You are a compassionate and dependable Home Services Assistant who is committed to supporting residents in their daily routines. You respond quickly to needs, foster meaningful interactions, and bring compassion and professionalism to every interaction in our communities. Responsibilities
Coordinates services provided by Home Service Assistants to meet client's personalized needs
Performs services to meet client's personalized needs
Supports Home Service Assistants to maintain program policies and procedures
Provides administrative support, including (but not limited to):
Scheduling and coordinating home services with clients and associates (including after normal business hours)
Answering phone calls, including emergency (including after normal business hours)
Perform data entry into software systems
Performing monthly billing, completing and submitting insurance paperwork
Generating necessary reports
Ordering and maintaining supplies
Filling medical data and other pertinent information
Maintaining records for Home Services
Requirements
High School Diploma or equivalent preferred
Previous leadership experience
Previous experience in Home Services position, or equivalent
Good computer skills, including use of Microsoft Office and data entry
Must possess a customer service background and orientation
Valid Ohio driver's license required

Requirements
CNA certification preferred.
2 years of experience in social or recreational programming in a health care setting working with residents with Dementia preferred.
Strong communication, interpersonal and organizational skills with demonstrated creativity.
Proficient in Microsoft Office Suite.

This role will provide onsite support as the Enhanced Service offerings of Care Coordination and Home Services begin at Concord Reserve. This individual will be an LPN who will play an integral part of campus associate relationship building as resident needs are identified. Manage care and services provided by Associates and ancillary staff working in Life Enriching Home Services. This management extends to any LEC (Life Enriching Communities) campus where Home Services are being delivered, and includes coordination and delivery of Associate onboarding, development, and delivery of Associate education to meet any applicable educational requirements, and ongoing evaluation of Associate performance.
Develops, coordinates, and implements service plan by using a collaborative approach to identify the client's wishes and abilities for optimum level of functioning and maintenance of dignity, independence, and confidence.
Utilizes the software systems for service plans, client admissions and discharges, scheduling support, client billing and Associate payroll.
Participate in developing, maintaining, and updating written policies and procedures and standards which guide the services provided by the Home Services Associates. Participate in developing, maintaining and implementing the Quality Improvement Programs for Home Services by utilizing input from clients, Care Partners, family and others and evaluating systems and service delivery.
Provide client services as needed to both residents and wait list members as deemed necessary.
Maintain contact with assigned members and/or their families based on the applicable member contact schedule and follow-up as appropriate including advocacy and health education for members.
Using current clinical screening tools and assessments (including member self-health assessments), the Care Coordinator will determine members' appropriateness or eligibility for: a) preventive care initiatives; b) community resources; c) other insurances such as Medicare and long-term care insurance, if applicable
Encourage members in accessing resources to help achieve their health and wellness goals. Resources may include the Connection, online resources or specially developed materials from various community health and wellness resources.
Requirements
Degree in allied health or nursing
Must have a minimum of 1 year experience in providing community-based services
Must have a minimum of 2 years experience managing or supervising care
Demonstrates the ability to problem solve effectively
Able to keep calm in emergency and stressful situations

Lead Vespers, Independent Living Bible Study weekly, pastoral care, and visitations as needed
Requirements
Individuals who exemplify kindness and positivity

Current Master's Degree of Divinity, and Ministry Ordination, preferred
Minimum of 2 years serving a congregation, preferably in a senior community or health care setting. Minimum of 1 unit CPE, or ability to achieve within first 18 months.
Strong knowledge of Advanced Directives demonstrated, and well as familiarity with end-of-life processes and care.
Board Certified Chaplaincy preferred
Microsoft Office proficiency. AOD experience a plus

What Is Your Role?
The Director of Nursing (DON) provides leadership in developing and implementing standards, goals, and objectives for resident care. This role oversees all aspects of the health services program, ensuring compliance with federal, state, and local guidelines while consistently promoting exceptional quality of care.
In This Role, You Are Responsible For:
Leading all infection control and prevention efforts, including compliance, monitoring, reporting, and training
Driving and overseeing all Quality Assurance and Performance Improvement (QAPI) initiatives
Ensuring compliance with all federal, state, and local regulations for both Long-Term Care (LTC) and Assisted Living/Residential Care Facilities (RCF)
Serving as the clinical lead during surveys and inspections, applying extensive regulatory knowledge to ensure readiness and successful outcomes
Ensuring residents' medical, emotional, psychosocial, and spiritual needs are met, and that they are always at the appropriate level of care
Collaborating with interdisciplinary teams, residents, and families to deliver person-centered care
Monitoring clinical outcomes and initiating interventions for quality improvement
Supporting professional development of nursing staff through training, mentorship, and performance management
Maintaining accurate documentation in accordance with organizational policies and compliance standards
We Are Seeking:
A current RN license in the state of Ohio; BSN preferred
Diploma from an accredited nursing school
Current CPR certification
Previous nursing leadership experience in a continuing care retirement community, hospital, or similar environment
Proven ability to mentor, coach, and develop team members

Strong customer service orientation and problem-solving skills

The Resident Services Director leads and elevates the overall resident experience.
