Most patients treated in this area need orthopedic care, with the remainder being various chronic disease patients. **A caregiver in this position works variable shifts between 8:00AM - 4:00PM, depending on patients' needs. Schedules will be received the night before each workday.** A caregiver who excels in this role will: + Perform initial and ongoing assessments of patient's condition. + Establish, revise, and evaluate a plan of care which is appropriate to problems identified and involves the patient/family according to their capabilities and desires. + Perform therapy interventions using standard physical therapy techniques and skills appropriate for the patient's age and condition. + Supervise activities of physical therapy assistants, students, and other support personnel. + Maintain complete and up-to-date documentation and files regarding patient condition and effectiveness and timeliness of care and interventions. + Participate in ongoing interdisciplinary assessment of assigned patients. + Participate as a member of the hospice interdisciplinary team when necessary.
