This position provides both direct patient care in a primary care office and works with care delivery providers to identify gaps in care, contacts patients to schedule required care, and provides referral follow up. The Medical Home Medical Assistant provides pre-visit planning for the practice's patient panel, coordinates messages through electronic portals, and assists in managing transitions of care. The Medical Home Medical Assistant will act as a clinical liaison to the physician care plan and actively communicate with patients. The ideal candidate will be a compassionate, detail-oriented professional who provides high-quality clinical and administrative support to ensure the best possible patient experience. * Assist providers with patient examinations, procedures, and treatments. * Obtain and record vital signs, medical histories, and patient information in the electronic medical record (EMR). * Administer medications and immunizations as directed by the provider. * Prepare exam rooms, sterilize instruments, and maintain clinical supplies. * Perform basic laboratory tests and specimen collection. * Schedule appointments, manage patient flow, and provide patient education. * Ensure compliance with clinic policies, OSHA, HIPAA, and other healthcare regulations. * Collaborate with providers, nurses, and administrative staff to deliver coordinated patient care.
