PURPOSE:
Provide direct assistance to individuals with a behavioral health disorder, in gaining access to critical mental health, medical, social, educational, and other agencies or resources whose functions are to provide the support, training and assistance required for a stable, safe, and healthy life within their community.
FUNCTIONS:
Provide BCM services to a caseload that will not exceed 30 individuals, meeting minimum performance and productivity standards as identified by BCM Supervisor.
Provide proactive outreach to the individual in their home, school, and community and engage with, empower, and advocate for needs.
Conduct ongoing assessment and assist individual with identifying strengths and needs.
Develop a personal goal plan in collaboration with the individual and any identified supports and routinely monitor progress.
Provide psychoeducation on behavioral health diagnosis and symptoms; respond to questions or concerns about their course of treatment including potential side effects from prescribed psychotropic medication.
Provide linkage to identified resources with individual's consent. This may include facilitating referrals for expanded mental health programming.
Assist in developing Activities of Daily Living (ADL) skills (e.g., developing a system for remembering to take medication as prescribed, navigating public transportation, budgeting, housekeeping, etc.)
Serve as a clinical liaison and coordinate care between all service providers and systems serving individual which may include but is not limited to: behavioral health, psychiatric, medical, dental, educational, and other evaluations and appointments.
As appropriate, assist clients with applying for MA and Social Security applications or troubleshooting other insurance barriers that impact access to care. This may involve accompanying clients to appointments at Public Assistance and Social Security offices.
Provide 24-hour crisis intervention on a rotating basis. Coordinate care with CRC or psychiatric hospital. This may involve accompanying the individual to the CRC or hospital.
Ensure continuity of care when individual is moving from one level of care to the next to prevent gaps in service coverage. This is especially critical for an adult being discharged from an acute setting.
Actively participate in all staff meetings and provide consultation and support to team members as needed.
Coordinate and lead interagency meetings for assigned cases for the purpose of service coordination and treatment and aftercare planning.
Maintain required records and documentation in compliance with Agency, county, state, and Managed Health Care regulations.
Maintains collaborative working relationships with service providers.
Participate in professional development as assigned, including mandatory annual trainings and 10 hours of elective trainings per year as recommended by BCM supervisor.
Function in compliance with agency policies/procedures and federal, state and local laws.
Participate in Continuous Quality Improvement (CQI) activities as requested by BCM Supervisor or Compliance Officer.
Other duties as assigned.
QUALIFICATIONS:
BCMs must meet one of the following criteria:
A bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education.
Be a registered nurse.
A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years' experience in human services with 1 year in direct client contact, preferably with adults.
A high school diploma and 5 years of mental health direct care experience in human services with employment as an ICM staff person prior to 4/1/89.
Must have a valid Childline, PA Criminal History, FBI Clearances prior to seeing clients.
Requires reliable means of transportation and valid driver's license from state of residence with a minimum of one (1) year driving experience and a driving record free of at-fault accidents or moving violations for the past year.
Must be available to work some evening hours.

PURPOSE:
Under the supervision of the Program Manager, the Lead Resident Advisor guides, assists and instructs residents in activities for independent functioning. This role will be cross trained across all homes and provide direct support to Program Managers with administrative tasks, as well as working open shifts where needed. Responsibilities will be performed in accordance with Interact's CHS policies and procedures.
SCHEDULE:
Must demonstrate flexibility with a schedule that will vary from week to week, depending on staffing needs, emergency situations and any other issues that may arise as it pertains to meeting the clients' needs. The schedule will include two 7 a.m. - 3 p.m. shifts, two 3 p.m. - 11 p.m. shifts, and one Saturday or Sunday shift each week.
FUNCTIONS:
The Lead Resident Advisor's responsibility is to prepare residents for independent living under the direction of the Program Manager.
The Lead Resident Advisor will assist the Program Manager with:
Shopping
Monitoring to ensure diets are being followed as prescribed
Calling the pharmacy for medication refills as needed
Other administrative tasks as assigned
The Lead Resident Advisor will assist, instruct, and monitor residents in all activities for independent functioning, including situations relating to their health, welfare, and safety, under the guidance of the Program Manager and in conjunction with the Interdisciplinary Team.
Fill in shifts as needed in any of the homes in the CHS program.
Incorporate being part of the team in the development of Individual Service Plans (ISP), and the implementation of goals
Ensure that team members are informed of issues and concerns, when necessary
Ensure documentation is completed daily in accordance with Agency and government policy and procedure
Complete skill assessments as needed
Assist in the planning, preparation for and implementation of activities for residents
Go on community outings with residents
Accompany and advocate for residents at medical, BSU, social service appointments, etc.
Assist in crisis intervention and follow established proce dures
Assist residents with maintaining appropriate family contacts
Interacts with clients and other staff in a positive, con structive and professional manner.
Participates in training as assigned.
Must function in full compliance with all applicable policies and procedures, federal state and local laws and Interact's Code of Conduct.
Must participate in Interact's Compliance Program including reporting suspected violations of the compliance program or applicable laws or regulations to supervisor/manager/director or the Compliance Officer.

QUALIFICATIONS:

COMHAR's Long Term Structured Residences (LTSR) are located in Northampton County. This position is located in Walnutport, PA. These therapeutic programs are designed to provide 24/7 residential and intensive treatment supports for adults with a variety of psychiatric and health related needs. Individuals requiring this level of care have access to a variety of services including individual and group therapies, psychiatric evaluation, crisis intervention, peer support, psychiatric rehabilitation and medication management. The multidisciplinary treatment team is comprised of staff that includes a psychiatrist, mental health professionals, licensed nurses, peer specialists, and direct care staff. Services are delivered both on-site and in natural community settings to maximize learning opportunities. Our focus is to assist individuals in developing skills, increasing their independence and stabilizing their psychiatric and physical health symptoms with the goal of moving into lesser structured settings.
The Mental Health Technician works from a recovery framework within the team providing individual supportive counseling and psychoeducational groups using therapeutic interventions (evidenced based practices preferred), cultural competence and trauma informed care to help individuals obtain their recovery goals.
The Mental Health Technician will provide direct services to residents, including assistance with personal hygiene and ADL's, as well as assistance with all tasks necessary for maintaining a therapeutic community and residence.
Conducting activities with residents, including educational, social and leisure programs. Inclusion of resident's input and suggestions in the planning process involved for all activities.
The Mental Health Technician is responsible for documentation in clinical charts and residential logs as required, including progress notes on assigned residents. Immediate reporting of all resident crisis and/or unusual incidents to Shift Supervisor.
Participation in meetings and training sessions as assigned, including but not limited to Quality Improvement Program, and multi-disciplinary treatment team meetings.

Services are provide to children, youth, and young adults across all settings (at the home, school, and community settings) in order to both help them reduce and/or replace problem behaviors with more positive, socially appropriate behaviors, with the highest priority placed on family engagement throughout the process.
Psychologist Position Summary:
The psychologist is responsible for providing evaluation and therapy services to clients, monitoring treatment planning and goals, and providing supervision and direction to identified staff.
Psychologist Responsibilities:
Evaluation services to clients in accordance with facility, licensing and third party payer standards and procedures. The ORLTSR is a therapeutic residential program that works with justice involved individuals, ages 18 and over. The individuals in the program have a diagnosis of serious mental illness and have been prosecuted on criminal charges. The program will provide mental health treatment to those who will be admitted on Conditional Release or those in need of competency restoration.
The Certified Peer Specialist works from a recovery framework within the team providing individual supportive counseling and psychoeducational groups using therapeutic interventions (evidenced based practices preferred), cultural competence and trauma informed care to help individuals obtain their recovery goals. The Certified Peer Specialist provides a wide range of tasks to assist consumers in regaining control over their own recovery process. This includes but is not limited to the development of natural supports, development of social interactions in the community and management of symptoms that challenge wellness in an individual.

Family Peer Specialist Position Summary
The Family Peer Specialist will provide support to children and their families by engaging with families, incorporating their voice into treatment, and by providing resources as needed. Creative Arts Therapist Position Summary
The Creative Arts Therapist, under the supervision of the Long Term Structured Residences (LTSR) Program Director, is responsible for designing and implementing therapeutic programming and related activities which reflect individual client's needs, interests, and the individualized recovery planning processes. The Creative Arts Therapist will provide education and training to both individuals in recovery and staff regarding the benefits and intricacies of art therapy. Facilitate activities with individuals and groups, including educational, clinical, social and leisure programs, ensuring inclusion of the individuals in the activities.
Creative Arts Therapist Duties and Responsibilities
Completes detailed documentation is completed for each individual in their clinical record and in other areas as directed. Such documentation should be timely, legible, and provide an accurate depiction of individuals' progress, challenges, etc. Satisfies compliance with regulations regarding record keeping and reporting.
Participates in the multi-disciplinary treatment team meetings, assists with recovery plan development, and identifies appropriate internal/external supports and services as directed.
Partners with program leadership to design and implement therapeutic programming and related activities which reflect individual needs, interests, and the individualized recovery planning processes.
Completes identified assessments for each individual upon admission and annually thereafter or more frequently if clinically indicated. Discipline-specific tools are utilized to ensure that the assessment drives the recovery planning process and goal development.
Facilitates activities with individuals and groups, including educational, clinical, social, and leisure programs for which individuals' input is elicited. Most of the services delivered are in a direct service/clinical capacity.
Provides consultation to staff regarding program and activity goals, parameters, and content, as they pertain to individual recovery plans. Provides education and training to individuals in recovery, as well as staff regarding the benefits and intricacies of art therapy.
Participates in Quality Improvement component in conjunction with COMHAR's Quality Improvement Program. Ensures that both internal and external incident reporting requirements are met for all crises and unusual incidents. Accurate and timely reporting of all non-emergent programs and/or staff problems to Supervisor.
Assists with connecting individuals to community resources as identified in their treatment/recovery plan.
Assists with the coordination of transportation needs entailed by activity schedule. Timely submission of all vehicle and driver requests in cost-effective manner.
Establishes partnerships with community-based support to promote community integration/inclusion and identifies methods for individuals in recovery to build social capital and develop networks of support.
Maintains compliance with all COMHAR policies and procedures with no unauthorized exception.
Establishes working relationships with Northampton County, OMHSAS, referring agencies, and related external service providers, either directly, or through delegation.
Develops working relationships within the various COMHAR Departments to ensure that all program needs are satisfied in a timely fashion.
Timely and accurate submission of activity budget projections and actual cost receipts.
Supervision of student interns and activity specialists as assigned.

Mental Health Technician Pay Rate:

Deliver quality and consistent individual and group activities, i.e., counseling sessions, skill teaching, house meetings, etc.
Provide assistance and support as needed with activities of daily living, e.g., medication management, social/leisure skills, budgeting, benefits/entitlements, health care, community participation, etc.

This position will manage approximately 4 - 5 Community Homes which provide direct care services to adults with intellectual and developmental disabilities.
As a primary, leadership role within the organization, the Program Director ensures that participants receive a high level of quality care and are treated with respect and dignity. The role advocates for the IDD individuals to support their person-centered care and healthy outcomes. The Director will facilitate the development and implementation of Individual Support Plans (ISPs), Individual Social/Emotional/ Environmental Plans, Behavior Plans, and Restrictive Plans.
Program Directors are responsible for oversight of frontline Supervisors and some direct care staff. They administer performance reviews, disciplinary actions, create work schedules, assist with training new staff, report on monthly metrics, and are a member of the Agency Communication Team.
This full-time position works Monday through Friday, daylight hours, with on-call duties.
Qualifications:

PURPOSE:
Provide direct assistance to individuals with a behavioral health disorder, in gaining access to critical mental health, medical, social, educational, and other agencies or resources whose functions are to provide the support, training and assistance required for a stable, safe, and healthy life within their community.
FUNCTIONS:
Provide BCM services to a caseload that will not exceed 30 individuals, meeting minimum performance and productivity standards as identified by BCM Supervisor.
Provide proactive outreach to the individual in their home, school, and community and engage with, empower, and advocate for needs.
Conduct ongoing assessment and assist individual with identifying strengths and needs.
Develop a personal goal plan in collaboration with the individual and any identified supports and routinely monitor progress.
Provide psychoeducation on behavioral health diagnosis and symptoms; respond to questions or concerns about their course of treatment including potential side effects from prescribed psychotropic medication.
Provide linkage to identified resources with individual's consent. This may include facilitating referrals for expanded mental health programming.
Assist in developing Activities of Daily Living (ADL) skills (e.g., developing a system for remembering to take medication as prescribed, navigating public transportation, budgeting, housekeeping, etc.)
Serve as a clinical liaison and coordinate care between all service providers and systems serving individual which may include but is not limited to: behavioral health, psychiatric, medical, dental, educational, and other evaluations and appointments.
As appropriate, assist clients with applying for MA and Social Security applications or troubleshooting other insurance barriers that impact access to care. This may involve accompanying clients to appointments at Public Assistance and Social Security offices.
Provide 24-hour crisis intervention on a rotating basis. Coordinate care with CRC or psychiatric hospital. This may involve accompanying the individual to the CRC or hospital.
Ensure continuity of care when individual is moving from one level of care to the next to prevent gaps in service coverage. This is especially critical for an adult being discharged from an acute setting.
Actively participate in all staff meetings and provide consultation and support to team members as needed.
Coordinate and lead interagency meetings for assigned cases for the purpose of service coordination and treatment and aftercare planning.
Maintain required records and documentation in compliance with Agency, county, state, and Managed Health Care regulations.
Maintains collaborative working relationships with service providers.
Participate in professional development as assigned, including mandatory annual trainings and 10 hours of elective trainings per year as recommended by BCM supervisor.
Function in compliance with agency policies/procedures and federal, state and local laws.
Participate in Continuous Quality Improvement (CQI) activities as requested by BCM Supervisor or Compliance Officer.
Other duties as assigned.
QUALIFICATIONS:
BCMs must meet one of the following criteria:
A bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling, or education.
Be a registered nurse.
A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years' experience in human services with 1 year in direct client contact, preferably with adults.
A high school diploma and 5 years of mental health direct care experience in human services with employment as an ICM staff person prior to 4/1/89.
Must have a valid Childline, PA Criminal History, FBI Clearances prior to seeing clients.
Requires reliable means of transportation and valid driver's license from state of residence with a minimum of one (1) year driving experience and a driving record free of at-fault accidents or moving violations for the past year.
Must be available to work some evening hours.
