Job Overview: The Intensive Case Management (ICM) Caseworker will address barriers to safety and wellness for especially vulnerable refugees, asylees, and other ORR-eligible populations. The ICM caseworker will be expected to provide strengths-based, client-centered case management to single-headed refugee households; survivors of domestic violence and gender-based violence; older refugees; youth and young adults without parents or permanent guardians; refugees experiencing mental health or psychological difficulties; refugees with physical disabilities or medical conditions; unanticipated refugee arrivals and secondary migrants; and refugees who identify as lesbian, gay, bisexual, transgender or queer (LGBTQ). They work closely with clients, colleagues, and external parties to develop service plans to meet individual client needs and preferences utilizing a broad range of financial, medical, social, vocational, and other services and resources in accordance with client eligibility and program requirements. Major Responsibilities: Responsibilities include, but are not limited to: Determine client eligibility for programming and conduct intake assessments, review program expectations, outcomes, and confidentiality with each potential client. Develop individualized, SMART Self-sufficiency plans for each client enrolled in the program. Evaluate client progress regularly during active program enrollment to determine the client's progress towards goals and work with the client to identify new needs and actions needed. Assist clients in navigating healthcare and social service systems, which may include helping clients apply for benefits and attend and schedule appointments. Conduct community outreach and maintain a strong network of service providers that meet client needs. The ICM caseworker may also be expected to maintain a strong relationship with local and state level refugee coordinators and health coordinators to ensure client needs are met. Provide clients with extended orientation on a variety of topics such as women's health, navigating transportation systems, and navigating physical healthcare systems at the local level. Respond to client crises, with guidance from supervisor as needed. Assist clients with requesting financial assistance when applicable and track the received financial assistance within case files. Maintain up-to-date documentation in client's case file to ensure services are properly documented throughout the client's service period; document all services provided in IRC's database, Efforts to Outcomes (ETO). Attend relevant trainings and meetings at the local and national level. May train and lead the activities of volunteers and interns. Other duties as assigned. Job Requirements: Education: Bachelor's degree in social work, public health, or related field of study preferred. Work Experience: Minimum of 1-2 years of case management experience with similar populations and/or client needs required. Demonstrated Skills & Competencies: Strong relationship building, diplomacy, and networking skills; ability to effectively build internal and external relationships. Demonstrated success working and communicating effectively in a multi-cultural environment. SCOPE OF WORK: The IRC's Home Study and Post-Release Services (HSPRS) for unaccompanied children in the U.S. is operating in more than 17 offices throughout the US. This is a national, multi-million-dollar project with ambitious delivery goals in a short time frame. This position is open to candidates residing in the following states: NY, NJ, PA, MD, VA, CT. The goals of the Home-Study Post Release Services Program (HS/PRS) are to: Assess the safety and suitability of a sponsor before an unaccompanied child is released from custody into a sponsor's care (Home Study) Provide case management support via remote and / or conducting in-home visits to ensure the continued safety and stability of children in sponsor's homes and linking children to resources in the community (Post Release Services). The Crisis Case Manager (Level 3) conducts Home Studies to determine a potential sponsor's ability to meet an unaccompanied child's needs upon release from ORR custody, and Post Release Services which help unaccompanied children and sponsors meet their presenting needs. The Crisis Case Manager (Level 3) focuses on children with specific challenges or circumstances and provides intensive services to ensure the safety and stability of the child and family. This includes developing service plans that address needs utilizing a range of financial, medical, social, vocational, and other services and resources based on client eligibility, and utilizing interventions tailored to the needs of the family and child. Major Responsibilities include but are not limited to: Initial in-home assessment with child and sponsors followed by weekly in-person contacts for the first 45-60 calendar days, depending on the presenting need(s) of the released child and sponsor family. As needs stabilize, services can be conducted remotely with monthly in-person visits throughout the remainder of the case. Develop individualized service plan that guides the provision of high quality, trauma-informed case management services. Conduct a comprehensive assessment to provide a recommendation for the purpose of reunifying children with their potential sponsors, including conducting interviews with shelter case managers, clinicians, the minor, sponsor, and all household members. This includes completing an in-person home visit and documenting findings in a comprehensive report within 10 days of referral. Continually assess ongoing changes in behavior and general conditions that could impact the safety, wellbeing, and success of children Provide crisis intervention support when necessary, including involving Child Protective Services, law enforcement and other local stakeholders. Serve in the lead role on a case if there are current suicidal attempts, safety concerns, or current trafficking concerns or play a supportive role to the Level 2 Case Manager when the minor is not in a crisis. Provide referral and connection to: Legal services; Education and English language classes; Medical care and behavioral healthcare; Placement stability and safety services; Assistance with immigration proceedings; Assistance with guardianship proceedings; Family stabilization/counseling; Substance abuse treatment; Gang prevention services; Youth programming (such as youth mentoring, tutoring, afterschool and vocation programs, sports clubs, or other extracurricular clubs, etc.); Services related to various global environments and contexts; and Integration and independence support as appropriate. Provide timely documentation of all check-ins, in-home visits, referrals and referral outcomes, and progress and outcomes in case file. Build and maintain relationships with community services providers in relevant areas. Maintain accurate records in electronic case files and required databases. Submit required reports and documentation in a timely manner. Use a child centered, developmentally appropriate and trauma-informed approach to address the needs and concerns of children and sponsors. Maintain a strong understanding of trauma-informed principles and approaches, protection and safeguarding, and mandatory reporting. Ensure the confidentiality of children and their cases. Job Requirements include but are not limited to: Master's degree in social work, psychology, or other relevant behavioral science in which direct clinical experience is a program requirement. The Licensing Case Manager will execute agent contracts, appointments, terminations, renewals and all other licensing related tasks to meet or exceed pre-defined productivity and quality standards. Essential to the success of Wraparound programming is ensuring staff are adequately trained, the program is sustainable, and there are adequate resources to support the needs of youth and families enrolled in the Wraparound program. This worker will assess client needs and strengths, create service plans and implement through referral and linkage to Agency and Community resources for entitlement programs, employment and vocational rehabilitation services, mental health services as well as to numerous other essential support services for homeless clients. Monitoring and ongoing assessment are essential components of this worker's duties to assist clients to become physically and mentally healthier, housed and in safe stable environments and as independent as possible. At all times, the Case Manager will provide -customer service to both internal and external customers that meets or exceeds the standards set by the Agency. Key Responsibilities: Data Collection and Reporting - All pertinent data will be entered into the HMIS database and will be updated in a timely manner. Client contact sheets will be maintained listing clients served, services rendered and the nature of direct and collateral contacts. Monthly, Quarterly and Annual reports will be generated which will include but not be limited to the number of unduplicated clients, client encounters, collateral contacts, access to case management, and a breakdown of the services rendered. Case load numbers, demographic information and other data is to be tracked, analyzed, and reported. Attends staff conferences and meetings related to the progress of clients. Contributes to program evaluation and research. Prepares case histories and other reports, maintains records on social services rendered, and monitors activities of patients/clients. Maintains contact with other health, education, and welfare agencies and professionals to assure that the needs of clients are met. Interprets specific program policies and procedures to clients and other interested parties. All other duties as assigned. Training and Experience Possession of a master's degree in social work from a college or university approved by the Council of Social Work Education, or possession of a license, issued by the Maryland Board of Social Work Examiners, as a Graduate Social Worker preferred. Knowledge, Skills, and Abilities (Entry Level) Knowledge of the principles, practices, and theories of social work and human behavior. Knowledge of the diagnosis, treatment, and management of social, mental, and emotional dysfunctions. Knowledge of current social, economic, and health problems. Knowledge of individual, family and group therapy dynamics and techniques. Skill in applying social work theories and principles to a variety of case settings. Writing skills are necessary to complete forms, case histories, and reports. Good interpersonal skills. Ability to gather and assess information for the purpose of evaluation or assessment. Ability to assess the needs of clients. Ability to assist in developing effective treatment or service programs for individuals and to provide crisis intervention. Ability to interpret and apply Federal, State, and local guidelines, regulations, and procedures for practicing social work services. Ability to prepare accurate and comprehensive reports. Ability to establish effective working relationships with all contacts. Knowledge, Skills, and Abilities (Full Performance) Knowledge of applicable Federal, State, and local regulations, procedures, and guidelines. Knowledge of alternate and additional resources for clients. SUPPLEMENTAL INFORMATION Physical and Environmental Conditions: The work of this classification is essentially sedentary but may include occasional walking, standing, and/or other limited physical activities. Some positions within this classification require traveling between work sites. Position Type/Expected Hours of Work This is a Full-Time position. Schedule to be discussed with Manager.
