Job Title & Role Description:
The Care Specialist is a key member of the team responsible for delivering chronic care management to high-complexity patients. The Care Specialist primarily works in patients' homes and communities (90% of the time) and engages in virtual care (10% of the time). This role involves direct outreach to patients, guiding them through the enrollment process, facilitating virtual appointments with providers, and ensuring that patients adhere to care plans. By building relationships, coordinating care, and providing education, the Care Specialist plays a critical role in promoting health, preventing unnecessary hospitalizations, and improving patient outcomes.
Skills Required:
Minimum of 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant, etc.).
High school diploma or GED required.
A valid drivers license, auto liability insurance, and reliable transportation to travel within the assigned territory.
Experience in chronic care management or working with chronically ill/elderly patients.
Technologically proficient with basic computer skills (typing, using EMR systems).
Experience with motivational interviewing, trauma-informed care, and care coordination.

The Family Advocate in the LYNC Program (Linking Youth to Natural Communities) will be the primary point of contact for bilingual youth and their families throughout the duration of the program. POSITION SUMMARY:
This position reports directly or indirectly to the Manager of Behavioral Health Crisis and the Clinical Nurse Manager. SCOPE OF DUTIES
Under the supervision of the Associate Director if Student Life, Dean of Students, and/or Coordinator of Student Support Services, the Residential Counselor is responsible for the supervision of children and adolescents during school and/or out-of-school hours and the assumption of other responsibilities as needed.
PRIMARY DUTIES
Responsible for the supervision of deaf and hard of hearing children and youth with special needs to include mental health and traumatic backgrounds as well as autism during in or out-of-school hours, including meals.
Planning and carrying out activities designed to promote personal and social growth.
Responsible for encouraging students to maintain good personal hygiene and attire, including bathing and feeding.
Assist students with mobility.
Encourage students to engage in extra-curricular activities.
Implementation of Positive Behavior Intervention Support (PBIS).

Participation in staff training and in-service activities, including Physical/Psychological Management Training (PMT), Trauma Informed Care (TIC), CPR, First Aide, OSHA and other applicable workshops.

Support the Residential Manager by:
>Teaching the staff on implementation of the BSP,

Participate in all Psychiatric Clinic meetings and be the point person for Psychiatrist with RN.
Education, Qualifications and/or Special Training:
BCBA (preferred), or
Master's degree in psychology, special education, social work or a related field (Proof of licensure for Licensed Clinical Social Worker, Marriage and Family Therapist or Professional Counselor) and
Must be able to provide two (2) behavioral support plans that were implemented within the last year. The information in the work samples should be redacted (e.g., removal of identifying names and date of birth) and be specific to individuals with developmental disabilities.

Position Overview
TEEG is seeking a motivated and compassionate 2nd-year graduate student in social work to
serve as a Bridge Counselor. This role involves providing short-term, solution-focused
counseling to youth, acting as "bridge" to connect clients with long-term resources and support
services. This is an excellent opportunity for a graduate student to gain hands-on experience in
clinical social work while making a meaningful impact. This is an opportunity open for either the
first or second semesters of the 2025-2026 academic year. The position will work directly in
TEEGs Youth Service Bureau for the Director and under the guidance of TEEGs clinical
supervisory staff.
Responsibilities
Provide short-term counseling (youth) to address immediate emotional, social, or
behavioral challenges as assigned by the Director
Connect clients with community resources, such as mental health services, housing, or
financial assistance programs
Collaborate with TEEG staff and community partners to ensure seamless service delivery.
Maintain accurate and confidential client records in compliance with ethical and legal
standards
Participate in supervision and team meetings to enhance professional development
Participate in program assistant group work
Perform research concerning social connectivity as case needs may indicate for needed
client resources
Qualifications
Current enrollment in an accredited Masters in Social Work (MSW) program, in the
second year of study.
Strong interpersonal and communication skills, with a commitment to cultural
competence and sensitivity.
Knowledge of crisis intervention, brief counseling techniques, and community resources.
Ability to work independently and as part of a collaborative team.
Familiarity with ethical guidelines and confidentiality requirements in social work
practice.
Prior experience in counseling or case management is preferred but not required.
Reliable transportation and willingness to travel locally, if applicable.
Schedule and Compensation
Hours: Approximately 15-20 hours per week, with flexibility to accommodate academic
schedules as well as the specific hours requirements of your graduate program.
Compensation: There is no monetary compensation, however, successful interns will be
highly considered for future employment. Professional development and practical hands
on experience will be offered.
