Visit us at *************** You will be responsible for managing high-quality Perinatal Case Management (PCM) to clients that support, enhance, and coordinate the health and other services recommended for mothers and babies. The role includes assessment, goal setting, care coordination, crisis management, coordinating process improvements and executing weekly standing tasks. You'll establish and maintain strong working relationships with peers and partners and provide regular status updates, summarizing issues to key points and actions required.
