The Mental Health Rehabilitative Specialist (MHRS) provides trauma informed, culturally sensitive intensive rehabilitative services to youth. This role focuses on teaching youth social skills, coping skills, emotional regulation, and social integration. The MHRS is a member of a multidisciplinary team that works together to create and implement personalized treatment plans, conduct therapeutic interventions, and support daily activities within the residential treatment setting. This position requires strong rapport-building abilities, knowledge of mental health rehabilitation, and the capacity to work effectively in a structured residential setting.
Qualifications:
Bachelor's degree in social work, psychology, child development or a related field required.
Must be at least 22 years old.
Four years of experience in a mental health setting as a specialist in the field of physical restoration, social adjustment, or vocational adjustment. Up to two years of graduate professional education may be substituted for the experience requirement on a year-for year basis; up to two years of post- associate arts clinical experience may be substituted for the required educational experience in addition to the requirement of four years' experience in a mental health setting.
A valid California driver's license is required of staff who have regular driving duties, and the driving record must meet the requirements of their position and be approved by the agency's insurance company.

Responsibilities:
Provide direct IHBS, ICC and CFT services to clients focused on individualized treatment plan.
Keep comprehensive clinical records that meet standards of the agency, Department of Health Care Services, The Joint Commission, and contract.
Provide individual and group counseling sessions, using evidence-based therapeutic approaches to help residents improve coping skills, emotional regulation, and interpersonal relationships.
Teach and support residents in developing essential life skills such as self-care, decision-making, problem-solving, and communication to promote independence.
Regularly assess and document residents' progress, updating treatment plans as needed to reflect changes in their mental health status or goals.
Respond to behavioral crises, providing immediate support and de-escalation techniques to ensure the safety of residents and staff.
Work closely with therapists, case managers, social workers, and other group home staff to coordinate care and provide comprehensive support for residents.
Advocate for residents' needs within the system and connect them with external resources such as educational, vocational, or additional mental health services.
Collect outcome data and consumer satisfaction data as required by the agency and contractor.
Must implement program goals and objectives to meet and maintain contractual requirements and productivity.
Participate in scheduled Peer Review.
Responsible for preventing and controlling infection.
Responsible for maintaining a culture of quality and safety.
Effectively use de-escalation skills and techniques for crisis prevention.
Properly administer the C-SSRS to help prevent suicide.
Effectively use Therapeutic Crisis Intervention techniques for crisis prevention and/or intervention.
Properly administer medication for the therapeutic treatment of clients.

The Youth Counselor - Youth Transition Focus provides structure, care, and mentorship to male youth ages 16-18 living in a therapeutic ILP-designated STRTP setting. You will work as part of Aspiranet's Therapeutic Community of Care, collaborating with behavioral health and case management professionals to implement individualized treatment and transition plans that balance responsibility, healing, and personal growth.

Qualifications
Candidates must meet at least one of the following criteria:

* Relevant life experience in child welfare, mental health, juvenile justice, or as a mentor/caregiver for youth who have experienced trauma.

Scope of Assignment
Under the supervision of the Dean of Student Services, the Part Time Special Programs Counselor is responsible for providing comprehensive counseling services to diverse student populations. Counseling services include all aspects of academic, career, and personal counseling. Primary assignment will be working with EOPS/CARE, and Foster Youth. Counseling may also include Justice Involved, Veterans, CalWORKs, and other disproportionately impacted student populations. Duties may include individual student appointments, walk-in assignments, group guidance courses, workshops, evening and weekend assignments, and other outreach activities; facilitates the successful completion of their goals and objectives in college through academic and support counseling, financial aid, and other support services. Must be able to use available technologies and apply current career theories and methods. In addition, counselors are to maintain effective relations with all areas of the college community, and promote - wherever appropriate - the counseling services of all Special Programs and Student Services departments.
The successful candidate will join a Student Services Division that continues to create specialized supportive services and curriculum responsive to student needs, increasing student completion through the implementation of strategies geared to increase student equity.

GENERAL PURPOSE

Develop financial management plans for clients who receive Representative Payee Services and authorizes appropriate expenditures.
Interacts productively within the team in order to provide holistic case management services to an identified group of clients.
Provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program.
Drives on program business using personal vehicle for the purposes of medication outreach, home visits, transporting clients to appointments/activities, community/county meetings/trainings, MH court.

Key Responsibilities:
Provides orientation to new patients to the programs' rules, policies, procedures, and patients' rights.
Conduct Intakes, Discharge Planning, Initial Assessments, and Initial Treatment Plans professionally and within time guidelines as required by applicable federal and state regulation, CARF guidelines and Pinnacle Policies.
Involve patients in the assessment, treatment planning, and intervention aspects of treatment, which provides the patient with a sense of entitlement to their own recovery.
Administer the biopsychosocial assessment, which evaluates all areas of patients' lives, including their addictions, medical and mental health history, socioeconomics, legal, etc. at intake and on an ongoing basis.
Recognize and utilize patients' Strengths, Needs, Ability, and Preferences (SNAP), to develop and implement individualized Treatment Plans that effectively help patients to meet their short & long-term goals. • Determine the clinical necessity of counseling services based on the clinical assessment and evaluation of the patients.
Monitor closely, and document any change of circumstances with the patients, especially as it relates to matters that may affect their recovery, identifying and addressing relapse indicators to promote relapse prevention interventions in an early stage.
Comply with and implement the Medical Q.A (Quality Assurance) and Clinical Risk Management Policies, including:
Evaluate the patients for any high-risk conditions (e.g., liver failure, pregnancy, overdose, bipolar, schizophrenia, etc.), which may endanger the patient (suicidal) or other parties (homicidal).
Administer the BAM-R, C-SSRS, GAD-7, and PHQ-9 screening tools to determine the severity of the patient's mental/emotional status.
Evaluate the risk factors concerning each such case, considering the appropriateness of, and professionalism in treating such individuals in an OTP in comparison to treatment in a residential facility or other treatment modalities.
Review and ask about other practitioners' involvement in the treatment of these high-risk conditions. Recommend immediate referral to such a specialist in case of need. Request the patient sign a waiver for release of information which will allow communication with the other practitioner in case of need (e.g., deterioration of the patient's condition).
Flag high-risk patients to the Medical Directors, Physician Extenders, and Regional Clinical Leadership (RCL) and highlight the conditions concerned.
Refer such patients to the Medical Directors, Physician Extenders or RCLs (Regional Clinical Leadership) based on the urgency, utilizing the informal Referral Policy or the scheduled Treatment Team/Case Conference session.
Review physician notes, following the Medical Directors or Physician Extenders' session with the patient, verifying that they noted and addressed the high-risk factor, or otherwise bring it immediately to the attention of the supervising counselor, as well as the clinic manager, until the matter's proper and professional handling by the physician or physician extender.
Advise, in writing, other staff members (including the clinic manager, supervising counselor, dispensing nurse and front office) of high-risk patients and their condition. Maintain and circulate minutes from treatment team/case conference sessions to the other staff members.
Provide professional counseling and referral services by:
Addressing patients' clinical and special needs by maintaining referrals to specialized (“second tier”) services rendered by physicians, physician extenders and counselors within Aegis, as well as referrals to Keys to Recovery (or other twelve-step) support groups, and other community resources (e.g., agencies and practitioners outside of Aegis), acting as a liaison to these agencies, as necessary.
Providing individual and group counseling as clinically necessary, to address the problem issues identified in the Initial Assessment and Treatment Plans. The counseling service will include protocols for the treatment of Relapse Prevention, Peer Pressure, Anger Management, Domestic Violence, Parenting, Family Preservation, Vocational Rehabilitation, etc.
Utilizing advanced counseling techniques such as Cognitive Behavioral Therapy, Motivational Interviewing, etc.
Scheduling regular counseling sessions with patients; issue patient appointment cards and record scheduled or rescheduled sessions in EHR scheduler.
Documenting counseling entries, including signature and date, as required by applicable federal and state regulations, CARF guidelines and Pinnacle policies.

Collecting research data when requested.

Qualifications
Candidates must meet at least one of the following criteria:
* Bachelor's degree in any field (Psychology, Sociology, Social Work, or a related field preferred), OR
* Valid Child Development Teaching Permit, OR
* Completed 12 semester units in early childhood education, psychology, adolescent development, criminal justice, or foster/kinship care education, OR
* Valid certificate as an alcohol and/or drug counselor, OR
* Previous employment as a staff or volunteer at a group home or STRTP for at least 1 year, OR
* Relevant life experience in child welfare, mental health, juvenile justice, or as a mentor/caregiver for youth who have experienced trauma.
Additional Requirements:
* Ability to work evenings, weekends, and holidays.
* Proficiency in Microsoft Word and Outlook.
* Meet all state conditions for employment in a licensed community care facility, including fingerprint and child abuse index clearance, health/TB screening, and a valid CA driver's license with a clean record.
