Intensive In-Home Counselor Job In Twin Falls, ID
This position assists patients in our Assertive Community Treatment (ACT) to achieve recovery goals related to education and employment. Utilizes motivational interviewing skills to assess and identify patient's work and education goals, and consistently articulates economic, medical, and social benefits associated with those goals. Provides support to assist patients in training available, and teaching skills needed to achieve employment goals and is working as a liaison between patient and employer and helping to problem solve issues as they arise. Develops relationships with local businesses through systematic job development and education on ACT Team support with employment placements.
Weekly work hours and pay are not dependent on billable hours and are not dependent on grant funding. 40-hour work week with on-call, weekend, and holiday hours that are shared with other team members. Requires working with patients both in the office and in the community. May provide cross-training to ACT team members as appropriate to the position.
$22.00 - $25.00 DOE
Employee's can earn up to $520 on the quarterly bonus.
MINIMUM QUALIFICATIONS:
Minimum bachelor's degree in human services field. 1 year experience as employment or vocational specialist preferred.
KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
Experience working with people who have a Severe Persistent Mental Illness (SPMI).
Able to use motivational interviewing skills with a diverse group of people.
Able to use computers proficiently; electronic charting in FHS' electronic health record will be required.
Competent in establishing supportive and trusting relationships with people diagnosed with SPMI.
Respect for patient rights and preferences in treatment.
Ability to organize work both in the office and community.
Ability to work with clients with limited English proficiency, including interpretation services when needed.
Ability to make independent decisions when circumstances warrant such action.
Ability to take initiative, problem solve, and effectively multi-task.
Knowledge of community and vocational services.
Model and practice interpersonal and communication skills, active listening, recovery-oriented living skills, and self-help strategies with individuals and groups.

DESCRIPTION OF DUTIES:
Assist patients in identifying their own unique strengths, abilities, and assets, and use them to promote recovery and resiliency as it pertains to vocational goals.
Provide engagement, vocational assessment, job development, job placement, job coaching, and follow-along support in the employment setting.
Educate ACT patients on disability benefits programs as it applies to the patient's needs and goals.
Provide vocational related assessments that include areas such as work and education history, strengths, interests, and barriers.
Develop relationships with local businesses through systematic job development and educating businesses about services that the employment specialist provides.
Consult with other ACT team members in helping the patient achieve their vocational goals.
Refer patients to appropriate social service agencies for financial assistance and other required services and assist them in accessing these services.
Assist patients in identifying and engaging with various community resources and supports.
Assist patients in working on overcoming barriers to maintaining independence, and help patients practice skill building.
Establish and maintain case files, referrals, and other related documents.
Establish and maintain contact with the Idaho ACT CoE for ongoing coaching and consultation.
Provide formal teaching, consultation, and in-service training to relevant professionals, while ensuring proper handling of a variety of matters.
Participate in regular supervision meetings with direct supervisor.
Plan and implement outreach and education programs, workshops, and activities.
Supervise activities of support staff members and interns for identified program.
Perform miscellaneous job-related duties as assigned

Responsibilities
**Summary**
This position provides support and structure to participants in developing program plans by managing a caseload of participants and administering the policies and procedures of the program as directed by the contracting agency and the Program Manager. Learns, uses and models the principles of the “What Works” system through evidence-based practices in all interactions with participants and other staff members.

* Facilitates psycho-educational/behavior modification groups using evidence-based curriculum.

* May orient participants upon arrival at facility to provide smooth assimilation into program including having them complete paperwork, take facility tours, or introduce to other participants.

Summary
This position provides support and structure to participants in developing program plans by managing a caseload of participants and administering the policies and procedures of the program as directed by the contracting agency and the Program Manager. 1. Provides counseling, therapy, and/or psychotherapy to clients and families as appropriate to the position; prepares treatment plans, discharge plans, and follow-up care programs; provides therapeutic crisis intervention and emergency services as required. 2. 3. Consults with other therapists and related professional and paraprofessional staff, as appropriate, in the performance of therapeutic and/or casework; refers clients to appropriate service agencies as required.
4. Provides and/or arranges for therapeutic interventions as appropriate for patients or clients in a crisis condition and for those with serious disturbance problems.
5. Refers clients to appropriate social service agencies for financial assistance and other required services.
6. Follows up to determine reliability of treatment used; changes method and degree of therapy when indicated.
7. Establishes and maintains case files, referrals, and other related documents for the treatment of clients; prepares related documentation for civil commitment hearings or other legal proceedings; keeps abreast of patient data to ensure appropriate treatment and care are delivered.
8. Consults with other legal and treatment agencies and individuals in relation to patient/client records, rights, and responsibilities. 9. Plans and facilitates community outreach and education activities, as appropriate to the position; may serve as a liaison and representative to community organizations and schools.
10. As appropriate to the position, performs various administrative functions such as monitoring budgets and preparing reports and correspondence; may participate in a variety of research projects to develop service plans, and may propose changes to program policies and procedures.
11. May provide formal teaching, consultation, and in-service training to relevant professionals in immediate and proper handling of and/or referral of a variety of matters.
12. Performs miscellaneous job-related duties as assigned.

**THE INTERVENTION SPECIALIST ROLE** The Intervention Specialist provides direct intervention for children and adolescents with developmental disabilities in the home, center, or community setting. Intervention Specialists work with children to develop positive behaviors and the skills they need to function in a typical home and community environment.
**DAILY RESPONSIBILITIES**
* Provide 1:1 behavior intervention to clients, between the ages of 3 to 21, with developmental disabilities.
* Record daily data and keep detailed notes during therapy sessions.
* Maintain open communication with clients and families on a set caseload. Provide family training when needed.

**POSITION REQUREMENT**
* Bachelor's Degree in a Human Service Field, or Bachelor's Degree in a non-human service related field with 24 minimum upper division semester credits in human services field.
* 1040 hours of supervised experience working with individual's between birth to 21 years of age who demonstrate functional or behavioral needs.
* Hold a current certificate in Habilitative Intervention. (*Those who hold a certificate of completion in Idaho prior to July 1, 2019, will be allowed to continue providing services as an Intervention Specialist.)* OR be willing to complete a 40-hour RBT training prior to start date.

Summary
This position provides case management support for a Day Reporting Center checking in participants and assisting Case Managers with case coordination, case management, and documentation. Learns, uses and models the Principles of Effective Intervention (What Works) through evidence-based practices in all interactions with participants and staff members.
Primary Duties and Responsibilities
Greets participants entering the facility. May complete drug screens according to assigned random schedule for each participant check-in.
May perform breath analysis on each participant who enters the facility for any purpose and provides a written or computerized result.
Completes initial intake process with participants to completion, including intake paperwork, input into Company database and assignment of groups and other services.
Provides general office administration duties including answering phones.
Provides general customer service to all who enter the facility, including participants, customers and the general public.
Assists Case Managers with case coordination, communication with appropriate agencies and preparation and distribution of reports.
Assists in managing a participant caseload using the Principles of Effective Intervention (What Works). Conducts risk/need assessments and creates individual treatment plans for each assigned participant. Meets regularly with participants and documents participants attendance, participation and progress or regress in Company database.
Facilitates evidence-based CBT programs and other life skills programs to participants in groups each week. Documents participants’ attendance, participation and progress in Company database.
Develops and monitors the participant’s progress relative to their CMP through all phrases of the program.
Files, prepares charts and performs data entry as assigned.
May collect participant payments, post payments on Company database, and make daily cash deposits.
May be responsible for office opening and closing procedures.
May be responsible for installing and removing electronic monitoring equipment on participants and forwarding documentation to the appropriate agencies and the Company monitoring center.
May transport participants to the center, community service events, job fairs, employment centers, clinics, using a Company vehicle.

Qualifications
Minimum Requirements
