concise member specific comprehensive medication reviews (CMRs); productivity and quality standards apply.
Drive meaningful medication interventions that result in medical providers' and members' adhering to evidence-based medication guidelines
Review member care reports, clinical notes and other medical records as needed for medication action plan development, targeted clinical outreach programs and quality assurance purposes.
Serve as a pharmacist resource for Monogram field clinicians regarding medication use and Monogram MTM resources.
Assist with the development and delivery of MTM training sessions aimed at building a standardized and effective MTM program for Monogram employees.
Assist with the development and maintenance of MTM materials for internal and external provider and member use.
Monitor important medication availability and use trends for populations under Monogram's care management. Stay current with evidence-based prescribing for individuals with kidney disease.
Participate in external and internal interdisciplinary care team case rounds or meetings and produce MTM related documentation to be shared with appropriate treating providers.
Implement medication optimization processes that meet the needs of managed care organizations and their members.
Support Monogram as necessary with any tasks required to deliver excellent personalized kidney care and perform all other duties as assigned

Bachelor's degree in pharmacy or PharmD in FL
Current pharmacist license

Provide patient education on medication and reason for use, frequency and dosages, side effects, and desired therapeutic effects. Enhance educational experience using drug information flyers or pamphlets appropriate for patient level of health literacy.
Fills in-house generated prescriptions in accordance with all federal regulations and all state regulations as per the NJ Board of Pharmacy.
Fills outside provider prescriptions in accordance to OHI Outside Prescription Policy in addition to all federal and state regulations as per the NJ Board of Pharmacy.
Serves as a resource of information on medications, pharmacokinetics and interactions, to providers.
Assist providers in the management of patients with polypharmacy including conducting medication reconciliation, writing medication lists, completing the patient medication card, and providing other means for patients to understand how to take their medications.
Works in the CEC periodically to assist providers with requests from pharmacies regarding therapeutic interchanges, questions regarding prescriptions and continuity of care to ensure patients get prescriptions in a timely manner.
Work collaboratively with the Coumadin Management Team to evaluate, assess and assist patients with their INR once notified of results by nurse. Then update Coumadin Management Flowsheet as required and renew warfarin prescription to ensure patient has sufficient supply.
Works collaboratively with referring primary care providers for the management of smoking cessation patients. Pharmacist will call patients are needed when due for follow-up and ensure patient has sufficient supply of smoking cessation aid.
Conduct prospective and retrospective chart reviews on selected patients and discuss findings with providers and/or Chief Medical Officer.
Maintains confidentiality of all pertinent information to assure that patient and staff rights are protected.
Assist in the identification of potential adverse drug events and in recording such events according to policy and procedures.

Fills in-house generated prescriptions in accordance with all federal regulations and all state regulations as per the NJ Board of Pharmacy.
Fills outside provider prescriptions in accordance to OHI Outside Prescription Policy in addition to all federal and state regulations as per the NJ Board of Pharmacy.
Serves as a resource of information on medications, pharmacokinetics and interactions, to providers.
Assist providers in the management of patients with polypharmacy including conducting medication reconciliation, writing medication lists, completing the patient medication card, and providing other means for patients to understand how to take their medications.
Works in the CEC periodically to assist providers with requests from pharmacies regarding therapeutic interchanges, questions regarding prescriptions and continuity of care to ensure patients get prescriptions in a timely manner.
Work collaboratively with the Coumadin Management Team to evaluate, assess and assist patients with their INR once notified of results by nurse. Then update Coumadin Management Flowsheet as required and renew warfarin prescription to ensure patient has sufficient supply.
Works collaboratively with referring primary care providers for the management of smoking cessation patients. Pharmacist will call patients are needed when due for follow-up and ensure patient has sufficient supply of smoking cessation aid.
Conduct prospective and retrospective chart reviews on selected patients and discuss findings with providers and/or Chief Medical Officer.
Maintains confidentiality of all pertinent information to assure that patient and staff rights are protected.
Assist in the identification of potential adverse drug events and in recording such events according to policy and procedures.
Perform literature based research and medication formulary reviews. Assist in development of patient education materials and literature that enhances the clinical pharmacy services provided, in language and at health literacy level of the patient population.

Utilizes PBM help desks to address any 3rd party prescription rejection claims.
Designates appropriate tasks and projects to pharmacy technicians and holds techs accountable for completion.

Oversees management and confidentiality of patients' personal medical data and other personal identifiable information and provides appropriate training to pharmacy staff.
Ensures all services are in compliance with professional standards, state, and federal regulatory requirements.
Maintains a safe and clean working environment by complying with procedures, rules, and regulations.
Maintains pharmacy inventories, including management of virtual inventories.
Reviews prescription orders for completeness, accuracy, and appropriateness.
Enters data, such as patient name, prescribed medication, and cost, to maintain pharmacy files, charge systems, and inventory.
Prepares and dispenses prescribed medications, drugs, and pharmaceuticals for high quality patient care.
Compounds medications using standard formulas and processes such as weighing, measuring, and mixing ingredients.
Directs pharmacy personnel engaged in mixing, packaging, and labeling pharmaceuticals.
Answers healthcare professional and patient drug information requests.
Provides counseling to patients regarding ordered medications, including, but not limited to, indication, directions for administration, potential side effects and interactions, expected time to desired effect, and appropriate storage.
Reviews department of health stock inventory and orders as completed by the pharmacy technicians for accuracy.
Reviews prescription renewal requests and authorizes or denies requests according to the renewal policy.
Maintains pharmacological knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional societies.

REQUIREMENTS:
EDUCATION & EXPERIENCE:
Graduate of an ACPE-accredited school of pharmacy with a B.S. or Pharm.D. degree required
Prior experience working in a community pharmacy and/or ambulatory care setting preferred
Experience working in an urban setting with racially and culturally diverse population preferred
Experience working in a low-income community preferred
LICENSURE AND/OR CERTIFICATIONS:
Licensed pharmacist in the state of New Jersey (or eligible and able to obtain prior to start date)
KNOWLEDGE, SKILLS, ABILITIES AND OTHER (KSAO's)
Bilingual English/ Spanish preferred

Clinical Pharmacist Job Responsibilities:
Interprets, prepares, and verifies medication orders as appropriate.
Provides clinical consultation and cost-effective alternatives.
