Your key responsibilities:
Conduct clinical reviews of various forms of medical documentation and records obtained from providers and facilities including but not limited to medical charts, patient account records, and member interviews.

Weekends/holidays are common workdays as claimants are more likely to be active.
This is an independent role often requiring long hours alone in your vehicle, regardless of weather conditions.
Must remain alert with no external distractions, ready to use videography equipment to document subjects.

Duties Description ·Conduct audits and examinations of taxpayer's books and records to determine compliance with the various articles of the NYS Tax Law;
* Develop and maintain working knowledge of the Tax Law, Criminal Tax and Penal Law as well as Department and Criminal Investigations Division (CID) procedures and policies, including the Code of Conduct, Ethics practices and security and confidentiality rules;
* Communicate effectively, both verbally and in writing, with supervisors and other Department staff, taxpayers and/or their representatives, and prosecutor offices;
* Develop and maintain skills in computer technology including operation of laptop computers, PCs, and various software and job-related applications;
* Travel frequently, including both local and overnight within and outside New York State and to locations not accessible by public transportation; and be able to perform field works in non-business hours if necessary;
* Carry computer equipment and case materials to various field audit sites, training classes, BCMS conferences, etc.
* Prepare cases for prosecution and assist prosecuting authority;
* Represent CID and advocate disputed cases at BCMS conferences and Small Claims
* Hearings; may participate at hearings as a witness for the Department to assist attorneys from the Department's Office of Counsel in the resolution of disagreed cases; testify in Grand Jury and Trial in criminal courts.
Conduct interviews of witnesses, taxpayers, or subjects under investigation with an investigative team;
* Perform various special assignments and miscellaneous tasks as requested; and
* Adhere to the Department's time and attendance rules and regulations.
Minimum Qualifications The ability to fill this position is subject to DOB guidelines for processing personnel transactions. Note: a budget waiver is not
required to fill positions via transfer to lateral or downward level positions within the same department or to change line items
within the same funding source; or to replace employees who have been terminated during their probationary periods. The
selected candidate must be eligible for appointment in accordance with the provisions of the Civil Service Law, Rules, and
Regulations. If mandatory rehiring lists exist for this title, appointment other than reassignment must be made from these lists.

Job Description
Provide quality incident investigations for all allegations within the agency. Ensures that appropriate recommendations are made to safeguard individuals and provide the highest quality of services. Essential Job Functions:
Maintains current and thorough knowledge of incident reporting regulations (14NYCRR Part 624 and Part 625) as well as NYS Justice Center/OPWDD directives and guidance regarding reporting, investigation and monitoring of incidents.
Completes OPWDD 147 Form and Immediate Response Manager Report as assigned.
Completes thorough and quality investigations (and all required forms) as assigned within regulatory and agency guidelines and timeframes.
Monitors and tracks incident follow-up for assigned investigations ensuring all documentation and corrective actions are completed by required due date.
Uploads assigned incident investigations into IRMA
Maintains investigation electronic folders.
Meets with supervisor as scheduled to review status of incident investigations, follow-up and performance.
Informs supervisor of any significant investigation concerns as they arise.
Completes Notable Event investigations as assigned. Ensures all follow up and corrective actions are addressed.
Reviews Event (notable/internal) program investigations as assigned.
Completes investigation review tool for all assigned investigations prior to submission.
Completes monthly incident site review as assigned.
Provides training to employees in the areas of abuse prevention, incident reporting, and reporting of Notable/Internal Events, as assigned.
Performs other duties relevant to the position as requested by the supervisor.
Advises direct supervisor of any obstacles to achieving responsibilities.
Knowledge, Skills, and Abilities
Associates Degree, Bachelor's Degree preferred.
Knowledge of OPWDD Part 624 Regulations, preferred.
Previous experience in Quality Assurance/Quality Improvement within OPWDD, OMH, DOH, OASAS, or OCFS preferred.

Job Description: Surveillance Investigators are the frontline of the private investigations industry. The primary responsibility of the Investigator is to conduct mobile surveillance of an individual that has an on-going insurance claim. Required Key Skills and Qualifications:
Bachelor's Degree in Human Resources, Healthcare Administration, Law or other relevant field preferred plus 5 years' experience in compliance, employee relations, workplace investigations or as a paralegal; or
Minimum of an associate's degree plus 10 years' experience required in Human Resources, Healthcare Administration or other relevant field including at least 5 years' experience in compliance, employee relations workplace investigations or as a paralegal.

Duties Description The NYS DOCCS Office of Special Investigations (OSI) is seeking qualified candidates for consideration to serve as a Senior Investigator in one of its five divisions with cross-over and collaboration among divisions. The (new) Public Integrity Division (PID) investigates a wide range of allegations of violations of the Department's policies and criminal laws, including staff misconduct, particularly physical and sexual abuse of incarcerated individuals; unattended deaths of incarcerated individuals; unauthorized relationships between staff and incarcerated individuals or parolees; various forms of harassment or other misconduct; theft and fraud; and escape plots. The Criminal Investigations Division (CID) investigates a range of criminal activities associated with NYS DOCCS, including but not limited to the following: violent acts resulting in serious injury or death, threats of violence, gun and drug distribution and possession, contraband smuggling, and criminal organizations engaged in extremist violence or terrorist activities. CID participates in several task forces and intelligence centers. The Canine Division has specially trained canines to detect dangerous drugs and contraband electronics (e.g., cellular phones) and is dedicated to interdicting controlled substances and other contraband. The Fugitive Investigations Division (FID) specializes in fugitive apprehensions and returning fugitives to New York. The Intake, Training, and Analysis Division (ITAD) is responsible for the processing of thousands of complaints annually, the training of OSI staff, and performing analysis of data to identify potential issues and prioritize resource allocation and provide OSI with a tactical, case by case analytical role, providing investigators with direction and support in their investigations.
Minimum Qualifications The NYS Department of Corrections and Community Supervision Office of Special Investigations (DOCCS OSI) is seeking qualified candidates to be considered for multiple openings across New York State as a Senior Investigator in one of OSI's divisions. The OSI division and office location would be determined based upon the needs of the Department and OSI.
A Senior Investigator is responsible for supervising approximately five to eight Investigators conducting investigations pertaining the subject matter of the assigned division within the assigned geographic region. A Senior Investigator is responsible to guide Investigators with their caseloads, review reports and investigative activities, and lead small groups conducting investigations. Senior Investigators also independently conduct investigative activities, and work well with superiors, peers, subordinates, facility staff, and all others. A Senior Investigator will ensure that OSI investigations are thorough, timely, and fair, as well as compliant with all relevant policies and laws. Senior Investigators also work with Investigators on special operations, such as surprise inspections, parole absconder details, and monitoring operations. The Senior Investigator will report directly to an Assistant Deputy Chief Investigator, who is reportable to the Deputy Chief Investigator for the division.
The minimum qualifications include at least four years investigative or law enforcement experience, as well as demonstrated leadership capabilities. OSI may accept relevant advanced educational degrees (i.e., above high school or GED) as a substitute for investigative experience on a two-years education for one-year investigative experience basis for these positions. For example, an associate degree (2 years) in criminal justice and forensic science can be substituted for one year of investigative experience, and a bachelor's degree (4 years) in a pre-law and criminal justice can be substituted for two years' investigative experience. Candidates should also have exhibited demonstrated leadership qualities. Knowledge of the subject matter is beneficial but not required. OSI investigative staff must possess, at a minimum, knowledge of the applicable laws, the highest levels of behavior including personal integrity and ethics; fairness; character; judgment; attitude; and respect for and ability to work with others inside and outside OSI. Appropriate licensure and certifications (including being or becoming a peace officer qualified to carry a firearm) are also required. The ability to travel is also necessary.
