Program Assistant Job in New York, NY
Located in the heart of the community of Washington Heights in Northern Manhattan is a 705-bed rehabilitation and nursing care facility with a mission to provide quality care through diverse programs and services designed to support health and independence to individuals needing short-stay or long term inpatient care.
This individual, working closely with the Program Manager, is responsible to coordinate scheduled activities with paid health and wellness and other consultants. They will assist with member outreach and the physical set up of weekly health and wellness activities within walking distance from the office. The position also includes working at a desk, assisting with day to day office management and providing support to the team to ensure accurate electronic record management. It includes direct contact with older adults to identify their needs and requests and link them with the team.
Ensures that Client Intake information and other electronic client records in the NYC Department for the Aging case management platform called Peerplace are completed in the electronic record and are up to date.
Ensures that client records are properly categorized and not duplicated, inputs and updates client information by speaking with clients or staff, and inactivates client records when appropriate. Reviews staff entries monthly, for accuracy of detailed information such as funding source, time entries, and service categories.
Creates processes so staff can easily see their due dates for annual Client Assessments, Health Indicator and Wellness Surveys, as well as other DFTA required client evaluations. Maintains and updates NORC resident and communication platform rosters on an ongoing basis.
Enters NORC group activities and attendance in PeerPlace STARS. Creates STARS reports as needed by Program Manager.
Assists the Program Manager to coordinate NORC health promotion events, such as educational workshops, health fairs, screenings and other special programs, activities and events for NORC residents. Preparations may include creative planning, scheduling, promotion, equipment and room set up. Collaborates with other organizations on partnership activities.
Makes purchases, enters expenditures, and closes out each purchase with the Finance office.
Implements health programs that are evidence-based for NORC members at a minimum of twice a year and may be asked to attend evidence-based training within their comfort level to implement a program. This may involve leading a group of older adults in activities and physically demonstrating movements at times.
Assists the Program Manager with creation of monthly NORC activities calendar, outreach fliers and communication, notifications and reminder calls to older adults, time permitting. Updates NORC website regularly.
In conjunction with the NORC team provides telephone and walk in client engagement, information, and support as appropriate.
Participates in other NORC program related functions along with team and performs other related duties as assigned.
Education:
BA/BS in a health related field such as Health Sciences, Education or Promotion, Sports and Exercise, Public Health, or Nursing. Associates degree with equivalent experience may also be considered.
Experience:
Previous paid or volunteer work experience leading group recreational activities or with older adults
Experience navigating database software
Certification and Licensure:
None
Special Skills:
Mature individual dedicated to working with older adults of varied backgrounds
Excellent written and oral communication in English and Spanish and willingness to speak with older adults in group settings
Demonstration of warmth and kindness in engaging older adults
Attention to detail is required due to the individual's responsibility for efficacy of electronic and other documentation data
Advanced computer skills; proficiency in MS EXCEL spreadsheets and MS Publisher (or willingness to learn) for the creation of outreach materials
Motivation to develop leadership skills
Interest in planning activities using a creative approach
