    The Social Work Care Manager I utilizes clinical expertise to perform psychosocial assessments, develop and implement care plans in collaboration with the appropriate care team, and assess crisis situations to provide clinical counseling, diagnosis, brief therapeutic interventions, and necessary resources or referrals. This role also includes providing individual and family treatment as indicated. The position works collaboratively with patients, their support persons, healthcare providers, insurers, community resources, and all other involved parties. Minimum Qualifications * Master of Social Work (MSW) from an accredited institution (degree verification required). * Current state licensure, as applicable, is obtained prior to or upon completion of required supervision hours. (CSW/LCSW) * Basic computer proficiency, including familiarity with word processing and spreadsheet software. * Strong written and verbal communication skills. * Demonstrated ability to apply critical thinking skills. Preferred Qualifications * Case Management Certification. * Experience in clinical care management, social work, or working with third-party payers. * Demonstrated understanding of care management principles and practices. * Demonstrated understanding of health insurance products and related processes. * Demonstrated understanding of coding, episode of care, and length of stay guidelines. * Ability to work independently, demonstrate self-motivation, maintain a positive attitude, and adapt to a rapidly changing environment. Does this position require Patient Care? Yes Essential Functions * Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues. As a Case Management Social Worker, you will be responsible for coordinating the care and services of patients through comprehensive psychosocial assessments, supportive counseling and linkage to community resources. This role involves intervention with individuals experiencing complex psychosocial or discharge planning challenges, provision of crisis intervention for patients and families, and facilitation of safe, effective transitions of care for high-risk patients. When you join our team, you will be part of a supportive caregiver family that will be united in shared values and a purpose to fulfill our promise of being the best place to receive care and the best place to work in healthcare. **A caregiver in this role works days from 8:00 a.m. - 4:30 p.m. with every 6th weekend rotation and 1 holiday per year requirements.** A caregiver who excels in this role will: + Coordinate the care and services of select patients. + Intervene with patients who have complex psychosocial and discharge planning needs. + Conduct detailed psychosocial assessments and coordinate with community resources and supportive counseling. + Offer crisis interventions to patients and families with psychosocial needs and facilitate the transition of care for high-risk patients facing barriers to care. + Manage all aspects of discharge planning for assigned patients in order to ensure a timely discharge and provide appropriate connection with post-discharge care providers. + Work with families exhibiting complex family dynamics. + Communicate with interdisciplinary team regarding the discharge planning status of all patients that are referred. + Educate patients, families and physicians regarding post-acute options and address freedom of choice and financial concerns. + Conduct initial and ongoing psychosocial assessment and interventions and make treatment recommendations to engage patients and families in adherence to the treatment plan by reducing or eliminating the social, psychological, financial, behavioral and regulatory barriers to successful medical outcomes. + Serve as a resource person to provide counseling and intervention related to treatment decisions and end-of-life issues. + Advocate for patients and families by providing intervention in cases involving child abuse and neglect, domestic violence, elderly abuse, institutional abuse and sexual assault. + Advocate for patient and family empowerment and independence to make autonomous health care decisions and access needed services within the health care system and community. + Meet core social work competencies and adhere to regulations. + Remain current with regulations which impact the care of patients and regulatory compliance. + Promote individual, professional growth and development and meet mandatory and continuing education requirements. + Serve as a preceptor, mentor, and resource less experienced caregivers and represent the department on committees. Minimum qualifications for the ideal future caregiver include: + Master's degree in social work (MSW) or Master of Science in Social Administration (MSSA) + Current state licensure as a Social Worker (LSW, LISW or LISW-S) + Basic Life Support (BLS) certification through the American Heart Association (AHA) OR American Red Cross Preferred qualifications for the ideal future caregiver include: + Certification as a Case Manager within one year of eligibility from an approved professional organization + Three years of experience in a healthcare system that includes exposure to and experience in pre- and post-acute care + Care Management/ Hospital Social Work experience + Knowledge of community resources + Critical thinking and problem-solving skills + Experience working with nurses + Insurance or field social work + Teamwork and group collaboration skills **Physical Requirements:** + Requires full range of motion, manual and finger dexterity and eye-hand coordination. + Requires standing and walking for extensive periods of time. + Requires normal or corrected hearing and vision to normal range. + Sedentary work exerting up to 10 pounds of force occasionally (Occasionally: active or condition exists up to 1/3 of the time) and /or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body.
