The Navigation Housing Case Manager provides case management services for Recovery Oriented Housing Program (ROHP) participants. ROHP participants are identified to be currently experiencing or at risk of chronic homelessness, high utilizers of publicly funded systems, possibly disengaged from services and in need of diverse supportive services. This role is tasked with identifying all types of barriers which may complicate a participant's ability to obtain and maintain housing, and use creative problem solving to address these issues. This Case Management position includes broad case review, scheduling and outreach, and various other supports to monitor client progress and ensure that ROHP outcome measures are met. This position works closely with Navigation's Housing Specialists, Facilities, therapists and prescribers, to identify and overcome barriers which complicate a resident's ability to remain housed, keep files compliant, and is a member of our Clinical Housing team. Essential Duties/Responsibilities: * Provides case management to voucher residents to include community-based services when needed, and will carry an average caseload of 20 clients. * Conducts outreach and engagement to residents identified as needing support to maintain housing. * Identifies potential barriers to maintaining housing and engages residents in goal setting, monitoring progress. Actively coordinates care with Jefferson Center, and other community agencies and health providers. Maintains appropriate professional standards and provides appropriate follow-up for consumers. * Identifies risk factors including lethality, and responds accordingly. * Provides needs assessment for living expenses and other needs such as food, clothing, etc., in addition to life skills education. * Responsible for the coordination and/or completion of needed paperwork including releases of information, HMIS intake and assessment, VI-SPDAT assessments, Homeless Prevention Demographics Profiles, and other needed documentation. * Maintains a positive focus and excellent customer service skills towards consumers, Jefferson Center staff, and the community. Works from a strength based, recovery based perspective and takes cultural considerations into account when appropriate. Effectively responds to client/consumer needs and problems, initiates and maintains positive interactions, timely response to phone calls, email and other requests. * Productivity/Documentation and Outcome Data: Documents all contacts according to Jefferson Center and Substance Use Outpatient Program standards while meeting productivity standards as identified by the Program Director. Other Duties: * Exhibits enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment. * Maintains effective interpersonal relationships with consumers, peers, subordinates, upper management, visitors and the general public. * Ability to multi-task, prioritize concerns, and effectively manage time independently. * Attends mandatory in-services, compliance with individualized training plan if required. * Participates in supervision by coming prepared with an agenda. Reports high risk/problem cases, and utilizes a problem solving approach as well as feedback. Attends supervision at times and intervals agreed upon with supervisor. * Submits 90% of all SRFs/Progress-to-Date forms within 3 working days (team specific). * Completes 95% of all paperwork by the due date. * Meets required number of consumer service hours or other measures of productivity as determined by Program Director. * Works cooperatively with other community agencies, as appropriate, and in agreement with supervisor. Volunteers to work on Center committees and/or task forces. * Participate in Evidenced Based Practices and other agency initiatives including Trauma Informed Care, Whole Health, Motivational Interviewing, Solution-Focused as well as other best practices. The Admissions Specialist is responsible for reviewing and coordinating admissions for Jefferson Center's Children's Crisis Stabilization Unit (CSU). They implement efficient admissions procedures and processes that are customer friendly to children/youth, parents/guardians, and the referral source. They will interact with both outside agencies and with team members with diplomacy, respect and professionalism. We are looking for day and night shift coverage. Day shift: 7am to 7pm. Night shift 7pm to 7am. Jefferson Center's Children's Crisis Stabilization Unit (CSU)'s mission is to use an interdisciplinary team of clinicians, residential counselors, medical staff and admissions staff to provide brief, 3-5 day, crisis stabilization treatment for children and youth with behavioral health issues, including depression, suicidal thoughts, other trauma reactions, and family issues. Our team coordinates treatment and provides tools and safety planning for children/youth and families so that they can safely return to their homes and the community. We are committed to serving Colorado's children/youth while maintaining a non-judgmental and inclusive approach. Essential Duties: Monitor and coordinate timely admission-decisions and admissions of children/youth for the Crisis Stabilization Unit to promote safe and appropriate admissions while maximizing capacity. Admissions decisions should be made and communicated to the referral source in 30 minutes or less, except for cases with unusual complexity. Greet clients and visitors upon arrival, sign them in, obtain insurance information and any other information needed, process payments for copays & facilitate signing of consents. Answer multi-line phones and direct caller to the appropriate party or handle the caller's needs directly, demonstrating courteous and professional communication and prioritizing confidentiality. Serve as admissions liaison between all stakeholders to facilitate efficient, professional communication and problem-solving activities. Acquire pre-authorization and collect insurance information, including verifying Medicaid Eligibility. Enter incoming child/youth's information into the electronic medical records (EMR.) Also provide accurate and detailed billing information in EMR, to include documenting primary and secondary insurance information. Schedule transport for new clients, if needed. Assemble client's chart and distribute intake information to key individuals. Accurately and concisely complete the Census Report for the end of each shift. Provide ongoing feedback and information on the status of admissions to clinical team members to coordinate intake and admission efforts. Maintain strong customer service including supporting children/youths and parents who may be in crisis with assistance from other JH staff members. identify areas for improvement of the admission process, and communicate these to the Admissions Supervisor Successfully completes all required training in a timely manner & attends key internal meetings as assigned. Follow and enforce all Jefferson Centers' Policies and Procedures, including but not limited to confidentiality, sexual harassment/harassment, dual relationships, physical and sexual abuse reporting, Corporate Compliance and Code of Ethics. The Manager of the Crisis Stabilization Unit (CSU) is a full-time onsite position and will provide the management and direction for both clinical and administrative functions of the Crisis Stabilization Unit. The Manager is responsible for clinical and evidence-based program/milieu planning and operation, customer service, standards and contract compliance and monitoring, and the development of applicable policies and procedures. The Manager also assists with the fiscal accountability of the program. CSU's mission is to use an interdisciplinary team of clinicians, residential counselors, medical staff and admissions staff to provide brief, 3-5 day, crisis stabilization treatment for children and youth with behavioral health issues, including depression, suicidal thoughts, and other trauma reactions. Our team coordinates treatment and provides tools and safety planning for children/youth and families so that they can safely return to their homes and the community. Education, Knowledge, Skills & Experience Required: Master's degree in psychology, counseling, social work, or related field required. Clinical License Required: LCSW, LPC, LMFT, or LP. Five or more years' related experience in the field with children/youth, preferably in a crisis or acute behavioral health setting required. Two years of supervisory experience required. Experience in crisis stabilization or inpatient settings, and/or integrated behavioral health, and/or community mental health setting preferred. Significant knowledge of evidence-based treatment modalities that pertain to youth and behavioral health treatment and familiarity with a CSU model of treatment. Achieves and maintains certification as a Qualified Medication Administration Personnel (QMAP). Acquires and maintains certification in Nonviolent Crisis Prevention and Intervention Training (CPI). Bilingual (English/Spanish) Preferred. Essential Duties/Responsibilities: Responsible for the supervision and oversight of the Program Coordinator. Participates in an on-call rotation for the CSU, in collaboration with the Program Coordinator. Responsible for the overall clinical and administrative supervision of administrative, clinical and program/milieu staff. Provides for the management and coordination of performance evaluations of applicable staff. Supervises licensed clinicians, clinical coordinators, interns, shift leads, admissions specialists, and residential counselors, with support from the Program Coordinator. Responsible for the scheduling of all staff in their respective departments and communicating schedules to coordinate the smooth operation of the program. Scheduling must comply with State regulations regarding staff to child/youth ratios, prioritizing safety, quality of programming, and resources, including budget. Works in coordination with the supervisors of the medical department to establish and maintain a positive, effective, and efficient team. Works with the Program Coordinator to develop therapeutic and evidence-based milieu programming. Provides and arranges for training of staff, as well as working with other departments for specific training appropriate to different positions. This includes creating and working with other departments to create a trauma-informed, safe environment for treatment and safety planning back to the community to be maximized. Ensures all training is in compliance with CDHS Volume 7 rule and BHA rule. Completion and reviewing of all necessary documentation, including Critical Incidents, Unusual Incidents, PQI Reports, Corporate Compliance Reports, Medication Occurrences, and accreditation documents. Will be trained in TRAILs and ensure compliance with reporting requirements to TRAILs. Responsible for compliance with regulatory agencies and referral sources (CDHS, BHA, payers, and all applicable federal, state, and local laws). Oversees administrative policy and procedures, analyzes the same and makes recommendations for improvement. Ensures that proper financial procedures are followed by staff. Monitors revenue generated and expenditures of the department. Monitors staff productivity, and other measures of performance. Participates in development of systems, which provide needed program management information for inclusion in the organization's Performance Quality Improvement Plan. Participates positively and constructively with the Leadership Team, by attending meetings and contributing needed information and ideas which will assure ongoing improvement in the Crisis Stabilization Unit. Ensures excellent customer service and satisfaction with all stakeholders. Monitors ongoing program functioning and makes progress reports as required. Has a physical presence on the unit and monitors and insures child/youth and staff safety. Willingness to step into other roles (therapist, milieu staff, intake coordinator) where assistance is needed. Effectively responds to the client/consumer needs and problems, initiates and maintains positive interactions with colleagues and community partners, and exhibits timely response to phone calls, email and other requests. Oversee and ensure effective and efficient processes and workflows for discharge and transition of care to community resources. Commitment to build a strong collaborative partnership with other clinical managers located at the Zang Site and build cohesion amongst all teams located at Zang.
