    Nationally certified Advanced Practice Provider who provides primary and/or specialty care in a variety of healthcare settings. Functions in collaboration with the health care team in accordance with certification and licensure. Practice emphasizes health promotion, disease prevention and the diagnosis and treatment of acute and chronic health problems. Responsible for dissemination of knowledge through instructing, leading and developing educational programs. Specifically, our Express Care department is seeking to add a qualified Advanced Practice Provider to its team. The provider in this role will work to the top of competency providing comprehensive care for patient age six months and up. All candidates must be graduates of an accredited program to be considered. The ideal candidate will have ambulatory experience, preferably in primary care and would be comfortable handling a wide range of responsibilities throughout the day including providing patient care, performing procedures and working with a team to meet the patients immediate care needs as we as facilitate follow up care. Preferred qualifications: 1 year advanced practice experience. Nurse Practitioners should have a family certification. Responsibilities: * Conducts thorough medical histories, performs complete physical examinations (where indicated), initiates appropriate lab, radiology tests or other special tests required for evaluation of illness, and scrutinizes lab data to establish a record of the patient's current health status and to develop a working diagnosis and treatment plan. Works in collaboration with the health care team. * Performs and interprets common laboratory, radiologic, cardiographic and other routine diagnostic procedures used to identify pathophysiologic processes as credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals. * Performs routine and specialized procedures as credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals. * May prescribe and monitor medications as allowed by license through applicable state board and Cleveland Clinic policies. * Educates patients and answers questions regarding their disease, treatments, related drug and treatment side effects and hazards. * Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members regularly to ensure quality patient care. * Refers patients to specialists as appropriate for consultation or for specialized health resources and treatment. * Ensures continuity of care by serving as a liaison between patient and other members of the multi-disciplinary care team or with other specialty areas as necessary. * Participates in quality monitoring thru the review of records and treatment plans for patient outcomes on a periodic basis to assure quality care. * Facilitates appropriate length of stay, discharge planning and compliance with regulatory standards for inpatient management. * Supports professional development of the health care team. * Promotes translation of evidence-based practices, guidelines, and research. * May conduct research studies.     Cleveland Clinic's Integrative Hospital Care Institute is seeking a Physician Assistant or Acute Care certified Nurse Practitioner for a full-time Intensivist position in the Medical and Cardiac Intensive Care Units. In addition to core MICU and CICU responsibilities, the provider will also support the Main Campus Sepsis Early Response Team. Akron General's Intensive Care Unit (ICU) encompasses both the 12-bed Surgical Intensive Care Unit (SICU) and the 12-bed Medical Intensive Care Unit (MICU). These units provide comprehensive, patient-centered critical care to diverse populations of patients ranging from young adults to geriatric patients. Patients in the ICU have a wide range of needs as these are Level I trauma, critical care and CRRT dialysis patients requiring one-on-one care. As a Patient Care Nursing Assistant (PCNA) on the team, you will perform various tasks, ensuring patients receive frequent attention and observation as needed. This is a hands-on role that requires a present, attentive and patient caregiver who is willing to learn new medical technologies and thrive in a fast-paced environment. On this team, you will gain a wide variety of skills you can use in advanced positions or other departments. A caregiver in this position works 7:00 a.m. - 3:30 p.m. with every other weekend and 3 holidays per year requirements. A caregiver who excels in this role will: * Prepare units, patient rooms and patient beds for receiving patient admission, returning surgical patients, transfers or emergencies. * Provide and document assigned care, such as bathing, changing/refreshing bed linens, dressing/undressing, feeding, etc. * Take and document the patient's temperature, pulse, respirations, blood pressure, pulse oximetry, weight and height. * Perform treatment procedures, such as venous blood draws, enemas, application of ice and heat packs, non-sterile compresses, skin preps, setting up oxygen equipment, bed scale weights, setting up overhead trapeze and Buck's traction and removing urinary Foley Catheters. * Assist with pre-operative and post-operative care, such as turning and positioning patients with proper body mechanics, applying/utilizing special equipment and ambulating the patient as directed in a safe, prescribed manner. * Assist and accompany patients during admission, transfer and discharge. * Perform hourly safety rounds, anticipate patient needs and respond to patient call lights promptly. * Collect, properly label and transport specimens to proper area. * Document patient care in the electronic medical record. * Assist in keeping patient rooms in order, report equipment that is out of order, clean equipment and inventory and distribute unit linen and supplies. * Complete ongoing competency requirements, education programs and mandatory training to maintain certification and/or licensure. Minimum qualifications for the ideal future caregiver include: * High School Diploma or GED * Basic Life Support (BLS) certification through American Heart Association (AHA) or American Red Cross upon hire or within new hire period * Prior job or educational experience providing basic computer knowledge and skills Preferred qualifications for the ideal future caregiver include: * State Tested Nursing Assistant (STNA) * Acute care experience * One year of recent acute care experience OR military experience as a Hospital Corpsman (HM), Combat Medic (68W) or Medical Service Technician (4N0X1) Physical Requirements: * Requires full body motion to move and lift patients, manual finger dexterity with good eye-hand coordination; involves extensive standing and walking. * Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. * Physical Demand requirements are in excess of those for Light Work.
