As a Registered Nurse Case Manager (RNCM), you will:
•Conduct In Person patient interviews and comprehensive physical assessments.
•Oversee the implementation and ongoing assessment of the patient's plan of care through the management of home health aides, LPNs, RNs, and other caregivers.
•Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
•Provide education to patients and families on proper home health care procedures. Ie. Wound care, IV administration, medication management.
•Work to decrease readmissions by promoting preventative care and ensuring continuity of care.
To qualify as a Registered Nurse Case Manager (RNCM) with us, you will need:
•Licensure: Current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position
•Current CPR/AED/BLS/First Aid certification
•Reliable transportation to/from care sites and/or work locations.
•One (1) year of professional experience practicing as a Registered Nurse (RN) in home health or similar setting; previous case management/utilization review experience preferred.
•OASIS experience preferred.
•Practical trach and/or ventilator experience preferred, not required.

Excellent customer service skills.
Ability to remain calm in stressful situations.

Job Overview
The Home Coordinator is responsible for providing direction and guidance to direct care staff caring for clients with intellectual and/or developmental disabilities in a supported living or community-based environment. Incumbents are on call twenty-four hours a day and must respond in a timely manner to unexpected events and/or emergencies with clients under their care. The Home Coordinator is also expected to provide direct client care.
Essential Duties and Responsibilities
Plans, organizes, and directs the care of clients.
Support related to outcomes and the support and supervision of the individual will be monitored to assure that the overall supports provided are in adherence with the individuals person centered plan.
Monitors the client care activities of direct support staff.
Monitors Individual Support Plans and delivery of client care plans as appropriate; takes actions as appropriate to ensure Standards of Care are met; participates in quality improvement process
May assist with determining staffing schedules, staffing levels, and staff schedules in coordination with the Program Manager.
Assures that staffing patterns developed are in adherence with the team recommendation and PAWS Allocation.
Responsible for preventing client abuse by adhering to all policies and procedures, attending periodic training, reporting suspicious and inappropriate behavior, and following mandatory abuse reporting requirements.
Complete administrative and clerical documentation including but not limited to reviewing daily logs and notes detailing client activities supporting the approved Individual Support Plan (ISP) that support services billed through Medicaid HCB Waiver Program.
Assures safe operation of all equipment in the house by completing routine site monitoring reports.
Plan and organize daily activities in accordance with Individual Support Plan.
Provides direct client care, including transportation, maintenance of household inventory, menu planning and preparation, etc.
Provides weekly oversight of individuals' finances by providing direct assistance with managing personal funds (personal ledger, petty cash in home and receipts). Reports any signs of misappropriation, exploitation, or financial impropriety regarding individual funds to the County Board through Unusual Incident (UI) reports and to his or her direct supervisor (PM)
Maintain and schedule medical appointments. Complete all follow-ups including maintaining the Appointments in Advisor and attaching related relevant documents.
Other duties as assigned.
Qualifications
One-year supervisory experience working with developmentally, intellectually and/or physically disabled individuals preferred.

As a Speech Therapist, you will evaluate and provide a program of treatment for clients to the home health agency with speech, language and/or hearing disorders. Reports to the Clinical Director.
ESSENTIAL FUNCTIONS:
1. Assess present and potential problems, needs and resources of the client utilizing standardized testing, observation, and review of the client's history. 2. Sets goals and plans clients' treatment in consultation with the client, the physician, and other disciplines. 3. Administers therapy sessions according to the plan of treatment and client tolerance level. 4. Responsible for the maintenance of necessary records and reports. 5. Communicates and cooperates with agency personnel, supervisors, physicians, and other disciplines to promote effective working conditions and relationships. 6. Attends and participates in educational programs, case conferences, and committees. 7. Adheres to Agency policies and procedures and assists in fulfilling the philosophy and objectives of the agency and maintains confidentially of client information. 8. Maintains a competent level of professional expertise through a personal program of continuing education.
MINIMUM QUALIFICATIONS:
Meets the education and experience requirements for a certification of clinical competence in speech pathology granted by the American Speech and Hearing Association.
SPECIAL REQUIREMENT:
-Car with required insurance coverage, Indiana Drives license.
-Driving vehicle in designated counties services by CAREGIVERS, Inc. Health Services with exposure to traffic hazards and weather.

In this pivotal role, you will be the primary link between our services and the seniors we serve. You will use a consultative sales approach to assess needs, develop tailored service plans, and ensure exceptional client satisfaction and retention across the Indianapolis, Indiana area. Core Responsibilities: Quality Care & Business Growth
Your primary mission is to ensure every client receives the perfect level of care while effectively communicating the value of our services to potential clients.
Service Acquisition & Sales: Prioritize and immediately address all service inquiries. Conduct thorough, empathetic care consultations to understand client needs, effectively communicate our value, and convert inquiries into new clients.
Client Care Coordination: Develop and continuously evaluate customized service plans. Conduct client/Care Professional introductions and manage ongoing communication and follow-up.
Quality Assurance (QA): Maintain high standards of quality by planning and executing a schedule that ensures each client receives at least one quality assurance visit per month/quarter. Use these visits to recognize and pursue opportunities to modify or increase service hours.
Relationship Management: Work with clients, families, and team members to resolve issues and ensure all clients are receiving the services they need.

What We're Looking For (Requirements)
We seek a detail-oriented, persuasive, and service-focused individual ready to make a significant impact.
Experience: High school graduation or equivalent is required. One year of related business experience, or a combination of education and work experience, is preferred.
Sales Mindset: Demonstrated sales-driven mindset with a focus on achieving targets and contributing to business growth.
Communication & Interpersonal Skills: Excellent oral and written communication skills. Must demonstrate effective interpersonal skills, sound judgment, and good decision-making.
Organizational Skills: Ability to organize and prioritize daily, monthly, and yearly work to maintain regular attendance at the office.
Travel & Availability: Must possess a valid driver's license and be able to work evenings or weekends as required (including potential after-hours phone coverage or assisting clients in their homes).
Technical Proficiency: Must have computer skills and be proficient in client care systems, Word, and Excel.
