Clinical Pharmacist The Clinical Pharmacist helps develop and implement Monogram's medication therapy management (MTM) program which will ensure appropriate use of medication management resources. The Clinical Pharmacist will perform timely written member medication reconciliations and drug therapy action plans for all Monogram members and communicate them to Monogram clinical staff and treating nephrologists, primary care physicians and other providers. He or she conduct telephonic consultations with medical providers when medication changes appear necessary and speak with members about medication use concerns. The pharmacist will work collaboratively with clinical (e.g. nurse and social worker) care manager operations personnel to integrate medication management into Monogram's overall care model and to ensure that field clinicians are executing on MTM policy and procedures. Roles and Responsibilities Perform and write concise member specific comprehensive medication reviews (CMRs); productivity and quality standards apply. Drive meaningful medication interventions that result in medical providers' and members' adhering to evidence-based medication guidelines Review member care reports, clinical notes and other medical records as needed for medication action plan development, targeted clinical outreach programs and quality assurance purposes. Serve as a pharmacist resource for Monogram field clinicians regarding medication use and Monogram MTM resources. Assist with the development and delivery of MTM training sessions aimed at building a standardized and effective MTM program for Monogram employees. Assist with the development and maintenance of MTM materials for internal and external provider and member use. Monitor important medication availability and use trends for populations under Monogram's care management. Stay current with evidence-based prescribing for individuals with kidney disease. Participate in external and internal interdisciplinary care team case rounds or meetings and produce MTM related documentation to be shared with appropriate treating providers. Reporting to the Pharmacy Coordinator, the Pharmacist will provide safe and effective pharmacy services in addition to dispensing medications. These services include the provision of pharmaceutical care, medication dispensing, patient counseling, drug information services and patient and staff education. Responsibilities Provide patient education on medication and reason for use, frequency and dosages, side effects, and desired therapeutic effects. Enhance educational experience using drug information flyers or pamphlets appropriate for patient level of health literacy. Fills in-house generated prescriptions in accordance with all federal regulations and all state regulations as per the NJ Board of Pharmacy. Fills outside provider prescriptions in accordance to OHI Outside Prescription Policy in addition to all federal and state regulations as per the NJ Board of Pharmacy. Serves as a resource of information on medications, pharmacokinetics and interactions, to providers. Assist providers in the management of patients with polypharmacy including conducting medication reconciliation, writing medication lists, completing the patient medication card, and providing other means for patients to understand how to take their medications. Conduct prospective and retrospective chart reviews on selected patients and discuss findings with providers and/or Chief Medical Officer. Utilizes the Liberty help desk to address any pharmacy software issues that cannot be handled by their immediate supervisor and shares that information with all pharmacy staff. Clinical Pharmacist Job Responsibilities: Interprets, prepares, and verifies medication orders as appropriate. Provides clinical consultation and cost-effective alternatives. Issues controlled substances in compliance according to the DEA and VA Board of Pharmacy requirements. Participates in the medication formulary review and approval process. Monitors drug therapies for interactions, allergies, and appropriateness. Reports adverse drug reactions promptly and accurately. Minimizes non-formulary procurement and supports substitutions. Supervises pharmacy support staff. Participates in continuous quality improvement. Participates in decentralized rounding with clinical teams. May perform additional duties as assigned, including, but not limited to: Spearheading grant work and submission related to the practice of pharmacy Lecturing and facilitating learning opportunities for pharmacy students, PGY1 and PGY2 pharmacy residents, and practitioners Performing duties on local, system, and national committees which contribute to the development and professional advancement of pharmacy practice Serving as a leader or facilitator on a regional or national professional organization. Minimum Qualifications: Education: Doctorate of Pharmacy (PharmD) or Bachelor degree in Pharmacy. Experience: Five (5) years as a clinical Pharmacist. Licensure: Licensed Pharmacist in the state of VA. Certification: Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS) for Critical Care and Emergency Medicine, and Pediatric Advanced Life Support (PALS) within 90 days of hire. MAJOR FUNCTION: To make, facilitate, and execute pharmaceutical buying decisions in order to ensure uninterrupted product flow. To coordinate product availability information externally and internally to ensure that all deliverable dates are met in order to reach facility service level goals. Regularly assess par levels and batching stocks to ensure optimum operational performance of pharmaceutical deliverables. QUALIFICATIONS: Education & Experience: High School diploma or equivalent required. At least one (2) years' experience in healthcare or medical pharmaceutical industry required. Familiarity with materials management/accounting systems and computers required. Certification/Licensure: Valid NJ Driver's license required. NJ Division of Consumer Affairs Pharmacy Technician Licensure required within 6 months of employment in this position. National Certifications for Pharmacy Technicians CPHT from the following agencies: PTCB, NHA, ASHP/ACPE preferred.
