POSITION SUMMARY:
The Youth Support Counselor is responsible for supervising and providing emotional support to our teen mothers and their babies/toddlers. After completing extensive training, you will be able to utilize a strengths-based, trauma informed approach to working with our teen mothers and help them learn good parenting skills. This position reports to the Program Manager with additional support from the treatment team and Program Director.

Execute good judgement when responding to site challenges and/or crisis that may arise. Be able to stay calm and professional in a demanding, fast-paced, needs-driven environment disbursing attention between daily tasks, conversation/counseling, supervision, documentation, communication, appointments and recreation.
Document all data on the appropriate forms, validating attachments and communication protocols being met. All extensions to deadlines must be requested at least 24 hours in advance to your direct supervisor.
Interpret and execute Behavior Support Plans, as applicable
Safely operate a 13-passenger vehicle to provide transportation to Individuals needing to integrate the community for doctor's appointments, recreation, grocery shopping and/or emergent needs. It is expected that Residential Counselors transport Individuals' who are in psychiatric/medical crisis to supportive facilities such as DASH, CPEP, ELIH, Southampton Hospital and PBMC to name a few.
Adhere to van safety protocols including but not limited to regular gas refills (should always remain above half tank), maintenance/servicing and regular car washes. All concerns must be immediately reported to Residential Manager or Program Coordinator/Operations Director.
Work well as a team member with supervisors, co-workers and other facets of Management. Follow supervisor's instructions, leadership and guidance to support your professional development. Initiate and precisely complete duties in a timely manner being open to positive or constructive feedback.
Maintain the residence in a safe, clean, and stress-free manner. COVID- related orders as outlined by the Executive Order, CDC, DOH, and OMH MUST be followed. Sanitizing/Sterilizing supplies must be used frequently throughout the day especially in high-traffic areas. While the primary function of the overnight staff is to DEEP CLEAN the residence (washing walls/baseboards/organizing pantries/inventorying supplies/washing curtains/scrubbing all common areas/wiping down furniture), it is expected that all team members maintain a clean home.
Maintain compliance with all trainings and certifications throughout employment; Comply with additional trainings as assigned.
Document monthly collateral contacts with service providers included, but not limited to physicians, therapists, ACT team members, etc.
Follow/Document OMH fire safety regulations that include, but are not limited to routine fire drills, weekly checks of smoke/carbon monoxide detectors, regular checks of dryer lint compartments, etc.
Assist Individuals in facilitating weekly community meetings, developing ideas for meal prep, recreational outings, and discussion of weekly Accomplishments.

WHAT WILL I BE DOING?
CHI is looking for a full-time Rental Supplement Program Counselor for our Patchogue location. This position will include travel and field related work. Great opportunity at a growing non-profit agency for the right candidate. Help us grow our Housing Retention Services in Suffolk County. This is accomplished by performing the following functions:
Respond to referrals via HMIS and collaborate with referring agencies to collect and upload client eligibility information.
Prepare initial, annual, and interim income certifications as outlined by Suffolk County's HUD CoC funded programs.
Work with new clients, including initial discussions, completion of all intake forms and review of financial documents. Execute basic math skills.
Contact all participating households at least once every three months regarding moves, changes in household composition or monthly income, budgeting issue and supportive services.
Responsible for all required reporting and compliance - cases must be managed in accordance with New York State requirements; including database and file management.
Experience working with clients experiencing homelessness/residing in shelter.
Knowledge of the local housing market and available resources.
Conduct annual unit inspections to ensure housing quality standards are met for potential participants.
Contact landlords to mediate Housing Quality Standard issue, lease renewal and potential client repayment plans.
Connect clients with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of the referral.
Maintain up-to -date records in AWARDS and HMIS. Document all interactions in a clear and concise manner.
Establish good working relationships with referring agencies.
Represent the company with community outreach and events.
Remain up to date with industry standards and increase professional and technical skills through continuing education annually
WHAT DO I NEED?
Education: Bachelors' Degree in a related field preferred. Extensive experience may be substituted for educational requirement.
Experience: Basic knowledge and understating of federal, state and local homeless and housing programs and rental subsidies.
Communication: Excellent verbal and written communication skills. Proficiency in English required. Bi-lingual (Spanish} is a plus.
Computer Skills: Basic competency in MS Windows, MS Office, and on the Internet. Math Skills: Must be excellent.
Physical Performance: Walking upstairs, driving, and set up conference rooms.
Reasoning Ability: Ability to prioritize, make appropriate decisions and judgment calls.
Other Skills: Ability to work independently or as part of a team, be highly organized, assume responsibility, work well under pressure, meet deadlines, multitask, be flexible, and maintain a high level of attention to details. Must be even tempered, have good negotiation skills, and the ability to meet unexpected deadlines.
Valid driver's license and automobile insurance.

Salary: $24.04 per hour (50,003 per year) Non-Exempt
Shift: Monday - Friday, 9 a.m. - 5 p.m.
