Utilizes the software systems for service plans, client admissions and discharges, scheduling support, client billing and Associate payroll. Participate in developing, maintaining, and updating written policies and procedures and standards which guide the services provided by the Home Services Associates. Participate in developing, maintaining and implementing the Quality Improvement Programs for Home Services by utilizing input from clients, Care Partners, family and others and evaluating systems and service delivery. Provide client services as needed to both residents and wait list members as deemed necessary. Maintain contact with assigned members and/or their families based on the applicable member contact schedule and follow-up as appropriate including advocacy and health education for members. Using current clinical screening tools and assessments (including member self-health assessments), the Care Coordinator will determine members' appropriateness or eligibility for: a) preventive care initiatives; b) community resources; c) other insurances such as Medicare and long-term care insurance, if applicable Encourage members in accessing resources to help achieve their health and wellness goals.
