Overview
This classification learns to perform eligibility determination work of average difficulty. An employee in this class is customer facing and learns to interview customers and determine eligibility for SNAP and TANF programs. This classification performs responsibilities at the learning level under direct supervision. This classification differs from the Eligibility Counselor 2 in that an incumbent of the latter functions at the working level.
*An applicant appointed to this classification will be reclassified to an Eligibility Counselor 2 after successful completion of a mandatory one-year training period; 'unacceptable' or 'needs improvement' performance ratings during the training period will result in automatic demotion or termination.
Responsibilities
Learns to manage a workload of initial and on-going eligibility determinations for SNAP and TANF programs based on specific demographic and economic criteria in accordance with Federal and State requirements through independently conducting probing interviews with customers by phone or in person.
Learns to provide case management for initial SNAP and TANF eligibility, ongoing eligibility, and customer changes to assess impact on continued eligibility through direct customer engagement via phone or in person.
Learns to calculate monthly income and expenses to ensure accurate budgeting. Documents information regarding verifications requested/received, benefits and services provided, and other case-related activity using a computerized case record and/or paper case files.

The Greater Nashville Regional Council (GNRC) is seeking to promote an experienced Social Worker to serve as Lead Intake Counselor for the CHOICES program within its Department of Aging and Disability Services. The Department's responsibilities include 1) planning and coordination of state/federal investments into programs and services aimed at improving quality of life for senior adults across the region, 2) coordinating services among a network of senior centers and providers, 3) providing counseling/assessments to help Middle Tennesseans enroll in programs and services, and 4) monitoring the performance of grant-funded programs to ensure compliance with the state and federal requirements.
GNRC's CHOICES Intake team helps to screen and assess older adults who may be eligible for the TennCare sponsored CHOICES program funded through Medicaid. CHOICES provides nursing facility services and home and community-based services for adults 21 years of age and older with a physical disability and seniors (age 65 and older).
Responsibilities of this position include:
Provide intake counseling services to assigned individuals seeking enrollment into the TennCare CHOICES programs and other TennCare programs as may be requested;
Complete the necessary forms, assessments, and reports to document the physical and financial conditions for prospective program participants;
Coordinate with the Program Manager, Enrollment Coordinator, and Financial Coordinator to ensure GNRC has completed and documented all necessary steps to help TennCare determine eligibility of prospective program participants;
Assist the Program Manager with training, coaching, mentoring of Intake counselors;
Assist the Program Manager with tracking the team's performance and preparing recommendations for process improvements and/or program changes for consideration by the Director of Aging and Disability Services and the Executive Director;

Qualified candidates will hold one or more of the following certifications/licenses:
Master's degree in counseling or a related field.
Must be licensed in the state of TN as LMSW Experience treating opioid-addicted patients is not a requirement but is preferred.
No legal impediments to serving in a licensed opiate treatment program.
Must have a valid driver's license.
If recovering, three years of continuous verifiable abstinence is required.
General experience working with underserved populations in a clinical setting.
Lived addiction experience is a plus.
Schedule and Hours
Expected hours for this role are 37-40 hours per week.
Monday: 9:00 am- 4:00 pm
Tuesday: 9:00 am- 3:00 pm
Wednesday: 9:00am- 4:00 pm
Thursday: 9:00 am- 5:00 pm
Friday: 9:00 am - 3:00 pm

Intensive in-home counselor job in Tennessee
The Counseling Internship in Integrated Behavioral Health is a one-year training experience with a specialized focus on substance use and misuse. This internship provides comprehensive clinical training in the delivery of substance use and recovery services within primary care settings. Interns will contribute to and learn from an interprofessional healthcare team-including behavioral health professionals, physicians, nurses, case managers, pharmacists, and trainees across these disciplines-by offering expertise in substance use disorders and integrated care practices. In addition, interns will complete structured didactic and professional development training through the Institute for Integrated Behavioral Health, supporting their growth as competent and collaborative behavioral health providers.
Knowledge, Skills, and Abilities
Knowledge of the principles and practices of behavioral health consultation, including assessment, diagnosis, treatment planning, cognitive-behavioral counseling strategies, and their application to population-based care for preventive and chronic care issues.
Knowledge and understanding of patient-centered medical home principles, motivational interviewing skills, and evidence-based short-term therapy strategies.
Comprehensive understanding of the interrelationship between medical and psychological systems and their impact on health.
Skill in efficiently organizing time so that unscheduled, same-day services are routinely available to care team members.
Skill in developing effective working relationships with peers, supervisors, and professionals in related disciplines.
Strong written and verbal communication skills.
Ability to act with minimal supervision on personal initiative using sound clinical judgment.
Ability to exercise sound judgment in appraising situations and making decisions regarding complex physical and behavioral health treatment issues in consultation with medical teams.
Ability to manage complex personal interactions.
Ability to maintain confidentiality in regard to patient care information and chart contents, as the use of patient records and the privacy of individuals must be protected to the fullest.
Ability to maintain documentation in compliance with State, Federal, Professional, and Ethical guidelines.
