Summary Reporting to the Program Director/Director of Social Services/Social Services Supervisor, the Licensed Practical Nurse (LPN) provides professional and clinical nursing care to clients in their residences/programs. The LPN assesses, develops and implements nursing care needs and case management services to clients. Responsibilities Program/Facilities Advocates for clients regarding plans for medical and psychiatric care. Assists with intake, medical update and documents medical data in client database system(s). Documents client progress and reports significant changes in an accurate and timely manner in client database system(s). Monitors client behavior, medical and mental health well-being, and medication intake. Coordinates patient care with program staff, care coordinators and external providers including medical, mental health, and community resources. Assesses, collaborates and coordinates discharge planning needs and readiness for discharge with other members of the health care team as required by Urban Pathways and funding and regulatory agency policies and procedures. Monitors client self-administration of medication in accordance with Urban Pathways policies and procedures. Works closely with program management to manage crisis situations. Reports incidents within 24 hours. Communicates with medical/mental health providers and program management regarding changes in client condition and/or significant health related events such as post-operative complications and medication errors. Assesses and communicates suicidal/homicidal risk factors and ideations. Ensures that client care is compliant with Urban Pathways, funding and regulatory agency policies and procedures. Facilitates wellness self-management groups as needed. Administrative Maintains and submits all necessary documentation and reports accurately, in a timely manner and as required by Urban Pathways, regulatory and funding agency policies, and procedures. Enters progress notes after each client meeting within 48 hours and/or as mandated by Program. Completes Intake/Admission documentation within 24 hours of client admission, as mandated by Program. Documents assessments in client records within required timeframe (not applicable for Outreach Program). Updates information in client records, as required. Community Relations Develops and maintains relationships and coordinates client care with medical, mental health, community, and public services and resources. Communication Communicates verbally and in writing in a professional manner at all times. Facilitates and participates in meetings to report on clients' progress, share educational resources and discuss clients' care and goals. Attends staff meetings, trainings, and supervisory sessions, as required. Responds to all communications including telephone, email and other oral and written inquiries in a timely manner. Performs all other duties as assigned and as required. Qualifications Education and Experience Graduate of an accredited school of Practical Nursing required. Minimum of one year of medical/ nursing experience in an acute care hospital a plus. Experience with homeless, mentally-ill and substance abuse population preferred. Skills Strong written and verbal communication skills. Strong computer skills and advanced knowledge of Microsoft Office (e.g. Outlook, Word, Excel, etc.). Knowledge of client database system(s). Excellent organizational skills and ability to manage multiple priorities. Strong assessment skills. Strong problem-solving skills and resourcefulness. Foreign languages a plus. Licenses Valid NYS Licensed Practical Nurse required. Valid driver's license with clean driving a plus. Other Required to work a flexible schedule including weekends, holidays and evenings. Physical Requirements All programs require field work, client escorts and use of public transportation. Ability to sit, stand, walk, bend, stoop, push, pull, lift (up to 25 lbs.), type, write, read, speak, see, and hear.

Responsibilities: Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatrist will be matched with the population that most closely meets their clinical expertise, desired work location and schedule and the clinical needs of the service. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs. Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information. Evaluate patients for the need for a higher level of care, including hospitalization, and refer to a higher level of care as clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care. Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. North Infirmary Command (NIC), West Facility (WF), and the Contagious Disease Unit (CDU) are the jail facilities that house a significant number of incarcerated individuals with advanced age, complex medical problems, disabilities, and/or enhanced nursing needs. The Senior CL Psychiatrist working in this facility will lead the team in diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for individuals housed in this facility with comorbid mental illness, including direct patient care as well as supervision of other psychiatric providers. The Senior CL Psychiatrist will also consult on the care of medically complex patients in other jail facilities. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. Responsibilities Include: Diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for elderly and medically complex patients with comorbid mental illness. Directly treat and oversee the psychiatric care of additional patients housed in NIC/WF/CDU Direct patients to the appropriate level of mental health care, within the facility or in the hospital as needed. Close communication with psychiatric and medical providers at Bellevue Hospital to ensure continuity of care as patients transition between the two facilities. The Supervising Psychiatrist is a psychiatric leader of a facility, clinical area, or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Senior Psychiatrist (if applicable) to ensure that a mission-driven, high quality, and innovative standard of psychiatric care is provided to patients with varying levels of mental health needs. In collaboration with the Mental Health Unit Chief, oversee the orientation of new staff to the facility or program. Conduct regular and multi-disciplinary case conferences. Report on an as-needed basis to the CHS Morbidity and Mortality review committee. Work in collaboration with the Mental Health Unit Chief to manage clinical schedules, vacation requests, CME activities and other relevant activities as they pertain to psychiatric staffing and education. Participate in policy revision and creation, and practice guideline creations, related to psychiatric care, including input into the psychiatric medication formulary. Lead at least monthly interdisciplinary mental health staff meetings in coordination with the Mental Health Unit Chief and Clinical Supervisor. Personnel management of clinical staff, including setting quality standards, establishing effective recruitment and retention strategies, and managing workforce complaints. At least 30% of time devoted to direct clinical care, which may include direct supervision of psychiatric trainees. For smaller facilities or programs with less supervisory need, more direct patient care will be expected. On-call responsibilities for two weeks at a time, one to times per year, for off-hours issues that require high level clinical intervention. Other duties at the discretion of the Medical Director of Mental Health or the Chief of Service. Minimum Qualifications Applicants must have all of the following: 1. Graduation from a medical school approved by the Council on Medical Education and Hospitals of the American Medical Association. 2. Completion of an acceptable internship in an approved hospital. 3. A license to practice medicine in the State of New York. 4. Certification by American Board in field of specialization or a faculty appointment of Associate Professor or higher in a medical school recognized by the American Medical Association. 5.
