The Housing Case Management Specialist will accomplish this by utilizing Matthew 25's housing programs, HOPWA and TBRA. Determine an individual's appropriateness for care management, medication management or therapy and assign to all that are appropriate. Disposition consumer to appropriate Agency for other mental health services when ineligible for care management. Identify individuals that are in need of MCRT services rather than a routine intake. If appropriate, facilitate crisis action. Assist in resolving any grievance received from a consumer regarding referral. Fill in for Admissions Coordinator as indicated by supervisor. Utilize Intake Committee for all consumers who meet criteria who have a history of violence, sexual crimes or serious criminal activity. Write completed intake and email to Intake Committee for review. Contact consumer with decision.
