* Responsible for working to prevent, assess, diagnose and treat speech, language, social communication, cognitive communication and swallowing disorders in adults.
* Provides aural rehabilitation for individuals who are deaf or hard of hearing and provides augmentative and alternative communication (AAC) systems for individuals with severe expressive and/or language comprehension disorders.
* Other duties as assigned.
Qualifications
* Current, unencumbered license to practice speech pathology specific to that state you are assigned to work.
* Current Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)* with completed graduate course work and a clinical practicum at a college or university whose program is accredited by the Council on Academic Accreditation in Audiology and Speech-Language Pathology (CAA).
* Current CPR certification.
Preferred
* One (1+) year of experience in speech pathology.
* Note: If the SLP - CCC certification has not been obtained the candidate must be put into the SLP Clinical Fellow (100910) job. Clinical Fellowship (CFY) is the transition between student and independent which involves an SLP supervisor, hour requirements and mentoring.

As a Home Health Speech-LanguagePathologist, here's a big-picture view of what you'll do:
Provide in house speech therapy to patients having communication, swallowing or cognitive difficulties due to an illness or injury
Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and OT, focused on the patient's plan of care and established goals
Observe and evaluate a patient's ability to communicate, swallow and process information
Conduct patient assessments, document progress and ensure patient goals are met
Assist with mouth, breathing and swallowing exercises, fluency, articulation and reading

Prepares and submits timely, legible, relevant and sufficient documentation of treatment and skilled intervention provided, as well appropriate reports of a patient's progress toward goals.
Advises and consults with the family and/or other caregivers to promote patient progress and planning for discharge.
Participates in educating the patient and the family, and other caregivers to promote patient progress toward mutually established goals.

General Purpose: The Speech/LanguagePathologist is responsible to assess, plan, implement and evaluate patient care programs designed to help prevent speech, language, cognitive, communication, voice, swallowing, fluency, and other related disorders; and assist patients in reaching their maximum function. Essential Functions:
* Performs age appropriate speech/language pathology assessment and procedures that require substantial specialized knowledge, judgment and skills based upon principles of psychological, biological, physical and social sciences.
* Makes patient care decisions based on individual's educational preparation and experience as a Speech/LanguagePathologist utilizing critical thinking skills, sound judgment and leadership abilities.
* Plans, implements, and evaluates patient care programs through the utilization of appropriate diagnostic and prognostic speech/language pathology tests/procedures based upon physician's orders, evaluation of findings, and within the scope of practice as permitted by state law or regulation.
* Communicates appropriate information in a timely manner with other professionals, units, services, agencies and personnel to update the plan of care for the patient, coordinate interventions, or to report adverse responses to treatment in accordance with the contracting employer's policies and procedures.
* Maintains a safe environment for all patients assigned by providing timely, safe and competent therapeutic interventions as specified in the plan of care.
* Participates in conferences, rounds, meetings or equivalent to improve patient care and staff performance as directed by the contracting employer.
* Responds to inquiries and complaints from physicians, patients, family/visitors, colleagues and other health care providers and supervisors.
* Provides professional, respectful and compassionate care to all patients.
* Provides written and verbal health teaching and direction to patients, family, licensed and unlicensed staff as appropriate.
* Establishes effective relationships with patients, physicians, families/visitors, coworkers and superiors.
* Recognizes changing patient conditions and emergency situations and appropriately intervenes.
* Displays a working knowledge of disease pathophysiology, common diagnostic tests and procedures, as well as treatment plans.
* Follows Standard Precautions, infection control and safety guidelines as established by the contracting employer.
* Reads and interprets policy and procedure manuals, written physician orders, operating instructions and safety manuals.
* Understands instructions furnished in written, oral, diagram or schedule form.
* Operates and maintains patient equipment and devices according to operating instructions and policies and procedures of the contracting employer.
* Provides legible, clear written or electronic documentation of all patient care provided, and the patient's response to care.
* May supervise professional and non-professional personnel providing patient care. Provides supervision to others through task assignments, direction, evaluation and feedback of assigned tasks.
* Works with the contracting employer who is also responsible for monitoring work in progress and reevaluating the status of the assigned work at the end of the shift/assignment.
* Completes other assignments as requested and assigned.

Minimum Education & Experience Requirements:
* Master's or doctoral degree in speechlanguage pathology, and who meets either of the following requirements:
* licensed as a speech-languagepathologist by the State in which the individual furnishes such services; or
* In the case of an individual who furnishes services in a State which does not license speech-languagepathologists:
* Has successfully completed 350 clock hours of supervised clinical practicum (or is in the process of accumulating such supervised clinical experience);
* Performed not less than 9 months of supervised full-time speech-language pathology services after obtaining a master's or doctoral degree in speech-language pathology or a related field; and
* Successfully completed an approved national examination in speech-language pathology
* At least one (1) year of Speech/Language Pathology experience within the past five (5) years and as defined by the contracting employer.
* Evidence of current Basic Life Support (BLS) training for infants, children and adults. CPR certification (must achieve within 60 days).
Knowledge, Skills & Abilities Required:
* Able to effectively communicate with all levels of the work force, both clinical and non-clinical.

As a Home Health Speech-LanguagePathologist, here's a big-picture view of what you'll do:
Assist the physician or other licensed independent practitioner in evaluating the patient's level of communication deficit, hearing and swallowing function, as well as in developing the resulting plan of care/treatment including appropriate long term treatment goals, selection of interventions, and the intensity, frequency and duration of services to meet the needs of an individual patient.
Completes initial and ongoing comprehensive assessments at appropriate time points as assigned.
Selects and teaches task-oriented therapeutic activities designed to improve comprehension and production of language in oral, signed or written modalities; speech and voice production; auditory training; speech reading; and communication strategies using multi-modal training (e.g. visual, auditory-visual, and tactile).
Regularly assess changes in the patient's status since the last visit/treatment day, and determine whether the planned services should be modified, performing a re-evaluation and obtaining revision orders from the physician as indicated.
Prepares and submits timely, legible, relevant and sufficient documentation - whether written or electronic - of treatment and skilled intervention provided, as well appropriate reports of a patient's progress toward goals in accordance with professional standards of practice, policy and procedures, and payor requirements.
Advises and consults with the family and/or other caregivers to promote patient progress and planning for discharge.
Participates in educating the patient and the family, and other caregivers to promote patient progress toward mutually established goals.
Actively effectively communicates with other members of the interdisciplinary healthcare team providing care in order to promote coordination of patient care and planning for discharge.
Visually and auditorially observes and assesses the patient.
Participates in quality and performance improvement measures
Participates in in-service training, as requested.
Completes other assignments as requested and assigned.
